[A case of advanced gastric cancer associated with smouldering adult T-cell leukemia (ATL)].

A 43-year-old man was admitted to our clinic because of a 2-month history of anorexia, general fatigue and an upper abdominal mass. Upper GI series and endoscopic examination revealed Borrmann IV-type gastric cancer. The histologic diagnosis was undifferentiated adenocarcinoma. His peripheral blood contained approximately 3% atypical lymphocytes. These lymphocytes were identified to be ATL-cell based on E-rosette formation assays and anti-ATL associated antigen-antibody tests. Although the patient had no typical ATL-symptoms, we made a diagnosis of double cancer, i.e. gastric cancer and smouldering ATL. This is the first reported Japanese case of gastric cancer associated with smouldering ATL relationship between gastric carcinoma and other malignant diseases, especially ATL, is discussed.

Serum gastrin levels in patients with thyroid dysfunction.

Fasting serum gastrin levels measured by radioimmunoassay were found to be elevated in patients with hyperthyroidism and low in patients with hypothyroidism. The oral administration of beef extracts resulted in more increase of serum gastrin in hyperthyroid patients than in normal subjects. After restoration of the euthyroid state by treatment, no more increase in serum gastrin levels was observed. Slight correlation between gastrin levels and serum T3 levels was observed in pretreated hyperthyroid patients (r = 0.40), but significant correlation between them was found after restoration of the euthyroid state by treatment (r = 0.50). However, it seemed to be able to divide into two groups in the pretreated patients. One was a patient group whose gastrin levels correlated closely to serum T3 levels (r = 0.83, p less than 0.01). The other was a group whose serum gastrin levels remained in low even in high T3 levels (r = 0.81, p less than 0.01). Different sensitivity to thyroid hormone in the G-cells of gastrointestinal tract may exist in these two groups, because patients age and duration of their illness were not different between them.

Hypercholinesterasemia in patients with hepatocellular carcinoma: a new paraneoplastic syndrome.

Two cases of hepatocellular carcinoma (HCC) were described, in whom hypercholinesterasemia was found. Histochemical examinations revealed that there was a significantly increase in enzyme activity of cholinesterase in liver tissue slice obtained from the part of carcinoma in case 1. It was found that cholinesterase activity in homogenized liver tissue in part of carcinoma was much higher than that of non-carcinoma, and even in other HCC cases, hepatic cirrhosis and control liver tissues. These results suggested that HCC cells were capable of producing cholinesterase and, therefore, that hypercholinesterasemia was an additional paraneoplastic syndrome in HCC.

Cronkhite-canada syndrome associated with gastric cancer: report of a case.

A 71-year-old man with characteristic clinical and pathological pictures of Cronkhite-Canada syndrome associated with gastric cancer is presented. Histological examinations of gastric and colonic polyps which resembled juvenile type revealed no malignant cells. The patient slowly recovered after subtotal gastrectomy and elemental diet supplement with antiplasmin agents. As for as is known to the authors, a total of 69 cases with this syndrome were reported in Japan including our patient. Of these cases 8 had cancer of the gastrointestinal tract. Two of them, including our case, had gastric cancer. Another one was associated with gastric cancer but not intestinal polyposis. The remaining 5 cases were associated with colonic cancer. Caution must be paid to the fact that cachexia or malabsorption in some patients with this syndrome results from gastrointestinal malignancy. The prognosis of this syndrome consisted of 20 deaths including 2 postoperative deaths, 33 of improvement including 4 of natural remission and 4 of remission after gastrectomy or colectomy, 7 unchanged and the remainder not described.

Clinical usefulness of serum cholylglycine determination in various liver diseases.

In order to study the clinical significance of cholylglycine (CG), CG in both fasting levels and after oral loading of 300 mg ursodeoxycholic acid (UDCA) were determined by radioimmunoassay in 313 patients with various liver diseases. Elevated fasting CG levels were found in 66.6% of patients with acute hepatitis (AH), 34.8% chronic inactive hepatitis (CIH), 79.3% chronic active hepatitis (CAH), 97.1% liver cirrhosis (LC), 94.7% hepatocellular carcinoma (HCC), and 80% of gall stones (GS). Changes of fasting CG during the course of AH were found to be parallel with values of transaminases. On oral loading of UDCA, different CG responses were found in CIH and CAH, and in compensated and decompensated LC. From these results we concluded that the determinations of serum CG gave additional informations of various liver diseases.

Pathological study on gold impregnation of fat-storing cells in human liver.

