Magnetic resonance imaging findings of a nonfunctional mediastinal paraganglioma with an unusual presentation.

A case of histologically proven mediastinal paraganglioma presenting with metastatic spread to supraclavicular lymph nodes is presented. Mediastinal paragangliomas are extremely rare tumors and their CT and MR imaging features have not been well documented in the radiologic literature. The T 1-weighted and T 2-weighted spin-echo MR findings of a locally invasive mediastinal paraganglioma with metastasis to the supraclavicular lymph nodes are described and the literature is briefly reviewed.

Radial scar and tubular carcinoma of the breast.

A case of tubular carcinoma within a radial scar pattern is reported. The aim of this study is to support the opinion of the authors who believe that there is a relationship between tubular carcinoma and radial scar. We think that surgical biopsy should be recommended in all cases of stellate lesions detected at mammography.

Overexpression of p53 in laryngeal carcinoma: clinicopathological implications.

The aims of this study were to determine the incidence of p53 overexpression in squamous cell carcinoma of the larynx and to establish whether or not this bore any relationship to survival, location, stage, histological differentiation, nuclear atypia, mitotic activity, stromal desmoplasia, tumor-associated tissue eosinophilia, or stromal lymphoplasmocytic infiltration. Paraffin blocks from 51 cases of laryngeal squamous cell carcinoma with a minimum follow-up period of 36 months were recut and stained immunohistologically with anti-p53 antibody using the streptavidin-biotin technique. Results were compared with clinicopathological features with Kruskal-Wallis and Mann-Whitney tests. Sixty-three percent of tumors showed positive staining for p53, and in addition, 15% of the sections with adjacent normal or dysplastic mucosa showed positive staining. No relationship between p53 staining and prognosis or any other one of the aforementioned clinicopathological parameters was observed. Although p53 overexpression is a common feature in laryngeal carcinomas, it does not seem to have an impact on prognosis and it does not bear any relationship to the aforementioned clinicopathological parameters.

Pituitary abscesses. Report of three cases.

Three cases of pituitary abscess are presented. In spite of improvements in radiological evaluation, preoperative diagnosis of pituitary abscess is quite difficult and definite preoperative diagnosis is rare in the literature. In our three cases, diagnosis was made postoperatively. Pituitary abscesses are associated with high mortality and morbidity. When first suspected, prompt antibiotic therapy should be considered. Early operative drainage seems to be an important factor in decreasing this high mortality and morbidity.

Choroid plexus carcinoma of the lateral ventricle. Report of a case in an adult.

Choroid plexus carcinoma is a rare neoplasm and occurs more frequently in children than in adults. Most of these neoplasms in adults arise in the fourth ventricle. A 22-year-old male with choroid plexus carcinoma presenting in the lateral ventricle is reported. The patient was treated by radiotherapy after surgical removal of the tumour. Modern management possibilities and its limitations are discussed.

Primary gastrointestinal lymphomas in Turkey: a retrospective analysis of clinical features and results of treatment.

UNLABELLED: Thirty-three patients with primary gastrointestinal lymphoma (GIL) followed at Ankara University Medical School have been evaluated. The most frequent locations of the disease are the small intestine (48.4%) and the stomach (39.3%). The intermediate and high grade lymphomas constitute 84.8% of the cases. The mean age of the patients with small intestinal lymphoma is 28.7 years and 47.1 years for those with gastric lymphoma. The patients treated with surgery and chemotherapy (S+CT) have a longer survival than those treated with chemotherapy (CT) alone. IN CONCLUSION: 1) Small intestinal lymphoma occurs more frequently than gastric lymphoma in our study. 2) The median age of the Turkish patients with primary GIL is approximately 10 years less than those in the Western countries. 3) The therapeutic results of S+CT are superior to those of CT in the early stages of the disease.

Human amniotic membrane as an intestinal patch for neomucosal growth in the rabbit model.

