Complete 5' noncoding region is necessary for the efficient internal initiation of hepatitis C virus RNA.

The mechanism of translational initiation by the 5' noncoding region (5'NCR) of hepatitis C virus (HCV) genome was analyzed. Using an in vitro translation system with artificial RNA containing a modified 5' NCR of HCV under the various KCl conditions, nucleotides (nt.) 62 to 341 of the HCV 5'NCR were not functional as an internal ribosome entry site (IRES). However, the full-length 5'NCR (nt. 1 to 341) produced an efficient internal initiation. To identify the essential region of the HCV-IRES, various mutants were produced in which stem-loops, predicted by secondary structure analysis of the HCV 5'NCR, were deleted. These constructs were analyzed by in vitro translation. Comparison of translation efficiency among these mutants suggested that the alpha- or both alpha- and beta-branches of domain II are essential for efficient translation. Moreover, the formation of correct secondary structure of IRES seems to be stabilized by the presence of domain I in 5'NCR. Furthermore, the uncapped 5'NCR of HCV promotes translation more efficiently than capped truncated 5'NCR constructs. Our results strongly suggested that complete 5'NCR containing all stem-loop structures is necessary for initiation by HCV-IRES.

Grafting of autologous-cultured epithelium after excision of tattoos.

Nine patients with tattoos were treated with cultured epidermal autografts. The tattoos were excised down to the deep dermal layer, and the wounds were covered with cultured epidermal autografts. The areas grafted with cultured epidermal autografts were an average of 907 cm2 in size. The average percentage "take" of the cultured epidermal autografts was 86%. The average time for complete wound epithelialization was 20 days. The pain at the grafted sites was minimal. There was minimal scarring and no tendency for scar contracture. The appearance and elasticity of the grafted sites resembled those of adjacent normal skin 2 1/2 years after grafting. These findings demonstrate that cultured epithelium autografting is a suitable method for coverage of the tangentially excised tattoo wound.

Association between a cytochrome P450 CYPIA1 genotype and incidence of lung cancer.

We analysed 267 samples from patients with lung cancer and 54 samples from patients with pancreatic cancer and classified them into three types (A, B and C) based on their P450CYPIA1 genotypes. This was performed by restriction fragment length polymorphism (RFLP) analysis of DNA (that had been amplified) using the polymerase chain reaction (PCR). Our results reconfirmed previous studies with RFLP analyses by revealing significantly higher frequencies of C type in lung cancer patients, particularly light smokers with squamous cell carcinoma (p < 0.001). Also when we compared the appearance rate of P450CYPIA1 genotype in lung cancer patients between metastasis group and no metastasis group, lymph node or distant metastasis was more frequently observed among the patients of squamous cell carcinoma, especially the light smoking group, with C genotypes (p < 0.05). These results suggested that this genotype is not only associated with the etiology of lung cancer but also with the status of metastasis. Furthermore, genotyping could be useful in predicting the incidence of lung cancer in the smokers and the general population as well as the prognosis of individuals who develop this disease.

[Anesthetic experience of emergency cesarean section for a patient with myotonic dystrophy].

We report an anesthetic experience of a 28-year-old female patient complicated with myotonic dystrophy who underwent emergency Cesarean section due to threatened abortion. Anesthesia was induced with intravenous thiopental followed by topical spray of 4% lidocaine 5 ml to intubate trachea and maintained with neuroleptanesthesia with droperidol, fentanyl and nitrous oxide in oxygen. No muscle relaxant was used. The course of anesthesia and emergence from anesthesia were uneventful. However, the female infant, with Apgar score of 1 point at five minutes after delivery, died due to multiple organ failure three weeks after the delivery. Anesthetic management of a patient with myotonic dystrophy was also discussed with a literature review.

Correlation of L-myc RFLP with metastasis, prognosis and multiple cancer in lung-cancer patients.

For further study of the correlation of L-myc restriction-fragment-length polymorphism (RFLP) and metastasis of lung cancer to lymph nodes or other organs at the time of surgery, L-myc RFLP was analyzed in 252 Japanese lung-cancer patients. A close correlation between L-myc RFLP and metastasis was confirmed in this large number of patients (p = 0.01). The correlation was particularly pronounced in cases of adenocarcinoma and squamous-cell carcinoma. Poor prognosis (additional metastases after surgery) was observed in lung-cancer patients with L-S (identified as long and short bands produced with EcoRI) and S-S type L-myc RFLP. In addition, the death rate of lung-cancer patients with the L-S and S-S types was greater than that of those with the L-L type. Lung-cancer patients of the L-S and S-S types had almost 4 times higher incidence of multiple cancer in the lung, pharynx and other organs than those with the L-L type. Our results indicate that, in patients with lung cancer, genetic disposition with respect to the L-myc gene influences the extent of metastasis, the incidence of multiple cancers and prognosis.

Epidural beta-endorphin in treatment of pain.

Epidural administration of 3 mg of synthetic beta-endorphin produced analgesia in 10 patients with intractable pain due to disseminated cancer. Mean onset of relief of pain was 24 +/- 3 minutes and the mean duration of analgesia was 19 +/- 3 hours. The onset of analgesia produced by the epidural injection of beta-endorphin was slower and the duration less than those observed after intrathecal injection.
