[Effect of patient age on use of the knee joint score].

AIM OF THE STUDY: Clinical studies are done with the help of scores though different factors of influence lower comparability. The underlying study examines the influence of patient age as this always presents a certain span. METHOD: 96 degree I damaged knee joints were examined by 3 examiners using the Larson-, Lysholm-, Marshall-, HSS- and OAK-score. Furthermore ratings by a VAS and the Tegner activity score were done. With the Friedman test, the rank correlation coefficient by Spearman and the contingency chart by Bowker it was tested if the examiners and the scores rate equally. To find out the influence of the age three age groups were made up. RESULTS: The examiners judged significantly different excluding the Marshall and the OAK scores. In between two examiners no significant difference could be found between the young and the middle-aged patient group. Between two other examiners no significant difference was found only for the Lysholm and the HSS score in the young group and only for the Lysholm score in the middle-aged group. In the group of the senior patients no significant difference for the scores by Lysholm, Marshall and OAK were found. In the comparison of the second examiner pair no significant differences could be proven by the scores by Larson, the OAK and the HSS. All other comparisons were significantly different. In the comparison of all five scores significant differences were seen between the Larson score and the HSS and the Lysholm and the HSS. The Lysholm score proved to be the strictest, the HSS to be the leanest. CONCLUSION: Especially for the senior patients in dependency of the examiner and the chosen score significant differences were found concluding that the relevance of results lessens for future times as the score results drop with patient age anyway. The age span in one study should have a maximum of 10-20 years to reduce the influence of age on the final result.

[Is GSB knee prosthesis implantation for patellar problems still justifiable? Experiences and results based on 230 implanted GSB knee joint prostheses].

In our clinical the GSB-prosthesis has proved to be a good therapeutic concept. In 1988 we controlled 230 patients who had got GSB-prostheses in the years 1979-1987. The average age of our patients was 75 years. Most times surgery had been done because of idiopathic osteoarthrosis, rarely because of post-traumatic or rheumatoid arthritis. Indications for surgery were usually severe pain, limited range of movement and deformities of the knee joint. In the beginning of the studied period we had implanted GSB-type I-prostheses, later we had implanted GSB-type II-prostheses with or without femoropatellar surface. We achieved good results in pain reduction and in range of movement. However the major complications were caused by the patella. These patients had problems at climbing stairs, pain at the beginning of movements and at getting up from chairs. They presented with increased pain sensitivity at the tip of the patella and to friction of the patella. Regular radiological findings were severe destructions or fractures of the upper patellar pole. These problems led in 40 cases (17% of our cases) to reoperations until 1988 (At all we reoperated 50 times because some patients needed additional reoperations). According to our studies results there will be a need for reoperations in about 50 other cases. We discuss the following reasons for this high rate of severe patellar complications demanding re-surgery: 1. Localisation of the patella: according to our studies the patella is frequently dranged cranially, seldom laterally. 2. Often we found an internal rotation of the tibia against the femur.(ABSTRACT TRUNCATED AT 250 WORDS)

Proprioceptive function in knees with and without total knee arthroplasty.

