[A case of renal cell carcinoma with hyperglycemia].

A 71-year-old male with hyperglycemia was incidentally found to have a right renal tumor. Hyperglycemia had been controlled by administration of 34 units of insulin. Immediately after radical nephrectomy, hyperglycemia was improved to be able to stop insulin. His fasting blood glucose level remained within normal limits without any treatment. Laboratory studies, however, failed to reveal any endocrinopathy derived from renal cell carcinoma responsible for the diabetic state.

[Comparison of cerebral vasoreactivity to acetazolamide in normal volunteer among 123I-IMP, 99mTc-ECD and 99mTc-HMPAO].

We studied the difference in the cerebral vasoreactivity to acetazolamide among three cerebral blood flow tracers, N-isopropyl-p-123I-iodoamphetamine (123I-IMP), 99mTc-ethyl cysteinate dimer (99mTc-ECD), 99mTc-hexamethylpropylene amine oxime (99mTc-HMPAO). Subjects were 4 healthy male adult volunteers at ages respectively of 30, 30, 35 and 41 years. Each subject underwent SPECT total 6 times. The three tracers were each used in the tests during rest and during acetazolamide loading, which were performed on different days. At least three days were allowed to intervene between one test and another, and all tests were completed within 2 months. The blood flow was calculated on the microsphere model with 123I-IMP, and Patlak plot method with 99mTc-ECD and 99mTc-HMPAO, to perform quantitative assessment. In the acetazolamide loading test, 1 g acetazolamide was intravenously administered, and each tracer was administered at 15 minutes after that. mCBF and BPI data on total 8 sides, one side representing the right or left side of each subject, were statistically analyzed. For rCBF, the region of interest was set up on the right and left sides of the frontal, temporal and occipital lobes on the slice passing through the third ventricle. The percent increase in mCBF with IMP was 29.3 +/- 10.5% (Mean +/- SD), significantly higher than 10.4 +/- 3.9% (p < 0.05) with ECD or 8.9 +/- 6.9% (p < 0.02) with HMPAO. The percent increase in rCBF with IMP was 30.8 +/- 13.4%, significantly higher than 14.3 +/- 13% (p < 0.001) with ECD or 8.7 +/- 14.8% (p < 0.001) with HMPAO. The percent increase in rCBF with ECD was higher than that with HMPAO, but not significantly. The percent increase in BPI calculated by Patlak plot method with ECD (26.4 +/- 14.3%) was higher than that with HMPAO (13.9 +/- 12.8%), but not significantly. It has been pointed out that 99mTc-ECD and 99mTc-HMPAO underestimate blood flow in the high blood flow area, which seemed to account for the underestimation of acetazolamide reactivity. The extent of underestimation in the high blood flow area is less with 99mTc-ECD than with 99mTc-HMPAO, while the stability of the preparation under acetazolamide is maintained, which seemed to explain difference in the reactivity.

[Rapidly deteriorating adult hydronephrosis due to an aberrant vessel: a case report].

Hydronephrosis due to ureteropelvic junction (UPJ) obstruction generally progresses gradually. We experienced a case of hydronephrosis due to UPJ obstruction deteriorating rapidly in an adult woman. In spite of percutaneous nephrostomy, 99mTc-DTPA renogram showed a non-functional pattern. A nephrectomy was done. Peri-operatively, the aberrant lower pole vessel was found to be obstructing the UPJ. The hydronephrosis deteriorated rapidly because the extrarenal pelvis dilated in the shape of box, became suspended from the aberrant vessel. Therefore, we believe that the box pelvis is one of the most important factors deteriorating hydronephrosis.

Age-related changes in the lipofuscin accumulation of brain and heart.

Lipofuscin is the end-product of intracellular lipid peroxidation and the accumulation results from the cellular metabolism during aging (life stage). We suggested that the accumulation of cardiac lipofuscin was dependent on the specific metabolic rate of mammals. Slower rate of cardiac lipofuscin accumulation (against absolute lifespan) was observed in the animals of larger body size. The rate of cardiac lipofuscin accumulation was correlated with the specific metabolic rate, and inversely correlated with the brain weight of mammals. The first appearance of lipofuscin granules was 8 weeks of age in the hippocampus and in the thalamus. In the case of cerebral cortex (laminae III), lipofuscin granules were first found at 3-month-old rats. The rate of lipofuscin accumulation was the highest in the hippocampus (y = 0.286x - 0.099, r = 0.963) among the three regions examined. In the case of cerebral cortex and thalamus, the slower rate of lipofuscin accumulation was observed (y = 0.072x - 0.14, r = 0.797 for cerebral cortex; y = 0.067X - 0.14, r = 0.953 for thalamus). It is noticed that the most abundant accumulation and the highest rate of lipofuscin accumulation are the hippocampus. Even the hippocampus, the rate and the magnitude of lipofuscin accumulation was low as compared with cardiac muscles. From these results, it is suggested that brains have better protective system against oxidative stress than other organs do.

