Nasopharyngeal carcinoma with spinal secondaries.

Nasopharyngeal carcinoma has been reported to have a higher incidence of distant metastases as compared to other head and neck cancers. Distant metastases of nasopharyngeal carcinoma has been reported in the bones, lungs, liver, distant lymph nodes, brain and porta hepatis. This report presents a case of nasopharyngeal carcinoma with secondaries in the extradural space at the thoracic level of the spinal cord which hitherto has been an unreported site.

The way we teach otolaryngology in an integrated problem-based curriculum.

The University Science Malaysia, Penang, the third medical school in Malaysia, is following an integrated curriculum. In this curriculum, the students are taught the subject of otolaryngology from the second year of their course unlike the traditional schools where they are taught in the fourth or fifth year. This paper describes how otolaryngology is introduced in this integrated, problem-based curriculum.

Association of rhinitis in adult asthmatic.

In a study of 124 adult patients with bronchial asthma, 65% of them had associated rhinitis. In the asthmatics who had associated rhinitis, both diseases usually started within two years of one another but either disease might develop first. In 21% of the patients, asthmatic attacks were preceded or precipitated by rhinitis symptoms. In the patients who had asthma alone or those associated with rhinitis, no significant difference were found in terms of age and sex distribution, age of onset, and a positive family history of asthma, rhinitis or allergic diseases. Response to skin prick test using six different types of allergens also showed no difference in the two groups of patients. Sensitivity to house dust was common among both groups of patients as well as in the normal controls suggesting a common occurrence of house dust mite in our community and making the skin prick test using this allergen unsuitable as a test for atopy in our population.

Ectopic tooth in the roof of the maxillary sinus.

Eruption of a tooth into a nonoral environs is rare. Ectopic eruption of the tooth into the nasal cavity and chin has been reported before. This is a report of an ectopic third molar tooth in the roof of the maxillary sinus, which has not, to our knowledge, been reported before.

Nasopharyngeal carcinoma with secondaries at the porta hepatis presenting as obstructive jaundice.

Recent reports have dispelled the previously held concept that head and neck cancer rarely metastases beyond the cervical lymph nodes. Nasopharyngeal cancer has been reported to have a higher incidence of distant metastases compared to other head and neck cancers, the common sites being bone, lung and liver. A case of nasopharyngeal carcinoma presenting as obstructive jaundice because of secondaries at the porta hepatis is presented here.

Orbital complications of acute sinusitis.

Orbital complications of sinusitis may occur either by spread along the venous system or by direct extension of infection between contiguous structures. The association between sinusitis and inflammation of the orbits has been known for centuries. But in the post antibiotic era there is a marked decline in the number of case reports in the literature. This paper presents 10 cases of sinusitis who had orbital complications. The classification of this disease complex as given by Chandler helps in planning the treatment. Groups I and II respond very well to antibiotics but in groups III, IV, V surgical intervention is necessary. Early identification and aggressive therapy reduces the morbidity of this disease.

Oesophageal food bolus impaction.

Impaction of food usually occurs in patients having obstructive lesions of the oesophagus. The food bolus impaction normally occurs in patients older than 60 years of age. Though food bolus impaction in the oesophagus is not uncommon, food bolus in the oesophagus causing complete obstruction resulting in total dysphagia is rare. This is a case report of a woman who had complete obstruction of the oesophagus following impaction by a piece of meat. The various treatment modalities available for such a condition are discussed.

Congo red-mediated regulation of levels of Shigella flexneri 2a membrane proteins.

The ability of Shigella spp. to bind Congo red from agar medium is generally correlated with their virulence properties. We used a metabolically active culture of Shigella flexneri 2a to determine the effect of Congo red on its membrane protein profiles. Virulent S. flexneri grown in the presence of Congo red at 37 degrees C showed increased levels of three proteins with Mrs of 43,000, 58,000, and 63,000 (43K, 58K, and 63K proteins) in the Sarkosyl-soluble membrane fractions. The observed phenomenon was temperature dependent. At 30 or 42 degrees C the protein levels remained unaffected by the presence of Congo red. Similar regulation of the levels of the 43K, 58K, and 63K membrane proteins was also observed with Shigella dysenteriae 1 and enteroinvasive Escherichia coli, but not with enteropathogenic E. coli. The cellular uptake of Congo red seemed to be essential, but not sufficient, for regulation. All three proteins reacted with human convalescent-phase sera in immunoblots of S. flexneri 2a Sarkosyl-soluble membrane fractions. Using the 43K-specific antiserum as the primary antibody, by indirect immunofluorescence studies, we detected an increase in the level of the 43K protein in S. flexneri which had invaded epithelial cells. These observations strongly indicate that the 43K, 58K, and 63K proteins are virulence associated. We propose that the observed regulatory effect of Congo red on membrane proteins of S. flexneri is mediated through induction. Since the same regulatory effect was also observed during the invasion of epithelial cells by S. flexneri, it is suggested that Congo red mimics some host tissue factor in vitro.

Nasal manifestations of yaws.

Yaws is an infectious, non-venereal disease of the tropical countries, which is caused by Treponema pertenue. Gangosa and goundou were seen commonly in cases of yaws in Africa, particularly in the early part of this century. After successful WHO mass treatment campaigns, these conditions are rarely seen now. A case of yaws with gangosa, goundou, and a nasopharyngeal carcinoma is presented here for its rarity.

A study on perennial rhinitis in Kelantan.

Ninety five patients with perennial rhinitis were examined clinically and various investigations were done in order to find out the common allergens and to assess the value of various tests in perennial rhinitis. In this study group 94% of cases were proven to be cases of allergic rhinitis. Cat fur was found to be the commonest allergen. Grass pollen which is a common allergen in European countries was found in only 18% of cases in the present study. X-ray of the paranasal sinuses as a routine investigation was not found to be of much use in perennial rhinitis. There was significant correlation between results for allergens tested by enzyme immunoassay and skin prick test.

A study of health and health related social problems in the geriatric population in a rural area of Tamil Nadu.

About 200 individuals aged 60 years and above, out of 317 total geriatric population of a small village in Tamil Nadu were subjected for complete clinical examinations with the objective of finding major health problems and associated social problems prevalent among these study population. Main causes of illness were found to be arthritis, cataract, bronchitis, skin diseases and malnutrition.

Impact of integration of NLEP into PHC system in Madurai district, Tamil Nadu, and status of integration of leprosy patients into the community.

Based on the encouraging results of the integration in Tamil Nadu, the Government of India introduced integration in the entire country. In this study on integration, Madurai district was taken as the study area. The impact of integration on essential indicators of leprosy elimination was analyzed and the results are discussed so that they will be useful to other states that are still considering integration.
