Semi-quantitative RT-PCR assessment of molecular markers in breast large-core needle biopsies.

Large-core needle biopsies are frequently used for the preoperative evaluation of the breast lesions. In addition to initial diagnostic information, they can show the status of molecular markers with predictive and prognostic value and further contribute to an optimal selection of treatment strategy. So far, the potential use of large-core needle biopsies in assessment of marker profile of the breast lesions was studied using the immunostaining approaches. In this work, we sought to determine whether analysis of the large-core needle biopsy by semi-quantitative reverse-transcription polymerase chain reaction reveals molecular data that correspond with the marker status of the subsequently removed tumor. Five molecular markers including ER, PR, c-erbB-2/HER-2/neu, c-erbB-3/HER-3, and CD44 were assessed on mRNA isolated from 23 large-core needle biopsies and corresponding surgical breast cancer specimens using beta2- microglobulin as an internal standard. Significant or highly significant correlation between core biopsies and excised tumors was observed for each marker when the mRNA expression status was scored as positive or negative, with the concordance of the data ranging from 73.9% to 86%. Using the dichotomous scoring, majority of the biopsies (75%) displayed molecular profile that was either identical to the profile of the related tumor specimen, or with the difference in one marker. However, no significant correlation was found when the levels of the markers were expressed as continuous variables, possibly due to intratumoral cell heterogeneity. These results suggest potential usefulness and reliability of semi-quantitative RT PCR in the evaluation of large-core needle biopsies with regard to marker positivity or negativity. On the other hand, the marker-related data expressed as continuous variables cannot be accurately assessed on the large- core needle specimens using this approach, indicating the need for methodological improvements.

Hepatoprotective effect of rooibos tea (Aspalathus linearis) on CCl4-induced liver damage in rats.

Hepatoprotective properties of rooibos tea (Aspalathus linearis) were investigated in a rat model of liver injury induced by carbon tetrachloride (CCl(4)). Rooibos tea, like N-acetyl-L-cysteine which was used for the comparison, showed histological regression of steatosis and cirrhosis in the liver tissue with a significant inhibition of the increase of liver tissue concentrations of malondialdehyde, triacylglycerols and cholesterol. Simultaneously, rooibos tea significantly suppressed mainly the increase in plasma activities of aminotransferases (ALT, AST), alkaline phosphatase and billirubin concentrations, which are considered as markers of liver functional state. The antifibrotic effect in the experimental model of hepatic cirrhosis of rats suggests the use of rooibos tea as a plant hepatoprotector in the diet of patients with hepatopathies.

[Primary hemochromatosis].

Authors describe in the case report a development of the primary hemochromatosis. At the time of the diagnosis in 1979 from organ complication the liver fibrosis and secondary diabetes mellitus were presented. Patient was treated with the venepunctions and with desferoxamine. In the 1998 the primary hepatocellular tumor was found and resected. Instead of the chemotherapy occurred the progression of the disease with multiple metastases and liver insufficiency and 66-years old patient died 19 months after surgery in the 20th year of the disease.

Effects of pleuran (beta-glucan isolated from Pleurotus ostreatus) on experimental colitis in rats.

The effects of pleuran, beta-1,3 glucan isolated from Pleurotus ostreatus, were studied in a model of acute colitis induced by intracolonic administration of acetic acid. There was a reduction of the colonic damage score, colonic wet weight and wet/dry weight ratio 48 h after single luminal 2% pleuran suspension pretreatment. Similar results were obtained after repeated intraperitoneal administration of pleuran in doses of 30 and 100 mg/kg. Pleuran given orally as a 10% food component over 4 weeks was effective in reducing the extent of mucosal damage, but did not prevent the increase of myeloperoxidase in the injured colonic segment. In the segment without macroscopic evidence of inflammation, myeloperoxidase activity was significantly lower as documented by histological examination. The results indicate a possible role of this immunomodulator in the treatment of ulcerative colitis.

The effect of red beet (Beta vulgaris var. rubra) fiber on alimentary hypercholesterolemia and chemically induced colon carcinogenesis in rats.

