Malignant fibrous histiocytoma of the nasal septum. A report of an unusual lesion.

A case of malignant fibrous histiocytoma of the nasal septum is described in a 51-year-old woman who has presented with nasal pain, nasal obstruction and a swelling at the base of the nose. Histological examination revealed a malignant fibrous histiocytoma. Total resection of the tumor was performed and, 26 months after operation, the patient was well and free of recurrence or metastasis. This is the first reported case of malignant fibrous histiocytoma of the nasal septum.

Congenital ossicular anomalies without malformations of the external ear.

Eighteen ears of congenital ossicular malformation without deformities of the external ear were presented. They were classified into three groups; eight incudostapedial joint separations, three malleus and/or incus fixations, and seven stapes fixations. The surgical correction for the first group was to connect the malleus handle to the stapes using a silicon tube specially designed or a gelfoam wire in the uncustomary way. This group showed the best surgical results with 35.4 dB of the average hearing gain in speech frequencies. In the second group, removal of the fixed part of the ossicles yielded satisfactory results, but mobilization failed to improve the hearing. For the group of stapes fixation, stapedectomy and the gelfoam wire prosthesis were performed. The result was not satisfactory in some cases with the undeveloped oval window. The embryological consideration leads us to the following conjecture; the incudostapedial joint separation results from the failure in build-up of "the secondary continuity", the malleus and/or incus fixation is the result of disturbances in separation process by the undifferentiated mesenchyme, and the stapes fixation is due to maldevelopment of the stapedial lamina. This classification for the congenital ossicular malformation is practically valid as it would suggest the maldeveloped point in ossicular genesis and results of surgical correction.

Cochlear and fast electrical responses to frequency modulated tones: a frequency specific stimulus.

Some characteristics of "the fast response" to frequency modulation were explored here by examining (1) CM and AP in animal experiment and (2) BER in human study. CM to frequency modulated tone showed frequency change and amplitude change, the latter of which is probably due to frequency characteristics of the sound conductive system of the ear. Compared with AP to tone burst, latency and amplitude of AP to FM tone depends less on stimulus intensity. AP to FM tone is suspected to be a result of new excitement of the restricted area stimulated by shifted frequency. The result of analysis on human BER is well consistent with the result of AP study in animal experiment, and latency curves of BER were to some extent linear to a log scale of shifted frequencies. Thus, FM tone is possibly a frequency specific stimulus.

Intraoral nerve block for glossopharyngeal neuralgia.

Glossopharyngeal neuralgia was treated by a new method of intraoral block. The procedure consists of block for the tonsillar branch and block of the lingual branch by the injection of 5% Phenol in Glycerin. This can be applied for neuralgia with a trigger zone around the tonsil. The result is quite satisfactory. This is extremely safe and simple, and can be performed repetitively when recurrences occur. Also, this procedure does not preclude later operations as a final step.
