Emphysematous pyelonephritis: surgical implications.

Emphysematous pyelonephritis in patients with diabetes mellitus is increasingly recognised as a disease requiring urgent and aggressive treatment. We present 3 cases of emphysematous pyelonephritis; 1 patient required percutaneous nephrostomy followed by nephroureterectomy but the other 2 were successfully managed with antibiotics and control of diabetes. Diagnosis was confirmed by sequential imaging techniques which demonstrated an inflammatory renal mass associated with gas and fluid.

Sachse optical urethrotomy, a modified technique: 6 years of experience.

Urethrotomy with the Sachse optical urethrotome has radically modified our management of strictures of the male urethra. A total of 143 patients underwent 210 procedures during 6 years. The strictures resolved in 85 per cent of the patients and they required no further treatment, including 52 patients followed for more than 6 years. A modified surgical technique using a pediatric cystoscope is described to inspect, traverse and cannulate the urethral stricture before optical urethrotomy, thus, enabling successful management of even the most difficult strictures. Our results confirm that the best prospects of cure were short and single urethral strictures, and that optical urethrotomy is the treatment of choice for strictures of the male urethra.

Estimation of glomerular filtration rate from plasma creatinine concentration in children.

The relation between the true plasma creatinine concentration (Pc) and the glomerular filtration rate corrected for body surface area (GFR/SA) was investigated in 108 individuals, and the following formula was derived: GFR/SA (ml/min per 1-73m2SA) = 0-43 Ht (cm)/Pc (mg/100 ml). This formula was tested in a second group of 83 children, and its accuracy and precision was compared to the 24-hour creatinine clearance. It was found to be superior to the creatinine clearance overall, and was as good, even if all results involving suspect 24-hour-urine collections were eliminated from analysis. The formula in SI usage is: GFR/SA (ml/min per 1-73 m2SA) = 38 Ht (cm)1Pc (mumol/l).

Childhood urolithiasis in the United Kingdom and Eire.

152 children with urinary calculi were treated in the United Kingdom and Eire in the years 1972 and 1973. There was a marked male preponderance particularly evident in early life. In 124 children the urine was infected on admission to hospital; in 87, particularly the younger children, this was with the Proteus species. Awareness of the relationship between Proteus urinary infection and matrix calculi is stressed.
