Activation of blood coagulation in patients with mitral stenosis and sinus rhythm.

The aim of this study was to assess whether there is blood coagulation activation in patients with mitral stenosis (MS) and sinus rhythm (SR) and to investigate the value of left atrial spontaneous echo contrast (LASEC) as a predictive sign of increased coagulation activity. Using thrombin-antithrombin III complexes and prothrombin fragment 1+2 as in vivo hemostatic markers, we concluded that there is a hypercoagulable state in patients with MS and SR when LASEC is present.

Dispersion of repolarization in paroxysmal atrial fibrillation.

Widened dispersion of refractoriness has been considered to lead to an increase in atrial vulnerability. In this study, we obtained simultaneous monophasic action potential recordings by 2 special catheters from 2 different loci in the right atrium in 9 normal subjects (Group 1) and in 7 patients with paroxysmal atrial fibrillation (Group 2). We measured action potential duration at 50% (APD50) and 90% (APD90) repolarization from both loci in the right atrium during steady-state through one of the two catheters. The differences between APD50 and APD90 recorded by the two catheters were designated as dispersion of APD50 and dispersion of APD90, respectively. While, the mean APD50 and APD90 were 149.7+/-16.9 and 228.3+/-44.7 ms respectively in Group 1, the corresponding values for Group 2 were 145.7+/-33.5 and 213.5+/-53.1 ms. The difference between the 2 groups was not statistically significant. However, the dispersion of APD50 and APD90 were 13.8+/-13.8 and 10.0+/-9.7 ms in Group 1 and 42.8+/-19.6 and 57.1+/-32.4 in Group 2, respectively and the difference between the 2 groups for both measurements was statistically significant (P<0.01). Another finding was a high correlation between age and dispersion of APD90 in the whole population studied (r=0.82, P<0.001). With these findings, although one can not derive the conclusion that patients with paroxysmal atrial fibrillation have a greater dispersion of repolarization, our finding of increasing dispersion of repolarization with age could be an explanatory factor for the increased prevalence of atrial fibrillation with advanced age.

Changes in knot-holding capacity of sliding knots in vivo and tissue reaction.

OBJECTIVES: To evaluate and compare the in vivo strength, knot efficiency, and knot security of 4 types of sliding knots, and to assess tissue reaction to study the effect of knot configuration, knot volume, and suture size. DESIGN: Randomized trial. SETTING: Experimental Medical Research Institute, Istanbul, Turkey. SUBJECTS: Wistar rats. INTERVENTION: To assess the tissue reaction, a midline laparotomy incision was made in 112 rats and sutured with various interrupted knots in silk and nylon sutures of 2/0 and 4/0 (United States Pharmacopeia) sizes. Suture loops were implanted in subcutaneous pouches in the rat abdomen. Sutures were all extracted at days 4, 7, 11, and 20 to determine their knot-holding capacity. MAIN OUTCOME MEASURES: Knot efficiency and percentage decrease in knot-holding capacity were examined. Knot, tissue reaction, and inflammatory sheath volumes were measured. RESULTS: The 4/0 knots lost more strength than the 2/0 knots. The alternating knots with different patterns were more efficient and secure than the simple alternating ones. The alternating parallel knot was found to be unreliable. The tissue response to all the knots, except 2/0 nylon, was similar. The inflammatory sheath volume varied depending on the knot volume, suture size, and knot configuration. CONCLUSION: The use of alternating sliding knots with different patterns is recommended to replace simple alternating sliding knots.

Adenosine sensitive left ventricular tachycardia.

Presented here is a 39-year-old male patient with no structural heart disease but a ventricular tachycardia with right bundle branch block morphology and right axis deviation, which is responsive to adenosine. The ventricular tachycardia was initiated by ventricular pacing, shown to originate from the mid-anterior free wall region of the left ventricle and terminated by adenosine. Radiofrequency current application at a site where presumed P potentials were recorded eliminated the tachycardia, a finding that suggests that the origin of the tachycardia may be closely related to the anterior fascicle.

Septal myectomy in hypertrophic obstructive cardiomyopathy: late results with stress echocardiography.