This experiment was carried out as a preliminary study, an attempt to grow new intestinal mucosa on human amniotic membrane in the terminal ileum in 37 rabbits. After ketamin sulfate anesthesia at laparatomy, 5-cm ileal defects were patched with human amniotic membrane (5 x 2 cm). These patched intestines were investigated on the first postoperative day and the 2nd, 5th, 10th, and 20th weeks corresponding to 4, 5, 5, 10, and 10 rabbits, respectively. Only three rabbits died in the early postoperative period. There was no evidence of intestinal obstruction or dilatation with barium meal. Microscopically, the neomucosa consisted of a thin layer of columnar epithelial cells at 2 weeks with more maturity of the villi and less irregularity and branching by 20 weeks. All patches were covered with neomucosa commencing at 2 weeks and covering the whole patch area by 20 weeks. This technique's advantages are the large size and the ease of the availability of the human amniotic membrane for neonates at risk without jeopardizing the neonates tissues. It is hoped that this method might be considered when neonatal material is scarce.

Malignant melanoma of the optic nerve head in a case of oculodermal melanocytosis.

Malignant melanoma of the uveal tract, orbit, and brain have been reported to occur in patients with oculodermal melanocytosis. A 60-year-old Caucasian man with oculodermal melanocytosis developed a malignant melanoma of the optic nerve head in the left eye. This case is the first reported example of a malignant melanoma developing in the optic nerve associated with oculodermal melanocytosis. After presentation the patient refused surgery for 19 months and the progression of the tumour necessitated an exenteration of the orbit.

Intraglandular toxoplasmosis lymphadenitis of the parotid gland.

Toxoplasmosis is a protozoan infection manifested by systemic findings as well as generalized or localized lymphadenopathy. Although lympadenitis is the most common clinical form, involvement of the intraglandular nodes within the parotid gland found is a rare occurrence during the course of the disease. This unusual form may mimic a parotid mass, and can cause difficulties in differential diagnosis. In this article, a case of intraglandular parotid lymphadenitis which developed due to toxoplasmosis is presented, and the relevant literature is reviewed.

Focal intrahepatic extramedullary hematopoiesis: color Doppler US and CT findings.

Focal intrahepatic extramedullary hematopoiesis is an extremely unusual condition that can mimic any other solid mass of the liver. Only seven cases have been reported in the literature. Color Doppler ultrasonographic findings of this rare entity have not been described. We report a case of focal mass in a myeloid metaplastic patient diagnosed as focal extramedullary hematopoiesis. Color Doppler findings of this rare condition are described and compared with computed tomography features.

The effect of montelukast on eosinophil apoptosis: induced sputum findings of patients with mild persistent asthma.

BACKGROUND: Apoptosis may be important in limiting airway eosinophilia. Treatment with leukotriene antagonists decreases the number of eosinophils in both peripheral blood and sputum. AIM: To assess the effect of montelukast on eosinophil apoptosis in a group of patients with mild persistent asthma (MPA) and to compare this effect with the apoptotic effect of fluticasone propionate (FP). METHODS: Randomly selected patients with MPA (n = 22) who had not taken anti-inflammatory therapy within the preceding 12 months were included in the study. The sputum induction procedure was performed and the patients were divided into two groups: group 1 (n = 10) received FP 250 microg/day and group 2 (n = 22) received montelukast 10 mg/day orally for 4 weeks. Sputum induction was repeated after the treatment period. The resulting cytospin slides were stained by Wright's stain and morphologic changes in apoptotic eosinophils were assessed by the use of light microscopy by two blinded expert pathologists. Serum soluble Fas ligand (sFasL) concentrations were measured by an ELISA method at baseline and after treatment in both groups, as well as in a group of healthy subjects. RESULTS: In within-group comparisons, the apoptotic ratio (AR) increased at the end of the study period in group 1 (p = 0.05). In the group treated with FP the ratio of sputum eosinophils significantly decreased (p = 0.02), and the AR significantly increased (p < 0.005). No differences were found in the two study groups in serum sFasL levels at the end of the treatment period compared with baseline values (p > 0.05). CONCLUSION: Our findings demonstrate that 4 weeks' treatment with a CysLT receptor antagonist (montelukast) resulted in an increase in eosinophil apoptosis comparable to that produced by FP, suggesting that induction of apoptosis may be a potential mechanism for the mode of action of CysLT receptor antagonists in asthma.