Examination of 51 human liver specimens with the modified Kupffer's gold impregnation method confirmed the presence and distribution of fat-storing cells in various kinds of diseased livers such as fatty liver, acute centrolobular necrosis, subacute massive necrosis and cirrhosis as well as in liver cell carcinoma. In normal liver, gold-reactive fat-storing cells were distributed in the central area or diffusely in lobules. In the liver with marked fatty change and obstructive jaundice, presence of fat-storing cells was able to be clarified by this method. In cases of acute hepatocellular necrosis, the necrotic areas contained a large number of fat-storing cells in contrast to adjacent areas. In cases of subacute massive hepatic necrosis and cirrhosis, the areas with abundant newly formed collagen fibers (type III collagen) contained many gold-reactive fat-storing cells. In the septa consisting of dense type I collagen fibers, by contraries, fat-storing cells were hardly visible. The features suggested that fat-storing cells are closely related to intralobular fibrogenesis. In one case of liver cell carcinoma, there were many gold-reactive fat-storing cells in tumour tissue.

Experimental chronic non-suppurative destructive cholangitis in rabbits following immunization with bile duct antigen.

Experimental chronic non-suppurative destructive cholangitis induced in rabbits following immunization with bile duct antigen. Histological changes similar to chronic non-suppurative destructive cholangitis were induced in rabbits by sensitization with antigen prepared from bovine gallbladder mucous membrane. The occurrence of these pathological changes was significantly higher in female animals than in males: 11 out of 14 female rabbits and 4 among 11 male rabbits exhibited histological changes. Immunoprecipitation using gel-diffusion and histological analysis by an immunofluorescence technique showed that this antigen was localized exclusively in the bile duct but not in either hepatocytes or other tissues. It is suggested that histological changes induced following immunization with bile duct antigen are specific immunological phenomena. This interesting experimental model may be helpful in explaining the pathogenesis of primary biliary cirrhosis.

Incidence and clinical significance of HBe antigen and antibody in HBsAg-positive various liver diseases.

The HBeAg was detected in 5 of 24 patients with acute type B hepatitis (20.8%), 33 of 95 with chronic hepatitis (34.7%), 6 of 33 with liver cirrhosis (18.2%), and 3 of 39 with hepatocellular carcinoma (7.7%). On the other hand, anti-HBe was found in 4.2% of acute hepatitis, 18.9% of chronic hepatitis, 9.1% of liver cirrhosis, and 12.8% of hepatocellular carcinoma. We found that an early detection of HBeAg in patients with acute hepatitis is of no prognostic value, but its persistence may provide the earliest evidence of potential chronicity. In chronic liver diseases, HBeAg-positive cases showed remarkable fluctuations of serum transaminase levels, severe histological changes and poor responses to treatment. Many of the HBeAg-positive patients lost their initial positivity of HBeAg within six months or one year and in some cases serocoverted to anti-HBe after acute exacerbation. Follow-up study more than several years revealed that the presence of anti-HBe reflect an inactive stage and a more favorable outcome, whereas persistence of HBeAg may provide an active and continuing hepatocellular damage. From these results, we believed that serial measurements of HBeAg/anti-HBe system are useful prognostic marker in patients with HBsAg-positive liver disease.

Persistence of anti-HBs and protective efficacy after administration of hepatitis B vaccine.

To evaluate the effect of hepatitis B vaccine on the persistence of anti-HBs and its efficacy in preventing hepatitis type B, anti-HBs and anti-HBc levels were studied over a period of 5.5 years. Plasma-derived hepatitis B vaccine, containing 20 micrograms of HBsAg protein, was injected subcutaneously in 122 healthy medical staff members, followed by two identical injections 1 and 6 months later. Anti-HBs and anti-HBc levels were then measured by radio-immunoassay. The anti-HBs titres were expressed as the sample/negative (S/N) ratios, and an S/N ratio of more than 2.1 was considered positive. The mean (and s.d.) anti-HBs titre peaked 7 months after the first vaccination with an S/N ratio of 153.6 +/- 149.8, after which it decreased with time. The mean anti-HBs titre dropped to an S/N ratio of 8.0 +/- 5.1 5.5 years after the first vaccination. The percentage of vaccinees who were anti-HBs positive also gradually decreased with time after a peak of 84.2% at 7 months following the first vaccination. The percentage of vaccinees who were anti-HBs positive was 38.9% 5.5 years after the first vaccination. The anti-HBc level was not positive in all subjects during the observation period. Five vaccinated volunteers who had developed anti-HBs after the basic vaccination, but whose acquired antibody level became negative within 4.5 years following the first vaccination, were administered a booster dose of 20 micrograms of HBsAg 4.5 years after the first vaccination. Only one of these subjects did not respond to the booster vaccination.(ABSTRACT TRUNCATED AT 250 WORDS)
