[Effectiveness and timing of one shot transarterial infusion and/or transcatheter arterial embolization therapy for recurrence of postoperative hepatocellular carcinoma].

From January '81 to April '91, one shot transarterial infusion therapy and transcatheter arterial embolization therapy were performed in five patients with recurrences of hepatocellular carcinomas after surgery. The results revealed as follows: (1) Mean duration from surgery to initial arterial infusion and/or embolization therapy was 44.3 months, except one recurrence occurred in a short time. (2) The longest survival was 100 months, and mean survival was 64.8 months. One expired with multiple organ failure after arterial embolization therapy, but the others survived and could return to life in society. These results suggested that one shot transarterial infusion therapy and/or transcatheter arterial embolization therapy was effective long-term intermittent treatment for recurrence of postoperative hepatocellular carcinoma.

Further studies on protein secretion by rat submandibular glands in response to the beta 1- and alpha 1- adrenoceptor stimuli during postnatal development.

Developmental changes in salivary proteins secreted by the submandibular glands of male rats from 2 to 10 weeks of age in response to the beta 1- and alpha 1-adrenoceptor agonists, isoproterenol, methoxamine, dopamine, phenylephrine, epinephrine, norepinephrine and alpha-methylnoradrenaline, were studied by different electrophoreses and amino acid analysis. The electrophoretic patterns of proteins secreted by the glands in response to both stimuli changed dramatically every week up to 7 weeks of age, but thereafter appeared similar to those of adult submandibular salivary proteins. At 2 and 3 weeks of age, a few differences were found between the types of proteins secreted in response to two stimuli. These findings were also confirmed by an analysis of amino acid composition. The glandular levels of cGMP from 2 weeks and cAMP from 3 weeks of age were significantly elevated in response to methoxamine and isoproterenol. We conclude that functional maturation precedes morphological maturation in the submandibular glands of male rats.

Objective visual acuity testing by optokinetic nystagmus suppression.

The features of optokinetic nystagmus (OKN) suppression were investigated in various kinds of visual disturbances including those of optical, retinal, higher cortical and psychogenic etiologies. A new OKN suppression device was developed, modifying the principle of Ohm. Objective visual acuities were calculated using the visual angle of OKN suppression dots which were arranged in 12 steps corresponding to the visual acuity from 0.03 to 1.0 as determined by Landolt's method. A total of 286 cases were tested. Results were classified into four types of coincidence between objective and subjective visual acuities. The first group showed equivalent acuities. Normal subjects, cases of corneal opacity, aphakia with intraocular lens implantation and functional amblyopia belonged to this category. In the second group, objective acuity could not be determined because of fixation disturbance due to cortical lesion. The subjective acuity better than the objective acuity was obtained in the third group which consisted of cases with macular hole, central serous retinopathy and optic neuritis with central scotoma. Disturbance of foveal fixation to the OKN suppression dot seemed to be the reason for this difference in the acuities. In the fourth group, the objective acuity was better than the subjective acuity. This occurred in cases of psychogenic visual disturbance and malingerer.

[Delayed development of binocular function in the long postoperative period of infantile esotropia].

Long-term observation of binocular function was carried out in 30 postoperative cases of infantile esotropia. The period of observation was 5 to 12 years and the targets used for evaluation were deviating angle and stereo acuity. In the successful group, 20 cases out of 30 (67%) achieved a stereo acuity of 60 seconds or better. Among the remainder, partial improvement was seen in 20% and no improvement in 13%. Preexisting amblyopia and ARC improved in the successful group, and the final deviating angle was a mean of 2.3 prism diopter. There were 2 types of cure processes in the successful group. The first group achieved a good binocular function in a relatively early postoperative stage (7 cases), while the second group had delayed development of binocular function over a period of several years (13 cases). The former cases had sufficient orthoptic treatment during admission and the latter showed a gradual improvement of stereo acuity accompanied by good eye position. It has been generally accepted that the rate of binocular function recovery is very poor among cases of infantile esotropia, but our study revealed that skillful orthoptics administered by a selected member of our team over an extended period enabled the achievement of good binocular function in infantile esotropia.

A comparative study of the localization of vasoactive intestinal peptide- and substance P-like immunoreactivities in the submandibular salivary glands of mice.

