Post-traumatic mitral regurgitation and ventricular septal defect in absence of left pericardium.

A six-year-old boy was hospitalized following a car accident. Cardiac catheterization demonstrated acute post-traumatic mitral regurgitation (MR) and ventricular septal defect (VSD). The patient was operated on for mitral valve replacement and closure of the VSD. On opening the chest absence of the left pericardium was noted. Cardiac catheterization performed in the postoperative period demonstrated a residual left-to-right shunt at ventricular level. The patient was reoperated on 10 months later for closure of the residual VSD. Six months after the last operation the patient is in good condition. The congenital absence of the pericardium is considered as favoring both the post-traumatic cardiac lesions and the postoperative pulmonary problems on the left side.

Double outlet right ventricle, non-committed ventricular septal defect and pulmonic stenosis. Anatomical and surgical considerations.

A case of double outlet right ventricle with pulmonary stenosis, a non-committed muscular ventricular septal defect occupying the sinus and trabecular portions of the ventricular septum, an additional small subtricuspid ventricular septal defect, situs solitus, atrioventricular concordance and aortic d-position is described. Atrioventricular valvular canal type malformations, splenic, systemic or pulmonary venous anomalies, all frequently seen in double outlet right ventricle with uncommitted VSD, were not present.

[Evaluation of K-canrenoate in post-surgical treatment of open heart paediatric surgery (author's transl)].

The variation of Aldosterone and its consequences on urinary Sodium and Potassium after cardiac surgical trauma are well known. We made a study on 2- pediatric patients operated by extracorporeal circulation (ECC). Ten patients were given an Aldosterone inhibitor (K-canrenoate), ten patients were the control group. The efficacy of treatment with the Aldosterone inhibitor drug is proved by: 1) progressive, significant drop of Natremia; 2) considerable heightening of Kaliemia; 3) significant higher ratio between Natriuria and Kaliuria. The importance of antialdosteronic treatment is demonstrated by: 1) diminished sodium retention implies a lessened water retention; 2) increased potassium retention protects from dangerous hypokaliemia in the early postoperative period; 3) possible inotropic and extrarenal activity. It's important to modify the dose of K-canrenoate in relation to the level of electrolytes. High levels of potassium may follow a dosage of 2,5 mg/Kg/24 h. In other cases higher dosage may be necessary.

[Treatment of coarctation of the aorta in paediatric age. Result of 7 years of experience (author's transl)].

Results of 83 consecutive operations for coarctation of the aorta in the paediatric age group (0-20 years), performed in our Institution between September 1971 and May 1978 are presented. Mean age at operation was 7.04 years. All patients have been divided into two groups according to age under or over one year. Group one comprises 13 cases under one year of age. There were two death (15.23%). In group two there were 70 cases with two deaths (2.85%). The main post-operative complications are analyzed. Particular attention is paid to the incidence of postoperative hypertension in relation to age bracket and with this point in mind discussion is carried out on the opportunity of early operation before stable hypertension has set in.

[Permanent pacemaker implantation in children after open heart cardiac surgery (author's transl)].

Between 1972 and 1977, a permanent pacemaker has been implanted in 9 children for complete heart block, after repair of a congenital heart disease. Children's age ranged between 2 1/2 years and 13 years (mean age 8 years). The block had been caused by the operation in 8 children and was pre-existent in the last one. A permanent pacemaker was not implanted in another four children with postoperative complete heart block. In seven cases the pacemaker was implanted in the subclavear zone and connected to a transvenous electrode; in the remaining two cases the pacemaker was positioned subcutaneously in the abdomen and connected to an epicardial electrode. VVI-type pacemaker have always been used. Three children died after implantation because of chronic cardiac failure complicated, in one case, by surrhenalic insufficiency. In two cases the pacemaker was replaced because of battery exhaustion, as suggested by routine controls; in five cases, wire breakage occurred and it was replaced together with the pacemaker. In another case transvenous electrode displacement occurred; skin infection at pacemaker site occurred only once. No skin breakdown at pacemaker site has ever occurred, or any problem due to excessive pacemaker dimensions. Reappearance of normal synus rhythm was noted in one patient. No complication has been reported so far for the cases with postoperative complete heart block not treated with cardiac pacing.

Aneurysm of the pars membranacea of the interventricular septum.

Six cases of aneurysmal formation of the pars membranacea septi (AMS) are presented. Three cases associated with a ventricular septal defect (VSD) and one with VSD, prolapse of the right aortic cusp and aorto-right ventricular fistula, were operated on and the aneurysm, unsuspected preoperatively, was found at the operating table. Two cases, both with aortic coarctation (CA), have been diagnosed on the basis of selective angiocardiography and operated solely for resection of the coarctation. In three patients a mild pulmonary infundibular pressure gradient was present at catheterization. This anomaly is considered rare. It has been reported to complicate surgery, may not require surgery, and is considered by some to be a prelude to spontaneous closure of a VSD. The English medical literature concerning aneurysm of the membranous septum has been reviewed and our opinions on the management of these patients are presented. Including our own six patients, we are aware of approximately one hundred and sixty five reported cases, with possibly an additional 16 cases to be added.

Successful repair of post-infarction heart rupture. Case report and review of the literature.

Left ventricular rupture secondary to acute myocardial infarction (AMI) if untreated, is invariably fatal. Successful surgical correction reported in the reviewed literature amounts to twenty cases. This is the case presentation of a 53 year old Caucasian woman admitted urgently to our Institution 6 hours after acute chest pain with a presumptive diagnosis of intrapericardial aortic rupture secondary to acute ascending aortic dissection. A cross-sectional echocardiogram demonstrated a posterior left ventricular rupture secondary to myocardial infarction. Emergency repair was carried out with the aid of cardiopulmonary bypass (CPB) and the patient was discharged after an uneventful recovery. However, five months later she was reoperated on for resection of a large pseudoaneurysm presumably secondary to incomplete resection of nonviable myocardium at the first operation. The patient made an uneventful recovery and remains asymptomatic and well. On the basis of this experience and review of the literature the authors propose a more aggressive approach in an attempt to improve the salvage rate of this not so rare complication of AMI.

Correction versus palliation of congenital heart diseases in the first year of life.

In the period 1971-1981 220 children below 1 year of age have been operated upon in our Hospital for Congenital Heart Diseases (CHD). Among them, 125 children were selected, in whom, because of the type of CHD present, either a palliation or a correction could have been chosen as the surgical approach. The first three common malformation treated are presented.