The effect of diet supplemented with 5% and 15% cellulose or with 15% fiber isolated from red beet (Beta vulgaris var. rubra) on the development of alimentary hypercholesterolemia and chemically induced colon carcinoma was studied in male Wistar rats. Hypercholesterolemia was induced by a diet containing 0.3% of cholesterol and colon carcinoma was induced by treatment with dimethylhydrazine (20 mg/kg, 12 doses applied s.c. in one-week intervals). Fibrous matter isolated from red beet contained 89% fiber, of which 9% was in water soluble form. Animals were killed 14 weeks after the application of dimethylhydrazine (i.e. 26 weeks after starting on the diets). Red beet fiber diet (and not the increased cellulose intake) caused a reduction of serum cholesterol and triacylglycerol levels (by 30 and 40%, respectively) and a significant increase in the fraction of cholesterol carried in HDL. This diet induced also a significant decrease (almost by 30%) of cholesterol content in aorta. Higher cellulose content in the diet and even more so the administration of red beet fiber caused a significant reduction of conjugated dienes content in plasma, erythrocytes and in liver. Also observed were increases in the activities of superoxide dismutase and catalase in erythrocytes and in colon and activities of glutathione peroxidase and glutathione-S-transferase in liver. The presence of both higher cellulose content and red beet fiber in the diet significantly reduced the incidence of precancerous lesions--aberrant crypt foci--in the colon. The diet containing red beet fiber did not affect significantly the incidence of colon tumours although the number of animals bearing tumours was reduced by 30%.

Laryngeal hamartoma: surgical management.

A newborn female infant with severe inspiratory stridor was found to have a laryngeal non-encapsulated hamartoma in the supraglottic area, medial to the hyoid bone and extending into the petiole of the epiglottis. Histologic examination revealed an extremely uncommon glandular hamartoma. Surgical management in the newborn period consisted of conservative endoscopic excision combined with open laryngofissure. The hamartoma was removed in the newborn period to avoid tracheostomy, because decanulation after tracheostomy in infants can be difficult.

Hypocholesterolemic and antiatherogenic effect of oyster mushroom (Pleurotus ostreatus) in rabbits.

The addition of 10% dried fruiting bodies of oyster mushroom (Pleurotus ostreatus) to the diet containing 1% of cholesterol reduced serum cholesterol levels by 65% and cholesterol content in liver, heart, long extensor muscle and aorta of male rabbits (Chinchilla) by 60, 47, 25 and 80%, respectively. The decrease in total serum cholesterol was affected primarily (by 70%) by reduced cholesterol content in very-low-density lipoproteins (VLDL) while the contribution of high density lipoproteins (HDL) cholesterol increased by a factor of 3. Oyster mushroom diet reduced by 60-70% the content of conjugated dienes in plasma, erythrocytes and liver, however, it did not affect significantly the activities of antioxidant enzymes. Oyster mushroom diet reduced significantly the incidence of atherosclerotic plaques as estimated by sudanophilia (absence of positive-reaction in 3 of 5 animals) as well as plaque size (26% vs. 2% of the area with positive reaction in control and oyster mushroom-treated animals, respectively). While all animals on control diet showed atherogenic changes in aorta, oyster mushroom diet prevented the development of these changes in three animals. Fatty streaks and fibromatous plaques were found in the remaining two animals from this group. Oyster mushroom prevented the formation of atheroma plaques (found in three cases from the control diet group) and reduced the incidence of segmental injury of coronary arteria and of focal fibrosis of myocardium. Oyster mushroom caused lower incidence of foam cells in all types of lesions.

[The oyster mushroom (Pleurotus ostreatus) effectively prevents the development of atherosclerosis in rabbits].

The addition of 10% dried fruiting bodies of the oyster mushroom (Pleurotus ostreatus) to the diet containing 1% of cholesterol reduced serum cholesterol levels by 65% and cholesterol content in the liver, heart, long extensor muscle and aorta of male rabbits (Chinchilla) by 60; 47; 25 and 79%, respectively. Oyster mushroom diet reduced the content of conjugated dienes by 60-70% in the plasma, erythrocytes and liver. However, it did not significantly affect the activities of antioxidant enzymes. [Paget's disease of bone--treatment with alendronate, calcium and calcitriol].

In the submitted case-history the authors describe the clinical picture, diagnosis and treatment of the non-complicated form of Paget's disease in a 68-year-old patient with typical X-ray findings on the vertebrae, pelvis and left tibia. The laboratory findings revealed liminal alkaline phosphatase (ALP) values, 2.5 times raised values of the aminoterminal telopeptide of collagen I in the organic bone matrix (NTx) in the 24-hour diuresis and slightly elevated osteocalcin (OSCA) levels. The finding of whole-body skeletal scintigraphy was atypical and thus was no diagnostic asset. Aimed transiliacal bone biopsy according to Bordier taken from the pathological focus confirmed the diagnosis of morbus Paget. The patient was treated for six months with the following preparations: Fosamax (monosodium alendronate tablets a 10 mg, Merck Sharp and Dohme, USA) 1 tablet per day, Vitacalcin (calcium carbonate, tablets a 250 mg elemental calcium, Slovakofarma, Slovak Republic) 2 x 1 tablet per day, Rocaltrol (calcitriol tablets a 0.25 ug, Hoffman La Roche, Switzerland) 1 tablet on alternate days). Because of gastrointestinal complaints the authors did not administer 40 mg alendronate per day, as recommended in the American literature. Based on the laboratory finding of normalization of the bone turnover, reduction of the number of osteoplastic foci on the X-ray image of vertebrae and pelvis, regression of the vertebrogenic algic syndrome and improved mobility of the patient, the authors consider the 6-month administration of alendronate, 10 mg per day, calcium carbonate and calcitriol therapeutically effective and successful.

