Misoprostol use in developing countries: results from a multicountry study.

OBJECTIVE: To identify information and service delivery needs for obstetric/gynecologic uses of misoprostol in developing countries. METHODS: The study included a survey of reproductive health providers in 23 countries and a qualitative study of misoprostol use in four developing countries. Researchers used purposive sampling methods for the survey and qualitative study and conducted a descriptive statistical analysis of survey data and computer-assisted text-based content analysis of qualitative data. RESULTS: In some developing countries, women frequently access misoprostol through pharmacies and self-medicate to induce early abortion. Some clinicians expressed concern about this use of misoprostol, but many stated that its availability had reduced serious complications resulting from unsafe abortions. CONCLUSION: Although misoprostol is routinely used for a range of off-label obstetric/gynecologic indications, evidence-based, up-to-date information about safety, effectiveness, and appropriate regimens is not widely available. This information is requested by providers, including pharmacists. Women need information and guidance about its use.

An evaluation of manual vacuum aspiration instruments.

OBJECTIVES: Manual vacuum aspiration (MVA) instruments vary in quality, cost, and availability. This evaluation of MVA instruments was conducted to provide decision-makers with information to assist them in determining which instruments to purchase for their specific health care settings. METHODS: Nine MVA devices were evaluated for durability, quality, safety, and usability. Study data came from device specifications, laboratory-based technical studies, and field evaluations. RESULTS: Equipment durability is compromised by high temperatures, quality is compromised by manufacturing defects, and design is key to safe reuse. All MVA instructions are written at advanced reading levels, and device assembly was difficult for most study participants. CONCLUSIONS: No single brand of MVA instrument is perfectly suited for every setting. Users can evaluate the appropriateness of MVA instrument characteristics by reviewing the elements assessed in this evaluation, which, by complementing the experience of administrators and clinicians, should enable them to determine which MVA is best for their environment.

A comprehensive and efficient process for counseling patients desiring sterilization.

To optimize the time spent counseling a sterilization patient, this article presents a 10-step process that includes all steps necessary to ensure a comprehensive counseling session: (1) Discuss current contraception use and all available methods; (2) assess the client's interest in/readiness for sterilization; (3) emphasize that the procedure is meant to be permanent, but there is a possibility of failure; (4) explain the surgical procedure using visuals, and include a discussion of benefits and risks; (5) explain privately to the client the need to use condoms if engaging in risky sexual activity; (6) have the client read and sign an informed consent form; (7) schedule an appointment for the procedure and provide the patient with a copy of all necessary paperwork; (8) discuss cost and payment method; (9) provide written preoperative and postoperative instructions; and (10) schedule a postoperation visit, or a postoperation semen analysis.