Alterations of expression of the cytoskeleton after immortalization of human fibroblasts.

Human diploid fibroblasts (HDF) have a limited in vitro lifespan of population doubling level (PDL) 50-70. The molecular mechanism underlying cellular senescence and immortalization is not thoroughly understood. It has been reported that the cytoskeleton has diverse functions and may have a role in growth regulation through association with other cellular components. To shed light on the relationship between functions of the cytoskeleton and senescence or immortalization, we investigated the alterations in gene expression after immortalization and measured the amounts of mRNAs for talin, vinculin, alpha-actinin, tropomyosin 1 (TM1), vimentin, lamin A and C, and alpha-tubulin by slot blot and Northern blot analyses. We found that the mRNAs for vinculin and vimentin were reduced and the mRNA for lamin A was increased in immortalized cells. We also studied the cytoskeletal protein levels and their intracellular distributions by Western blot analysis and immunostaining. Most of the proteins studied behaved in a way similar to the mRNAs through senescence and immortalization. Vinculin, tropomyosins and vimentin showed their altered distributions in immortalized cells.

Does bacitracin have an absorption-enhancing effect in the intestine?

We studied the absorption enhancement effects of three types of protease inhibitors, aprotinin, bacitracin and soybean trypsin inhibitor, in the rat intestine. Of these protease inhibitors, bacitracin enhanced the absorption of FD-4 and phenol red from the rat small and large intestine without mucosal toxicity. Thus, it was suggested that bacitracin has not only a protease-inhibitory but also an absorption-enhancing capability.

Transforming growth factor beta 1 and beta 2 induce down-modulation of thrombomodulin in human umbilical vein endothelial cells.

To investigate the effects of transforming growth factor-betas (TGF-betas) on endothelial anticoagulant activity, we assayed thrombomodulin (TM) activity and antigen levels of human umbilical vein endothelial cells (HUVECs) incubated with TGF-betas in vitro. TGF-beta 1 suppressed surface TM activity and surface TM antigen levels maximally 12 h after incubation in dose-dependent manners. TGF-beta 2 was almost equipotent with TGF-beta 1 for the suppression of them. Both TGF-betas suppressed total TM antigen level in HUVECs, and the time course of the suppression was similar to that of the cell surface TM antigen level. The maximal reductions of TM mRNA levels by TGF-betas were observed at several hours ahead of those observed in both surface and total TM antigens levels, suggesting that the TGF-beta-mediated suppression of TM antigen of HUVECs is primarily regulated at the TM mRNA level. Our present work suggests that the down-modulation of TM level induced by TGF-betas in HUVECs contributes in vivo to promoting the thrombogenesis either at the sites of injury of vessel walls, such as atherosclerotic lesions where TGF-beta 1 is released from platelets, smooth muscle cells and monocytes, or at neovascular walls in tumors secreting TGF-beta 2.

[Effects of combined aprotinin and prostagrandin E1 therapy on aortic arch replacement].

This study was undertaken to compare the effects of combined aprotinin and prostaglandin E1 therapy on aortic arch replacement. Twenty patients were divided into 2 groups with (group A; n = 10) or without (group B; n = 10) the treatment (200 KIU of aprotinin and 0.01-0.02 microgram/kg/min of prostaglandin E1 during cardiopulmonary bypass (CPB) and the first postoperative day. Preoperative evaluation of respiratory function and all parameters related to CPB procedure were revealed to be equal between the groups. Postoperative A-a DO2 and respiratory index (RI) as functional parameters of oxygenation capacity, dosage of dopamine were monitored at 0, 3, 6, 12, 18 and 24 hr after termination of CPB and at extubation period. Serum creatinine, platelet numbers and blood coagulation function (PT, APTT) were also assayed postoperatively. The recovery of respiratory and cardiac function were superior in group A with treatment, but renal and blood coagulation function showed no difference in the groups. We suggest the combined therapy with aprotinin and prostaglandin E1 for aortic arch replacement may emerge as a valuable treatment to save postoperative respiratory and cardiac function.