Dose- and time-dependent hypocholesterolemic effect of oyster mushroom (Pleurotus ostreatus) in rats.

The effect of the dose of oyster mushroom in the diet (1.0, 2.5, and 5.0%) and of the period of application (8, 16, 28, and 52 wk) on cholesterol accumulation in blood and body organs was studied in weanling male Wistar rats fed a diet containing 0.3% cholesterol. Reduction of cholesterol in serum and body organs was found to be dependent on the amount of dietary oyster mushroom administered. A negative correlation between the mushroom dose and cholesterol level was found after 8 and 28 wk of feeding (r=-0.9821 and -0.9803, respectively; P < 0.02 for both cases). The dose of 1% oyster mushroom did not affect cholesterol levels in serum or body organs. A significant reduction of cholesterol levels was observed in serum (31-46%) and liver (25-30%) at a dose of 5% of oyster mushroom for all periods. Reduced cholesterol content in very low-density lipoproteins (VLDL) was also observed at this level. The highest dose of oyster mushroom induced a decrease in conjugated diene levels in erythrocytes and an increase in the levels of reduced glutathione in the liver and stimulated the activities of catalase and glutathione peroxidase in the liver in the final period of the experiment.

[Rhabdomyosarcoma of the orbit].

The authors evaluated 9 years' experience with the diagnosis and treatment of embryonic rhabdomyosarcoma of the orbit in children. They evaluated in detail a group of 5 children treated and followed up for 1-9 years. Due to comprehensive surgical, radiation and chemotherapeutic treatment all patients survive and in 80% the visual function is preserved. For differential diagnostic problems, with regard to the variable manifestation of rhabdomyosarcoma which may imitate orbitocellulitis, chalaseon, epibulbar lipodermoid or papilloma, the authors emphasize the importance of rapid primary diagnosis by NMR and biopsy. In the treatment they appreciate greatly a combination of radiotherapy and chemotherapy which is a modern trend and can eradicate the tumour without radical surgery.

[Therapeutic effectiveness of local administration of corticosteroids in the treatment of orbital capillary hemangioma].

The author evaluated three years experience with the treatment of capillary haemangioma of the orbit and eyelids in nine infants. They used several therapeutic methods. However, they injected in all patients depot corticosteroids-Triamcinolone-into the tumour. Indication for treatment was a marked cosmetic defect, imminent amblyopia and progression of the haemangioma. The results of steroid treatment are very good from the cosmetic and functional aspect. Indications for treatment, results and differential diagnostic problems are discussed.

[Atelocollagen/hydroxylapatite composite material as bone defect filler in an experiment on rats].

Many types of biomaterials are used as skeletal bone fillers in reconstructive surgery. An attention is paid to hydroxyapatite due to its high biocompatibility with the surrounding tissue. The paper deals with the testing of new collagen/hydroxyapatite composite material applied to the bone defect on os parietale of rats. The composite material was prepared from the bovine atelocollagen dispersion and the dispersion of hydroxyapatite. Collagen serves as a matrix in which the particles of hydroxyapatite are anchored. The composite presents the advantage that after the saturation with physiological solution it is compact and can be shaped. The composite material was implanted in the form of plate into six male Wistar rats to the ground bed on the surface of os parietale. The implants were taken out after four moths. The macroscopic finding of soft tissue, bones and implants gave evidence about good healing without any undesirable reaction. This was also confirmed by the histological observations. Collagen was resorbed and the rest of the material strongly adhered to the bone. The marked osteocytes were present in the zone of the newly formed bone and the dividing line between new and old bone was clear. The experimental results give preconditions to the clinical use of this new composite implant material the structural improvement of which is in progress. (Fig. 2, Ref. 12.)

[Thanatophoric nanism and histopathologic bone changes].

The authors describe in a neonate with thanatophoric nanism histopathological changes of the bones. According to X-ray findings type I was involved, the histopathological changes corresponded to type II according to Langer's classification. The consanguinity of the parents suggested the autosomal recessive type of heredity and the presence of pseudo-diastrophic dysplasia in two grade 1 cousins is considered by the authors as an incidental coincidence. The described case confirms the assumed clinical and genetic heterogeneity of thanatophoric nanism.
