Proteasome localization and ultrastructure of spermatozoa from patients with varicocele--immunoelectron microscopic study.

Localization of proteasomes in spermatozoa from patients with varicocele-associated sterility was studied by means of immunolabeling with the MPC21 monoclonal antibody detecting the C3 subunit of the 20S proteasome. The reaction was visualized for electron microscopy using the secondary Nano-Gold-coupled antibody with Gold-Enhancement in pre-embedding technique. We found that semen samples from varicocele patients contained a large amount of abnormal spermatozoa characterized by the presence of dispersed chromatin and large residual bodies (cytoplasmic droplets) as well as spermatids at various stages of spermiogenesis. In normal spermatozoa, the immunolabeling was found in the acrosome, postacrosomal regions, nuclear vacuoles, in the neck and in the middle-piece as well as in the residual bodies, while chromatin remained unlabeled. In varicocele spermatozoa, the immunolabeling was also associated with chromatin and large residual bodies (cytoplasmic droplets). In contrast to normal, mature spermatozoa, the chromatin of the cells at earlier stages of spermiogenesis was strongly immunolabeled. The association of proteasomes with sperm chromatin and large residual bodies can be the sign of abnormality and disturbances in spermatogenesis associated with varicocele.

Localization of a proteasomal antigen in human spermatozoa: immunohistochemical electron microscopic study.

The ultrastructural localization of a proteasomal antigen in human spermatozoa was studied by means of immunolabeling with the MPC21 monoclonal antibody and secondary gold labeled antibody with 1.4 nm gold particles in combination with silver enhancement reaction using pre-embedding technique. The labeling was found in the acrosomal and postacrosomal regions, in the connecting-piece (neck) and, in some cases, in the middle-piece and also in the residual bodies. There was no significant reaction in condensed chromatin. In some abnormal forms of spermatozoa, in which the chromatin was not well condensed, the labeling in nuclei was present. The nuclear vacuoles with looser chromatin were usually strongly labeled. The nuclei of cells representing different stages of spermatogenesis, that were present in semen samples, were also labeled.

[Semen parameters in secondary infertile couples].

OBJECTIVE: The aim of this study was to evaluate sperm values in the group of secondary infertile couples. MATERIAL AND METHODS: The material of the study included 35 couples with abortion in anamnesis, 12 couples with delivery in past and 11 couples conceive and deliver during studies. Sperm analysis was performed according to the WHO guidelines. RESULTS: Men from couples with abortion in past had significantly higher sperm concentration compare to couples with delivery in past. In other parameters there was no significant differences observed. CONCLUSION: Sperm concentration can be important factor in secondary infertility.

[Results of clomiphene citrate ovarian stimulation vs laparoscopic electrocoagulation of the ovaries in infertile women with PCOS].

OBJECTIVE: To assess the results of clomiphene citrate ovarian stimulation vs laparoscopic electrocoagulation of the ovaries in infertile women with PCOS. DESIGN: 47 women with PCOS were included into the study. 25 of them had ovarian stimulation using clomiphene citrate (100 mg for 5 consecutive days, during 6 cycle) and 22 underwent laparoscopic electrocoagulation of the ovary (6 cycle follow-up) No significant differences were detected between age and body mass index. RESULTS: In the first group ovulatory cycles were confirmed in 68% of women, pregnancy rate was 28%. In the second group ovulations were observed in 90.9% of women and pregnancy rate was 63.6%. There were statistically significant differences. CONCLUSION: In summarising, the results showed lower, statistically significant differences in pregnancy rate in the treatment of infertility associated with PCOS. It seems that the much more successful result of the PCOS therapy may be achieved after laparoscopic ovarian diathermy in comparison with stimulation of the ovaries with CC.

[Testicular artery Doppler flow evaluation in men with degenerative changes of seminiferous tubules].

AIM: The aim of this study was to assess the eventual impact of degenerative changes of seminiferous tubules on Doppler flow parameters in testicular vessels. MATERIAL AND METHODS: There were 12 men with degenerative changes of seminiferous tubules (diagnosed by testicular biopsy) in study group and 24 healthy men with normal semen parameters in control group. Patients' age in both groups was respectively 32.22 +/- 5.31 and 29.64 +/- 3.17 (ns). Sonographic biometry of testes volume was performed and the values of qualitative flow indices in terminal section of testicular artery were evaluated. The both groups results were compared with themselves by T-test. Also the values of qualitative flow parameters in testis after biopsy and contralateral one were compared. Additionally the correlation between vascular flow parameters and selected hormone levels (FSH, LH, T, E2 and Prl) was determined. RESULTS: The mean testes volume in the study group was 10.81 +/- 3.77 cm3 and in the control one 14.42 +/- 5.42 cm3 (ns). The values of particular flow indices in both study and control group were respectively: S:D 3.20 +/- 0.95 and 2.92 +/- 0.96 (ns), PI 1.46 +/- 0.46 and 1.23 +/- 0.41 (ns), RI 0.69 +/- 0.09 and 0.66 +/- 0.08 (ns). There were no differences between flow parameters in testis after biopsy and contralateral one. Testosterone was the only hormone correlating with analyzed flow indices (S:D, PI, RI) and the correlation coefficient value was respectively 0.51, 0.49 and 0.52 (p < 0.05). CONCLUSIONS: The significant influence of degenerative changes of seminiferous tubules on vascular flow in testicular artery was not observed. There were no differences between flow parameters in testis after biopsy and contralateral one. The testosterone was the only hormone significantly correlating with analyzed flow indices.

[Analysis of female infertility structural factors].