BACKGROUND: This study was performed to assess the functional capacity of the survivors of septal myectomy for the treatment of hypertrophic obstructive cardiomyopathy in long-term follow-up as assessed by dobutamine stress echocardiography. METHODS: Sixty-nine patients with hypertrophic obstructive cardiomyopathy underwent septal myectomy between 1975 and 1996. The mean age was 25.4 +/- 13.6 years (range, 6-58 years), and 10 of the patients were women. The early mortality was 4.3%. Hospital survivors (95.7%) were followed up for a mean of 43.8 +/- 28.7 months (range, 6-114 months). RESULTS: The postoperative mean functional capacity of the group was 1.47 +/- 0.56. No late deaths were reported. Forty-nine patients (74.2%) were evaluated with standard echocardiographic techniques, and 29 (43.9%) patients underwent dobutamine stress echocardiography. There was a significant decrease in the thickness of the interventricular septum after surgery. The mean preoperative and postoperative septal thickness was 1.99 +/- 0.59 cm (range, 1.3-3.8 cm) and 1.55 +/- 0.41 cm (range, 0.96-2.8 cm), respectively (p < 0.004). The mean posterior wall thickness was significantly less than the preoperative value (p = 0.008) and the left ventricular end-diastolic diameter was slightly greater in the postoperative measurements, but the difference was not significant (p = 0.162). Postoperative left ventricular outflow systolic gradients were reduced significantly when compared with preoperative values (preoperative mean, 78.4 +/- 33.6 mm Hg, range, 50-212 mm Hg versus postoperative mean, 17.9 +/- 15.9 mm Hg: range, 0-40 mm Hg; p < 0.0001). CONCLUSION: Septal myectomy for patients with hypertrophic obstructive cardiomyopathy is a safe procedure with excellent clinical and functional results in the long-term follow-up.

Helicobacter pylori infection in a child with gastric augmentation.

The existence of Helicobacter pylori (HP) infection in those children who had previous bladder augmentation with gastric patch has not been described before. In this report, a girl with bladder exstrophy, who previously underwent gastric augmentation is presented. She had multiple admissions to our unit with persisting urinary symptoms (perineal pain, dysuria, hematuria) and gastric symptoms as well. Enzyme-linked immunosorbent assay (ELISA) test results for HP infection were positive, and plasma gastrin level was high. The histopathologic examination of the biopsy specimen showed HP infection in the gastric part of the reconstructed neobladder. This report indicates that the pediatric surgeon should think about HP infection in gastrocystoplasty patients to eliminate the potential risks of HP colonization.

Comparison of exercise stress testing with dobutamine stress echocardiography and exercise technetium-99m isonitrile single photon emission computerized tomography for diagnosis of coronary artery disease.

To compare the value of exercise electrocardiography with dobutamine stress echocardiography and exercise technetium-99m isonitrile single-photon emission computed tomography for coronary artery disease, 70 patients with either suspected or proven coronary artery disease underwent dobutamine stress echocardiography, exercise technetium-99m isonitrile single-photon emission computed tomography (mibi-SPECT) and treadmill exercise electrocardiography (ECG). Dobutamine echocardiography and exercise mibi-SPECT revealed a higher overall sensitivity than exercise testing (90 vs 57%, p < 0.001; 96 vs 57%, p < 0.001, respectively). Dobutamine stress echocardiography showed a higher specificity than both exercise mibi-SPECT and treadmill exercise electrocardiography (90 vs 71%, p > 0.05; 90 and 62% p < 0.05, respectively) but the difference between dobutamine stress echocardiography and exercise mibi-SPECT was not statistically significant. Diagnostic accuracy of dobutamine stress echocardiography and exercise mibi-SPECT was higher than that of exercise testing (90 vs 59%, p < 0.001; 89 vs 59%, p < 0.001, respectively). Dobutamine stress echocardiography and exercise mibi-SPECT have superiority over exercise testing in the diagnosis of coronary artery disease and dobutamine stress echocardiography is an alternative for exercise mibi-SPECT.

Genomic instability in colorectal cancers in Turkey.

Microsatellite instability in a subset of colorectal cancers from North America, Europe and Japan has been reported. We examined 88 colorectal cancers from Turkish patients. Five different microsatellite loci (1 mono- and 4 dinucleotide repeat regions) were tested. Eight tumors displayed replication errors (RERs) with at least 2 different markers. Right-sided tumors showed significantly higher frequency of microsatellite instability compared with left-sided tumors. The frequency of RER phenotype was slightly higher in tumors occurring in younger (<50 years old) than in older patients (13% vs. 8%), and there was no association between sex and genomic instability. The frequency of genomic instability in our study group was 9%, whereas the reported frequencies in tumors from other countries varied between 12% and 16%.