The goal of this study was to evaluate the differences in angle reproduction capability after nonconstrained posterior cruciate ligament retaining total knee arthroplasty after a follow-up time of 63.9 months compared with the healthy contralateral leg and a control group. In 28 total knee arthroplasty patients (mean age, 65.7 yr) and 25 control subjects (mean age, 55.7 yr), 16 measurements were made between 0 degrees and 90 degrees in 30 degree steps. The leg was positioned by the examiner and then relaxed; afterward, the subject was asked to reproduce the original position. Each measurement was made with the patient or control being blindfolded and not blindfolded to assess the influence of visual control. We found significant differences in total knee arthroplasty patients in contrast to healthy subjects. Without visual control, the mean deviation of the total knee arthroplasty group starting with a 0 degree angle was 7.7+/-5.9 degrees and 4.6+/-4.7 degrees for the healthy subjects. With visual control, the mean deviation in the patient group was 11+/-7.5 degrees, and in the control group, it was 7.2+/-5.0 degrees. Total knee arthroplasty patients did not show significant differences between the operated on and the contralateral knee. Also, in the total knee arthroplasty group, significant differences could not be found comparing reproduction with and without visual control and comparing both starting positions. In the control group, significant differences could be found comparing visual and nonvisual control in the 60 degree angle. The comparison between the 30 and 60 degree repositioning from both 0 and 90 degree starting positions showed a significant difference in the 60 degree angle. In summary, reduced proprioceptive capabilities are present after knee arthroplasty in both the operated on and the contralateral leg in our study group. Postoperative complaints can perhaps be explained by a loss of proprioceptive capabilities, especially at the 60 degree angle. The operative technique should involve special care of ligaments and muscles to preserve stability and receptors as well. Rehabilitation should pay more attention to the remaining receptors.

The European Basic Core Curriculum for Nephrology Nursing, the process of writing, publishing and disseminating.

In August 2002 the European Basic Core Curriculum (BCC) for Nephrology Nursing was published on the EDTNA/ERCA homepage. At the EDTNA/ERCA annual conference in The Hague in September 2002, it was presented to the delegates as an oral presentation and a poster presentation. Publicity flyers were translated into seven languages and distributed to delegates. This paper will describe the aims of the BCC, the process of writing and publishing, the use of the BCC in nursing schools and the measures that have already been taken in implementation and dissemination.

Additive impairment of the barrier function and irritation by biogenic amines and sodium lauryl sulphate: a controlled in vivo tandem irritation study.

Biogenic amines are potential irritants e.g. in fish-, meat-, milk- and egg-processing professions like cooks, butchers and bakers. The aim of this study was to test the irritative and barrier-disrupting properties of the biogenic amines ammonium hydroxide (AM), dimethylamine (DMA) and trimethylamine (TMA). A repeated sequential irritation of 30 min twice per day was performed over a total of 4 days (tandem repeated irritation test) on the back of 20 healthy volunteers of both sexes with AM, DMA, TMA and sodium lauryl sulphate (SLS). The epidermal barrier function was assessed with a Tewameter TM 210, stratum corneum surface pH was measured with a Skin-pH-Meter 900, inflammation was assessed with a Chromameter CR-300 on the a* axis for redness and a visual score was recorded. All tested biogenic amines (AM, DMA and TMA) induced a barrier disruption and a pH increase paralleled with a 1-day-delayed onset of inflammatory signs. These effects were further enhanced and accelerated by a sequential application of SLS together with the biogenic amines, and inflammation occurred earlier than with the single compounds. Acetic acid (AA) in contrast did only show mild barrier disruption and no significant inflammatory signs. Our system allowed a ranking of the different compounds in their irritative potential in the tandem irritation with SLS: SLS > NaOH > TMA > AA > AM > DMA. The results are suggestive that in the food-processing industry the simultaneous contact with biogenic amines and harmful detergents like SLS should be minimized.

Regulation of human renin gene transcription by cAMP.

To delineate the cis-acting elements of the proximal promoter responsible for cAMP-induced human renin gene transcription, 5'-flanking regions of the human renin gene were fused to a luciferase reporter gene and transfected in chorionic cells. Forskolin treatment induced the expression of luciferase by 2.4 fold when the reporter plasmid contained the promoter region (-582 to +16). Mutation or deletion of the CRE diminished (1.7 fold) but did not abolish cAMP-induced transcription, demonstrating that region containing the CRE and region containing a Pit-1 site were both necessary for cAMP maximal induction. Taken together, these results show that the cAMP response of the human renin gene may involve CREB binding the CRE and tissue-specific factors (from chorionic and kidney cell origin), different from Pit-1, that interact with the Pit-1 response DNA elements.

The "hybrid" approach for revascularisation: direct myocardial revascularisation and coronary angioplasty.