Intrathoracic desmoid tumor of the posterior mediastinum with transdiaphragmatic extension. Report of a case.

We report on a de novo desmoid tumor of the posterior mediastinum with transdiaphragmatic extension in a 40-year-old man. The tumor was inoperable because of its location and the invasion of adjacent structures. Although extremely rare, desmoid tumors of the posterior mediastinum should be considered in the differential diagnosis of posterior mediastinal masses.

p53 codon 213 (A-G) polymorphism in a Turkish population.

We studied a rare polymorphism at exon 6 (CD 213) of the p53 gene in a healthy Turkish population (n = 26) and in Turks with different types of tumors such as malignant lymphoma (n = 12), osteosarcoma (n = 5), and nasopharyngeal sarcoma (n = 4). The polymorphic allele was found in only one of the malignant lymphoma patients (4.16%). Our data revealed that CD 213 (A-G) polymorphism is very rare in the Turkish population and there is no association with the tumors.

The mechanism of bronchial hyperreactivity in allergic rhinitis patients. A light microscopic study on BAL and bronchial biopsy.

Epidemiological data indicated that allergic rhinitis often coexists with and may precede the development of reactive airway disease. In particular, ARP with BHR are more likely to develop asthma. However, the pathogenesis of BHR associated with allergic rhinitis is still remains uncertain. Therefore we designed the study on ARP with/without BHR. The aim of this study were to investigate the presence of an inflammatory process in lower respiratory tract in ARP and to relate these changes to airway responsiveness Eleven ARP with BHR (Group I), eleven ARP without BHR (Group II) and two control patients (Control group) were studied. All of the ARP were judged atopic on the basis of positive skin prick test to common inhalant allergens. Bronchial challenges were performed with increasing concentration of M. All the subjects underwent fiberoptic bronchoscopy, BAL and bronchial biopsies were obtained for pathologic examination. The mean total cell and the mean percentage of macrophages, lymphocytes, neutrophils and eosinophils in BAL fluid were in normal range in all groups without any significant differences between the groups. There weren't any correlation between PC20 to M and the total cell counts and percentage counts of these cells. In bronchial biopsy samples, the absolute numbers of lymphocytes, neutrophils, eosinophils and mast cells in the submucosa showed no differences between the three groups. The epithelial shedding was more extensive in ARP than control subjects (p = 0.05). The thickness of the epithelium was prominent in Group I (p < 0.05) but there was no significant differences in the basement membrane thickening between the three groups. We could only find an inverse correlation between PC20 to M and the mast cell counts in the submucosa (r xy:-0.815 p < 0.05). In conclusion, we couldn't observe any prominent morphological changes which indicate that may cause of BHR in ARP except the increased epithelial shedding in Group I. However, the increased epithelial shedding is not a reliable criterion to comment because of the possibility of mechanical damage of bronchial biopsies caused by the forceps.

The effect of corticosteroids and pentoxifylline in caustic esophageal burns. A prospective trial in rats.

BACKGROUND: Caustic Esophageal Burns (CEB) usually results in scatritial tissue and stricture formation. Management requires preventing the massive inflammatory process that ensues in its early phase and decreasing bacterial complications. METHODS: An animal model was created to investigate the effect of corticosteroids and pentoxifylline in CEB using 52 male Wistar rats. The injury was produced using an indwelling esophageal catheter through which 3N of 12% sodium hydroxide was infused. The rats were grouped as control, CEB, CEB and ceftazidime (CEB-C, 100 mg/kg/day im. bid. 10 days), CEB and ceftazidime plus dexamethasone (CEB-CD, 0.1 mg/kg/day im. bid. 4 weeks) and CEB and ceftazidime plus pentoxifylline (CEB-CP, 50 mg/kg/day im. tid. 4 weeks). The groups were evaluated making use of esophagograms, hydroxyproline (OH-P) contents and histologic examination of the specimens 28 days after injury. RESULTS: No significant statistical differences were observed among the dexamethasone (CEB-CD), pentoxifylline (CEB-CP), antibiotics (CEB-C) and the untreated CEB groups.