The localization of vasoactive intestinal peptide (VIP)- and substance P (SP)-like immunoreactivities within the submandibular salivary glands (SMG) of Balb/c male mice at 4 to 5 weeks of age were investigated. Two immunocytochemical methods using polyclonal, monospecific antibodies, the indirect immunofluorescence and peroxidase-antiperoxidase (PAP), were used to elucidate the localization of VIP- and SP-like immunoreactivities in the SMG. Both VIP- and SP-like immunoreactivities were positive in the SMG of Balb/c male mice at 4 to 5 weeks of age. The VIP-like immunoreactivity was found in the dense plexuses around the salivary duct and in the small scattered nerves surrounding glandular acini, and particularly in the nerve cell bodies of the submaxillary ganglion. The SP-like immunoreactivity was conspicuously abundant in the excretory ducts with a rich supply of SP-positive nerves, but also very scanty in the acinus areas close to the striated ducts and interlobular excretory ducts. It could be conclusively observed that the localization of the VIP- and SP-like immunoreactivities were different in the submaxillary ganglion, in the nerves around blood vessels and acinus areas, but very similar in the areas of the excretory and striated duct of the SMG.

Short latency visual evoked potentials in functional amblyopia shown using moving topography.

The conduction from the eye to the visual cortex through the brainstem was investigated in 43 cases of functional amblyopia by means of moving topography of short latency visual evoked potential (SVEP). Anomaly of the SVEP such as a defect of the main component and remarkable reduction of amplitude was found in cases with severe amblyopia whose visual acuity was less than 0.5. The incidence of abnormal SVEP was recognized in 44% of anisometropic amblyopia (18 cases), 52% of strabismic amblyopia (15 cases), and 50% of deprivation amblyopia (10 cases). The results suggested a reduction of electric activity in the subcortical visual pathway including transmitting nuclei.

Moving topography of human presaccadic spike potentials in visually triggered saccade and optokinetic nystagmus.

The spatio-temporal nature of presaccadic spike potentials (SP) associated with the visually evoked saccade and with the fast phase of optokinetic nystagmus was investigated by means of moving electroencephalographic topography. In both cases, SPs were recognized about 15-25 msec before the quick eye movements (peak time 3-7 msec). The amplitude of SPs was +5 to +7 microV for the visually guided saccade and +3.5 to +4.5 microV for optokinetic nystagmus. The location of the SPs was the mid-parieto-occipital region on the scalp, but their topographical character was that of a brainstem potential. In the case of the visually guided saccade, before the appearance of SP, an antecedent potential in the form of a slow positive charge appeared in the parietal region. From their spatio-temporal characteristics, the SPs investigated in this study were interpreted as mass potentials of the brainstem nuclei which generate eye movements.

Topographic visually evoked potentials induced by stereoptic stimulus.

Topographic processes of brain activity during stereopsis were investigated by means of two different principles, with a real stereo target and a computer stereogram. Use of either principle produced the same tendency: an electrically negative focus started from the central region of the scalp and moved to the parietal and occipital regions. These flows of excitation were seen during a period of 90 to 170 ms. The difference between these two stimulus represented a return of the negative focus from the occipital pole to the parietal region in the real stereo target and a spread of the negative focus to the temporal region in the computer stereogram. Since monocular viewing of a real stereo target produces a similar visually evoked potentials wave form but with less intensity, the negative focus in binocular viewing may be due to the enhancement of binocular cells and disparity sensitive neurons in a wide area of the brain cortex. Thus stereoptic brain responses start from the central and parietal regions and move to the occipital region, making a flow of excitation.

[Endobronchial metastasis of testicular embryonal cell carcinoma--a case report].

Four cases of endobronchial metastasis in testicular embryonal cell carcinoma have been reported in the literature. We now present a new case, a 39-year-old male who underwent right orchiectomy for testicular embryonal cell carcinoma. Ten months later, tumor like shadows were identified in the left upper lobe on a follow up chest X-ray. Bronchoscopy revealed an endobronchial tumor mimicking a bronchogenic carcinoma in the left superior posterior bronchus. A metastatic embryonal cell carcinoma of the testis was histologically diagnosed. Left upper lobectomy was performed, and the patient is still alive 11 months thereafter.
