Surgery for synovial chondromatosis. 26 cases followed up for 6 years.

Surgery was performed in 31 patients with synovial chondromatosis: 12 had synovectomy and removal of loose bodies, and 16 had removal of loose bodies only. The remaining 3 patients had more radical surgery: 2 had total hip replacement and 1 had resection arthroplasty. Twenty-six patients were reviewed after an average of 6 years and 4 months. Thirteen patients were symptomatic and 11 had residual signs of limited joint motion and localized tenderness. In 1 case of metatarsophalangeal joint involvement, recurrence occurred. Removal of loose bodies only did not differ from synovectomy and removal of loose bodies.

Cerebral germinomas and Klinefelter syndrome. A review.

Patients with Klinefelter syndrome appear to be predisposed to the development of extragonadal cerebral germinomas. A case of a pineal region germinoma in a boy 15 years of age with Klinefelter syndrome is documented in this article. In view of three other cases of cerebral germinomas associated with Klinefelter syndrome in the literature, a hypothesis for this predisposition is suggested.

Utility of 24 hour bone scan in evaluation of bone involvement by soft tissue sarcoma.

Accumulation of Tc-99m MDP in soft tissue malignancies occurs frequently. When tumor overlaps adjacent bone structures, the bone margins can be indiscernible on a regular bone scan. Additional views should be taken to differentiate bone from tumor, however, separation is sometimes difficult and not always successful. This report presents a patient in whom accumulation of Tc-99m MDP in a liposarcoma of the thigh disappeared on a 24 hour scan and bone margins were seen clearly. A possible mechanism for this phenomenon and its utility are discussed.

Painful piezogenic pedal papules in patients with Ehlers-Danlos syndrome.

Painful piezogenic pedal papules were observed in 10 of 29 patients with Ehlers-Danlos syndrome. The diagnosis was made by examining each patient while he or she stood with full body weight on the heels and by observing the appearance of the painful papules on the medial, posterior, and lateral aspects of both heels. Biopsy specimens of papules demonstrated a thickened and dense dermis. The subcutaneous fibrous trabeculae were thin, with resultant poor compartmentalization of the fat. Painful piezogenic pedal papules are due to herniation of subcutaneous fat into the dermis, which is possibly due to structural defects of the connective tissue. The high prevalence of painful piezogenic pedal papules in our group of patients (34.5%) makes its direct association with the Ehlers-Danlos syndrome highly feasible, most probably because of the connective tissue defect that occurs in patients with Ehlers-Danlos syndrome.

Cardiovascular complications in the Ehlers-Danlos syndrome with minimal external findings.

Ehlers-Danlos syndrome (EDS) is clinically and genetically a heterogeneous disorder of connective tissue. Eleven different types of EDS have been documented, several of which have major cardiovascular complications as part of their clinical manifestations. The purpose of this report is to call attention to a form of EDS with minimal external features but severe internal vascular complications.

Villonodular synovitis of the shoulder in the elderly.

Villonodular synovitis of the shoulder developed in two elderly patients. Huge soft tissue mass, marked joint effusion, and radiographs demonstrating severe osteoarthritic changes characterized these patients. Localized shoulder muscle atrophy accounts for residual painfree limitation of active shoulder motion following synovectomy of the shoulder joint.

Genetic disorders associated with severe alopecia in children: a report of two unusual cases and a review.

Moderate to severe alopecia in children may be due to a genetic disorder. This paper reviews the heritable causes of alopecia in children and gives a detailed account of two affected unrelated children. One has alopecia universalis while the other has alopecia postulated to be due to a new disorder of genetic etiology. The article concludes that for purposes of genetic counseling and prognosis it is crucial that a correct diagnosis be made.

Dermatomyositis and Hodgkin's disease. Case report and review of the literature.

The association between dermatomyositis and malignant tumors, especially carcinomas, is well documented. However, lymphoreticular malignancies have rarely been reported. A patient in whom Hodgkin's disease was diagnosed 5.5 years after the onset of dermatomyositis is described. The independent course of the two diseases is discussed and previously reported cases of lymphoma and dermatomyositis are reviewed.

Torsion of the ovary: sonographic features.

The sonographic findings in 19 cases of surgically proven torsion of the ovary are described. In all cases the affected ovary was markedly enlarged; in 32% of cases a small amount of fluid was detected in the cul-de-sac. More significant in terms of specificity was the demonstration in 14 (74%) of the 19 cases of small cystic structures scattered around the periphery of the organ. Sonographic demonstration of these structures, presumably prominent follicles, can suggest the preoperative diagnosis of ovarian torsion.
