Therapy-related myelodysplasia and leukemia occur infrequently following VP-16 priming and autotransplantation without total body irradiation.

The use of VP-16 for stem cell mobilization has been cited as a significant risk factor for the development of therapy-related myelodysplasia/leukemia (tMDS/tAML) following autologous transplantation. The present study analyzed a large cohort of patients who underwent autotransplantation following stem cell mobilization with VP-16 and radiation-free preparation in order to determine the risk of tMDS/tAML. The estimated incidence of 9.9% at 7 years suggests that in the absence of TBI, VP-16 priming is not associated with an increased incidence of tMDS/tAML.

Music as an adjunct to antiemetic therapy.

PURPOSE/OBJECTIVES: To test whether use of music as a diversional intervention during high-dose chemotherapy administration would affect perception of nausea and episodes of vomiting. SAMPLE: 39 patients undergoing bone marrow transplant. A total of 33 patients were included in the data analysis, with 17 in the control group and 16 in the music intervention group. METHODS: Patients were assigned randomly to a control group (usual antiemetic protocol) or the experimental group (usual antiemetic group plus music intervention during the 48 hours of high-dose cyclophosphamide administered as part of the preparative regimen). MAIN RESEARCH VARIABLES: Use of a music intervention, perception of nausea, and instances of vomiting. FINDINGS: Significant differences were found between group scores on a visual analog scale for nausea and number of episodes of vomiting, demonstrating that the experimental group experienced less nausea and fewer instances of vomiting. CONCLUSION: This study found that music is an effective adjunct to a pharmacologic antiemetic regimen for lessening nausea and vomiting, and this study merits further investigation through a larger multi-institutional effort. IMPLICATIONS FOR NURSING PRACTICE: Using music as a diversional adjunct intervention to antiemetic therapy is helpful in decreasing nausea and vomiting. The intervention can be initiated independently by nurses and individualized for each patient, leading to greater patient comfort and compliance with high-dose chemotherapy.

Survey of oral hygiene regimens among bone marrow transplant centers.

Oral complications may occur as an adverse effect of the preparative regimens used in bone marrow transplantation (BMT). A variety of oral-care regimens are used to manage the prevention and treatment of oral complications. The purpose of this descriptive survey was to identify similarities and differences in oral-care regimens among various BMT centers. A 35-item questionnaire was sent to 92 BMT centers, with an 80% response rate. The responses were analyzed using descriptive statistics. Following a variety of guidelines, routine oral assessments were performed by physicians (41%), nurses (33%), and dentists (25%). Thirty-eight percent of the BMT centers used WBC counts as a basis for changing the oral hygiene regimen; 90% used platelet counts. When toothbrushing was discontinued, 50% of the centers used toothettes. In 54% of the centers, flossing was not used in oral hygiene regimens. The two primary mouth-rinses used were chlorhexidine and normal saline. In summary, few similarities existed in the assessment and management of oral care.
