Pediatric pain assessment.

Pain in children is frequently underrecognized or undertreated based on misconceptions and misjudgments about the experience of pain in this age group. The goal of pain assessment is to obtain sufficient and accurate data, depending on the developmental stage of the child. This information can come from questioning the child directly, including using pain rating scales, and evaluating physiological and behavioral changes. Assessing the emotional state of the family and caregivers is also crucial. Health care providers need to be persistent and thorough when evaluating pain in children and use this information to formulate and modify an appropriate treatment plan.

JNC-VI, hypertension, and you.

More than 50 million U.S. adults are estimated to have hypertension--defined as systolic blood pressure (SBP) of at least 140 mmHg and/or diastolic blood pressure (DBP) of at least 90 mmHg--or take antihypertensive medication. Hypertension is directly implicated in a range of conditions, including coronary artery disease (CAD), renovascular disease, and cerebrovascular accidents.

Providing nourishment at home.

In many aspects of patient care, technologic advances have changed the home care landscape. Treatment once believed available only to hospitalized patients has become, with the right multidisciplinary home care team, both feasible and equally effective. Nutrition delivery is among these advances in home care.

ET, phone home: telehome care.

No doubt that advances in technology are making a substantial impact on the delivery of health care. The idea of using some of this high-tech equipment within the home setting can seem quite unusual, perhaps extraterrestrial, giving the phrase "Reach out and touch someone" a whole new meaning. This aspect of health care even has a new name--telehome care--and we are just at the threshold of this frontier.

Quality of care for dialysis patients: national initiative focuses on improved treatment.

The National Kidney Foundation Dialysis Outcomes Quality Initiative (NKF-DOQI) is a groundbreaking project established to develop evidence-based dialysis treatment guidelines. Highlights of the guidelines were presented to the renal community in November 1996 at the National Kidney Foundation's Annual Scientific Meeting in New Orleans.

Home-based immunoglobulin therapy reduces incidence of neonatal RSV infection.

Respiratory syncytial virus (RSV) infections are responsible for more than 90,000 hospitalizations and 4500 infant and childhood deaths each year. RSV infection, which peaks between January and March, can cause respiratory problems, such as bronchiolitis and pneumonia, in children younger than 2 years old.

Special home visitation program provides long-term benefits.

In recent years, home visitation services have been widely promoted as a means of preventing health and developmental problems in children from vulnerable families. The U.S. Advisory Board of Child Abuse and Neglect, for example, recommended that home visitation services be made available to all parents of newborns to prevent child abuse and neglect.