Prevalence and predictors of atrial fibrillation in rheumatic valvular heart disease.

The highest frequency of AF in RHD occurs in those with mitral stenosis, mitral regurgitation, and tricuspid regurgitation in combination. AF, while occurring in 29% of patients with isolated mitral stenosis and in 16% with isolated mitral regurgitation, is an infrequent finding (1%) in patients with aortic valvular disease. Left atrial diameter by univariate analysis, and age and left atrial diameter by multivariate analysis have been shown to be the most important parameters to determine the occurrence of AF in patients with RHD.

Fistulous connection between the left pulmonary artery and the innominate vein.

A 66-yr-old woman was admitted to our clinic with signs and symptoms suggesting either pulmonary embolism or pneumonia. Subsequently she was diagnosed as having pneumonia, but since pulmonary artery pressure was high on echocardiography, pulmonary angiography was performed to exclude the possibility of pulmonary embolism. No findings suggestive of pulmonary embolism were evident on pulmonary angiogram, but there was a fistulous connection between the left pulmonary artery and the innominate vein. To our knowledge, this report represents the first case of a fistula between the pulmonary artery and a systemic vein.

Comparison of exercise stress testing with simultaneous dobutamine stress echocardiography and technetium-99m isonitrile single-photon emission computerized tomography for diagnosis of coronary artery disease.

The object of our study was to compare the value of exercise stress testing with simultaneous dobutamine stress echocardiography and technetium-99m isonitrile single-photon emission computed tomography for the diagnosis of coronary artery disease. Sixty-nine patients with either suspected or proven coronary artery disease underwent simultaneous dobutamine technetium-99m isonitrile single-photon emission computed tomography and stress echocardiography, and treadmill exercise electrocardiography. Dobutamine echocardiography and technetium-99m isonitrile single-photon emission computed tomography revealed a higher overall sensitivity than exercise testing (94 vs 60%, P < 0.001), but dobutamine stress echocardiography showed a higher specificity than both technetium-99m isonitrile single-photon emission computed tomography and exercise testing (86 vs 64%, P < 0.05, for both tests). In addition, the diagnostic accuracy of dobutamine stress echocardiography and technetium-99m isonitrile single-photon emission computed tomography was higher than that of exercise testing (91 vs 61%, P < 0.001; 86 vs 61%, P < 0.001, respectively). Dobutamine stress echocardiography and technetium-99m isonitrile single-photon emission computed tomography are superior to exercise testing in the diagnosis of coronary artery disease, and dobutamine stress echocardiography can act as an alternative to technetium-99m isonitrile single-photon emission computed tomography.

[Syndrome of solitary rectal ulcer--a rare cause of recurrent rectal hemorrhage].

While the clinical symptoms and signs of SRUS are quite obvious, etiology and treatment of the disease remain to be obscure. Although the lesion is usually not solitary this name has been used for the disease. According to other authors, we also find this term inappropriate. During the last 5 years 6 cases of SRUS have been diagnosed, treated and prospectively assessed in the surgical clinic of Cerrahpaşa Medical Faculty (Istanbul). Three patients were treated successfully by conservative means, transanal excision was performed and defects were sutured primarily in the other three. In one patient recurrence was seen 8 days after the transanal excision; transabdominal rectopexy was performed 6 months later. All patients have been followed up with no complaints.

Lentiginosis and right atrial myxoma.

A right atrial myxoma was detected by cross-sectional echocardiography in a 21-year-old woman with cutaneous lentiginosis. The lentigines were present mainly on the right side of the trunk.

Peritoneal cytology in the determination of free tumour cells within the abdomen in colon cancer.

The presence of free tumour cells at the mesothelial surface in patients with intraperitoneal colon cancer is an important prognostic factor and may alter the decision regarding adjuvant chemotherapy. In this prospective study of 72 patients we aimed to show the existence of malignant cells in the peritoneal cavity by scraping the serosa and by peritoneal lavage. Intraoperative peritoneal cytology was performed in 72 colon cancer patients who underwent curative surgery in our department between 1992 and 1995. Specimens were obtained by peritoneal lavage and by scraping the serosa overlying the major tumour mass and an area of normal serosa at least 10 cm from the primary site. Slides were stained with haemotoxylen-eosine stain. Statistical analysis was performed with chi-squared and Fisher's exact tests. In conclusion, peritoneal washing and serosal cytological study appeared to be a simple and reliable method to detect serosal involvement and the existence of malignant cells in the peritoneal cavity.
