[Diagnosis of aortocoronary bypass complications].

Thirty-four patients with ischemic heart disease were examined during operation for revascularization of the myocardium. The blood flow in the shunts was measured during the change from extracorporeal to natural circulation. In rise of venous and arterial pressure the blood flow in the shunts increased (positive dynamics) in some patients and decreased (negative dynamics) in others. Positive dynamics were associated with a favourable course of the postoperative period, negative dynamics--with ischemic-type complications. Factors invalidating the myocardium aggravated the initial condition of patients with positive dynamics of the blood flow less than that of patients with negative dynamics. Comparison of dynamic and short-term measurement of the blood flow in the shunts as a diagnostic test for perioperative complications showed the advantage of the dynamic test.

[Universal system for cardioplegia].

A universal system for the delivery of cardioplegia solutions has been described. The use of the system makes it possible to conduct all the kinds of cardioplegia, i. e. antegrade, retrograde, combined and shunt cardioplegia (the combination of the first and second types with the delivery of cardioplegia solutions via coronary anastomoses). The system is a distributor with six taps, each of them supplied by a cannula for the delivery of cardioplegia solution via autovenous shunts via the aortic root and coronary sinus. Besides, at the stage of lateral aorta clamping (during aortocoronary bypass surgery) the delivery of oxygenated blood from cardiopulmonary bypass apparatus to the respective myocardial segments via autovenous shunts is possible. The above system improves even distribution and cooling of various myocardial zones, especially in the area of affected coronary arteries and makes it possible to shorten the period of warm-induced ischemia of myocardial shunts.

[The effect of fluorinated prostaglandins on platelet aggregation].

The effects of fluorodeoxy prostanoids on platelet aggregability were studied. It was shown that introduction of fluorine into positions 9, 11 or 15 of prostaglandin F2 alpha led to enhanced proaggregation activity. The most active compound among fluorodeoxy analogs was 15-fluoro derivative; bisfluoro analog was moderately active, and 11-fluoro compound had the least activity. In the group of fluorodeoxy prostaglandins E2, a contrary effect was registered. Thus, the most active compound was 1-fluoride and the least, 15-fluoride. The incorporation of fluorine into position 15 of prostacyclin led to insignificantly lower antiaggregatory activity just as this modification of 6-keto-prostaglandin F1 alpha was accompanied by a dramatic increase in its ability to inhibit platelet aggregation.