The study was aimed to evaluate, within the population of infertile women, the interrelations between such characteristics as patient's age, duration and type (primary, secondary) of infertility, its etiologic factor as well as the treatment outcome (pregnancy rate). Retrospective analysis was performed in 176 women aged 20-43 years (mean age 29.5 years). Primary infertility was less common in older women (above 35 years). Duration of primary infertility was independent of age, on an average 1.5 years shorter compared to secondary infertility. Participation of all causal factors in the structure of infertility was steady--11.6-20.2%. The most frequent reasons of secondary infertility were unexplained infertility 55.3% and tubal pathology--19.1%. The pregnancy rate was 30% in all treated women. In the studied population of women, pregnancy rate was not significantly influenced by the patients's age, type of infertility and its cause.

[The naloxone test in the degenerative changes of seminiferous tubules].

AIM: Are there any changes in hypophyseal and gonadal hormones levels during the naloxone test in the degenerative changes of the seminiferous tubules? MATERIAL AND METHODS: The naloxone test (0.4 mg i.v.) was performed in 13 patients with degenerative changes of the seminiferous tubules. The plasma FSH, LH, prolactin, testosterone and estradiol levels were determined before and 30, 60, 90, 120 minutes after the administration of the drug. RESULTS: In the examined group of patients the plasma FSH, prolactin and estradiol levels were significantly higher, testosterone levels significantly lower and LH levels were in the same range compared with the patients with azoospermia and normal spermatogenesis. The plasma FSH, prolactin and testosterone levels did not change significantly their values during the test. The plasma estradiol levels decreased significantly and plasma LH levels increased significantly during the test. CONCLUSION: Elevated plasma FSH, prolactin and estradiol levels, diminished testosterone levels and the decrease of plasma estradiol levels and the increase plasma LH levels during the naloxone test indicate the degenerative changes of the seminiferous tubules in the cases of azoospermia.

[The naloxone test in patients with varicocele].

AIM: Is there any influence of naloxone administration on plasma levels of hypophyseal and gonadal hormones in varicocele patients? MATERIAL AND METHODS: The naloxone test (0.4 mg i.v.) was performed in 15 varicocele patients The plasma FSH, LH, prolactin, testosterone and estradiol levels were determined before and 30, 60, 90, 120 minutes after the administration of the drug. RESULTS: The prolactin levels decreased significantly, and FSH, LH, testosterone and estradiol levels did not change their values. The similar results were obtained in patients with azoospermia and normal spermatogenesis. CONCLUSION: The treatment of varicocele patients with anti-opioid drugs is not recommended because the naloxone did not increase LH and testosterone plasma levels.

[Doppler flowmetry measurements in testicular artery of aging men].

AIM: Testicular artery Doppler flow measurements in aging males were performed for the evaluation of eventual disorders of vascular flow in vessels supplying the testes. MATERIAL AND METHODS: 40 men aged between 51 and 80 years (studied group) and 40 healthy men aged below 40 (control group) were examined. The volume of testes and qualitative flow indices (S: D, PI, RI) in terminal section of testicular artery were measured. The difference between particular values obtained in both groups and the correlation of those parameters with testes volume and patients' age was determined. RESULTS: PI and RI values were significantly higher in studied group when compared to the control one. The correlation coefficient between values of flow indices and testes volume as same as patients' age appeared to be unsignificant in all cases with the only exception in case of S: D and testes volume in studied group. CONCLUSION: The values of qualitative flow indices indicate the increased vascular resistance in testicular artery of aging males.

[Congenital malformations of the epididymis and vas deferens as reasons for male infertility].

Between 1966-1991 330 patients where the semen analysis revealed azoospermia and the histopathological examination of biopsy specimen of testis shown retained spermatogenesis and spermiogenesis underwent the surgical intervention. In 80% of cases congenital malformations of epididymis and vas deferens were found. The types of malformations detected during the surgery are discussed.

[Fertility disturbances in males working as professional computer operators].

From 2376 patients who were diagnosed as infertile between 1986-1991, 20 professional computer operator males in the age of 26-40 were chosen (0.8%). Their working time was at least 5 hours daily and they have been working for 2-12 years (mean 7.7 +/- 3.3 years). The mean number of spermatozoa in 1 ml of sperm was 45.5 +/- 23.7 X 10(6)/ml, the percentage of properly motile spermatozoa equaled 21.5 +/- 12.2% and the percentage of pathological forms equaled 26.6 +/- 6.1%. The percentage of properly motile spermatozoa was decreased in all patients. There was not significant tendency of decreasing of the number of spermatozoa in 1 ml of sperm and percentage of properly motile spermatozoa dependent on the years of working in front of computer monitors. Whereas percentage of pathological forms increases statistically significantly in dependency of the years of working.

Male infertility and its treatment: a review of over 4700 patients treated for male related infertility.

Changes in semen parameters are presented for 4708 patients treated for infertility. The largest group comprised patients with oligoasthenozoospermia of various grades (57.7%). Improvement in the seminal state was noted in 70.3%. The percentage of achieved normospermia increased proportionally to the initial semen state. Pregnancy rate also increased proportionally to the initial seminal state. Among the patients' wives 20.4% conceived: 28.2% of these pregnancies had occurred after having achieved normospermia, while in 71.8% the seminal states were still below the accepted normal values.

Investigation and results of treatment in male sterility.

The authors present the results of treatment in 6548 cases of male sterility. According to their findings 598 (9.1%) men were ineligible for treatment. The various kinds of therapy resulted in an improvement of semen quality in 51.8% of the cases, 57.0% of the cases being eligible for treatment. The best results were obtained in patients with oligozoospermia, asthenozoospermia and teratozoospermia. The percentage of pregnancies was much lower (8.5%) than the improvement of semen quality.