We report the cases of two patients where catheter-based laser direct myocardial revascularisation has been coupled with conventional coronary angioplasty at the same sitting using the Biosense left ventricular electromechanically guided laser procedure. In both the cases, the non-revascularizable ischaemic target zone was identified using left ventricular electromechanical mapping signals, and Ho: YAG laser channels were placed at the designated target lesion following successful coronary stenting. The results did not show any procedural complications. This preliminary report suggests the feasibility and safety of this "hybrid" percutaneous approach for myocardial revascularisation.

The effect of a water soluble adjuvant on the immune response to synthetic polypeptides.

The effect of a water soluble adjuvant (WSA) extracted from mycobacteria on the immune response to synthetic polypeptides has been studied in rabbits and inbred mice. WSA increased the antibody response of rabbits to the synthetic polypeptide poly(Tyr,Glu)-poly(DLAla)--poly(Lys) designated (T,G)-A--L when injected together with either incomplete or complete Freund's adjuvants (FIA or FCA). As compared to the respective controls, the effect of WSA was more pronounced when injected with FIA. No detectable antibody titre was observed in rabbits immunized with (T,G)-A--L and WSA administered in an aqueous medium. WSA had no enhancing adjuvant effect on the antibody response of C3H.SW high responder mice to (T,G)-A--L when given either with FIA or with FCA. However, the antibody titres of C3H/HeJ and SJL low responder mice to (T,G)-A--L were increased by WSA when injected in combination with FIA or FCA. The immune responses of mice to two other synthetic polypeptides poly(Phe,Glu)-poly(DLAla)--poly(Lys), designated (Phe,G)-A--L, and poly-(Tyr,Glu)-poly(Pro)--poly(Lys), designated (T,G)-Pro--L, were not affected by the addition of WSA. Thus, WSA "corrects" selectively the genetic defect of low responder mice to synthetic antigens.

Failure of insulin-like growth factor 1 to improve radiocontrast nephropathy.

Exogenous insulin-like growth factor 1 (IGF-1) has been reported to improve experimental ischemic acute renal failure. We investigated a possible beneficial role of IGF-1 in a model of radiocontrast nephropathy induced by indomethacin, nitro-L-arginine ester and iothalamate. Multiple injections of recombinant human IGF-1 (or its vehicle) at 150 microg/100 g body weight/day were given for 24 h starting 1 h after radiocontrast, or initiated 1 day after the insults and continued for 48 h. IGF-1 prevented neither the fall in creatinine clearance nor medullary thick ascending limb necrosis observed at 24 h. Similarly IGF-1, given for 2 days after renal failure had been established, did not accelerate functional recovery at 72 h, did not ameliorate catabolism and did not alter the morphological evolution of intrarenal damage. In conclusion, IGF-1 had no beneficial effects in this model of radiocontrast nephropathy.

[Crystalline structure of endoprosthetic UHMWPE in relation to sterilization and mechanical processing].

We analysed the changes in crystallinity in sterilized and machined UHMWPE products used in the manufacture of total joint components. For this purpose, so-called DC measurements of absolute crystallinity and infrared spectroscopy for determining the relevant crystallinity were carried out. The DSC measurements are based on a further development of differential thermo-analysis (DTA), and a comparison of the melting enthalpy of an unknown material in comparison with known material. The results show particularly high values for gamma sterilized products, and the lowest values, i.e. the least brittleness, after sterilization with ETO. The crystallinity of the original material was 55%, that after gamma sterilized 56.5%, and after steam autoclaving 57.9%; after gamma sterilized in argon 56%, and after ETO sterilized 57.7%. During the period prior to the manufacture of the end product, a wide range of values is determinable. The average crystallinity of the end products varies between 55.4% and 61.6%. Among other things, changes in crystallinity during storage and after implantation must be taken into account. To summarize, we recommended ethylene oxide sterilization, after which the best values of crystallinity were seen in both semi-finished and finished products (prostheses).
