[Osteomyelitis in tropical countries].

A retrospective study was made of 13 patients admitted to the pediatric ward of Gondar College of Medical Sciences from October 1987 to May 1988. It was found that the disease occurs mainly in males. In the most cases there are clear clinical and radiological findings. The main reasons turned to be traumatic and spontaneous with unknown causes. Gram cocci takes the leading bacteries found on cultures taken from the discharging sinuses. Treatment was performed with appropriate antibiotics and surgical intervention when indicated. In regard to response to our treatment most of the patients showed good clinical improvement. With early treatment the course is found to be much more benign.

[Etiology and therapy of peritonitis in childhood].

The causes of peritonitis in childhood, particularly in infancy, were found to differ substantially from those in adult age. An overall account is given in this paper of general and differential diagnosis of extra-abdominal diseases which may be potentially mistaken for peritonitis. Clinical diagnosis is of crucial importance in childhood. Perforated appendicitis was the absolutely major cause of peritonitis in 119 children who received surgical treatment in the course of 18 years. An initial antibiotic combination of gentamicin with ampicillin, and metronidazole together with immune treatment have proved to be an effective therapeutic approach to this form of peritonitis.

Appendicitis↗

[Appendicitis in childhood].

Appendicitis in infancy has proved to be accompanied by particular problems in general and differential diagnosis. Recommendations are made for proper establishment of case histories as well as for clinical examination and surgical approach, with reference being made to the authors' own experience. The authors' perforation rate was 7.3 per cent in more than 1,000 appendectomies on children (up to 20 per cent in literature). Reported in this paper is experience obtained from treatment of severe cases.

[Use of ketamine anesthesia in experimental animal investigations].

The use of the Ketamine anesthesia as an intramuscular injection proved itself as favourable in the animal experimental investigation. The duration of effect amounts to 20-30 minutes at a doses of 5-8 mg/kg body weight. Protective reflexes are preserved. The Ketamine anesthesia is qualified for difficult examination techniques thereby. Surgical interventions including the laparotomy become well-tolerated. The use of Ketamine is simple. The postoperative care is uncomplicated by the continuous analgesic effect.

[Studies following bladder reconstructions in cystoschisis].

The question how to perform the best surgical repair of bladder exstrophy is still being discussed. By various urodynamic investigations following bladder reconstruction we came to the conclusion that the new built bladder and its outlet tract can function normally.

[Therapy of urine incontinence by electro-stimulation (author's transl)].

The most common cause of urine incontinence in childhood is a neurogenic disturbed micturition in cases of myelo-dysplasia. Of decisive significance for the diagnosis and choice of therapy are urodynamic examinations. A report is given on the effectiveness of the author's own examinations using transurethral electro-stimulation.

[Functional dynamics of obstructive ileal conduits in children (author's transl)].

Bad late results following ileal conduit are usually caused by stomal obstruction. Poor emptying of the conduit, ureteric reflux and infection all combine to produce a pathological result. A manometric procedure to assess the functional performance of the conduit is presented. This allows for the diagnosis of early obstruction and decompensation. The most important measurements are a resting pressure of 1.2 kPa and the emptying pressure of 1.7 kPa and a compliance varying from 10 ml per kPa to 71 ml per kPa. The conduit must be isoperistaltic.

[Fractures of the distal end of the humerus in children (author's transl)].

This study reports on follow-up controls of 53 children with fractures of the humerus near the elbow-joint. 31 children had supracondylar fractures, 11 condylar fractures and 11 other lesions of the lateral epicondylus. The examination was carried out with special attention to functional aspects. A new own method for measuring the flexion force of the elbow is described. Baumanns angle and the dia-epiphyseal angle were taken into consideration when examining the X ray pictures. We obtained the best results when treating supracondylar fractures by Baumann's extension method. All condylar fractures should be operated on. The follow-up results were also excellent in the operatively fixed fractures of the lateral epicondylus.

[Histological and immunohistochemical investigations on the transplanted isolated islets of Langerhans in diabetic rats (author's transl)].

Isologous isolated islets of LANGERHANS were transplanted in the peritoneum and through the portal vein in the liver of diabetic rats. With the help of histological and immunohistochemical investigations, estimation of blood sugar and glucose tolerance tests was tested over a period of 12 month, whether the peritoneum or the liver is more suited as transplantation site for isolated islets of LANGERHANS. After transplantation of islets of LANGERHANS in the peritoneum of diabetic rats the blood glucose decreased and this effect could be maintained over a period of three months. The histological investigations showed strong periinsulinar reactions and the immunohistochemical insulin and glucagon proofs prospered over a period of three months. After transplantation of isolated islets in the liver it resulted in a normalization of blood glucose levels up to 12 months after transplantation and in this time prospered the immunohistochemical insulin and glucagon proof. These results have suggested, that the liver is more suited as transplantation site for isolated islets of LANGERHANS than the peritoneum.

Transplantation of isologous islets of Langerhans in diabetic rats. Long-term immunohistochemical results.

Isologous isolated islets of Langerhans were transplanted into the peritoneum and through the portal vein into the liver of diabetic rats. This resulted in the normalization of fasting blood glucose levels, with clear improvement of glucose tolerance tests. The results after intrahepatic implantation were better than after intraperitoneal implantation. Three months after implantation of islets into the peritoneum diabetes recurred and after one year only 2 of the 15 rats were alive, with high blood glucoe levels. In the group with intrahepatic islets the normalization of blood glucose was maintained over a period of one year. Immunohistochemical insulin proofs were positive in the peritoneum over a period of 3 months and in the liver up to one year after implantation. Glucagon proofs were positive in the intraperitoneal islets for 6 weeks and in intrahepatic ones for one year. Morphological examination of transplanted islets suggested that the liver is better suited for islet transplantation than the peritoneum.