[Cauda equina cavernous hemangioma associated with hydrocephalus--case report].

Spinal cavernous hemangioma is rare, and it is extremely rare for cavernous hemangioma to develop in the cauda equina. There has been only one report of hydrocephalus associated with cavernous hemangioma in the cauda equina. We report a case of cavernous hemangioma in the cauda equina diagnosed on the basis of the headaches due to hydrocephalus. A 67-year-old man was being treated for Parkinson's disease because of tremor of both upper extremities for several years. In December 1991 he complained of occasional headaches. On February 15, 1992 the headaches became severe and frequent, with nausea and vomiting, and his gait became unsteady. Four days later he came to our hospital. Neurological examination revealed fine finger tremor and truncal ataxia. Computerized tomography scanning and magnetic resonance imaging of the head revealed ventricular enlargement, but there were no mass lesions obstructing the cerebrospinal fluid pathway. Lumbar puncture at the L3-L4 level yielded bloody cerebrospinal fluid, and the pressure had increased to 410 mmH2O. Cerebral angiography showed no abnormal findings. Magnetic resonance imaging of the lumbar spine demonstrated an intradural tumor at the level of vertebral body L2. Spinal angiography showed no evidence of abnormal vascularity in the mass at the L2 level. On March 10, 1992, laminectomy at three levels, L1 to L3 was performed, and a well-defined blueberry-like intra-cauda equina tumor 1 cm. in diameter, was removed. One spinal nerve root passed through the tumor. The pathological diagnosis was cavernous hemangioma. After removal of the tumor, the patient's headaches improved, and a follow-up computerized tomography scan six months later showed normal ventricle size.(ABSTRACT TRUNCATED AT 250 WORDS)

[Antegrade or retrograde blood cardioplegic method: comparison of postsurgical right ventricular function and conduction disturbances].

This study was undertaken to compare postsurgical right ventricular function and the occurrence of conduction disturbances after employing cold blood antegrade or retrograde cardioplegia during open heart surgery. Thirty-four patients were divided into AC (antegrade) and RC (retrograde) groups for the difference of route for delivery of cardioplegic solutions. Preoperative evaluation of cardiac and respiratory function revealed to be equal characteristics between the groups. Postoperatively, A-aDO2 and respiratory index (RI) as functional parameters of oxygenation capacity, LVSWI, RVSWI, dosage of dopamine and conduction disturbances were monitored at 0, 3, 6, 12 hours after termination of cardiopulmonary bypass and at extubation period. Although the recovery of respiratory function and left ventricular function were similar in both groups, temporal suppression of right ventricular function was indicated in RC group during early period after surgery, and then recovered to the same values of AC group within 3 hours. In RC group, several type of conduction disturbances were detected in 28 per cent of patients. But none of the persistent conduction disturbances were remained in all patients. We suggest retrograde coronary sinus perfusion may emerge as a valuable alternative to antegrade methods for delivery of cardioplegia.

[Comparison of postsurgical respiratory function employing either cold blood or crystalloid cardioplegia during open heart surgery].

This study was undertaken to compare postsurgical respiratory function after employing cold blood or cold crystalloid cardioplegia during open heart surgery. Fourty patients were divided into two groups for the difference of the cardioplegic solutions. Preoperatively, left ventricular stroke work index (LVSWI) and respiratory function test was evaluated. Postoperatively, A-a DO2 and respiratory index (RI) as functional parameters of oxygenation capacity, LVSWI and dosage of dopamine were monitored at 3, 6, 12, 18, 24 hours after cardiopulmonary bypass and at extubation period. The recovery of respiratory function and cardiac performance were superior in blood cardioplegia with a less accumulation of water in the pulmonary interstitium as a result of hemodilution effect.

Cloning of cDNAs with possible association with senescence and immortalization of human cells.

Normal human diploid fibroblasts (HDF) have a finite life span in vitro and have been used as a model system for the study of in vivo aging. Little is known about how changes in gene expression may affect the immortalization of human fibroblasts. We looked for cDNA clones whose mRNAs were differentially expressed between mortal senescent SV40-transformed human fibroblasts (B-32) and the immortal counterparts (B-32F) derived from B-32 cells. We identified three cDNA isolates by subtractive differential hybridization with 32P-labeled cDNA probes from B-32 cells and B-32F cells. Nucleotide sequence analysis of these cDNA clones revealed that they were homologous to the human vimentin, a human mitochondrial gene and a human gene of unknown nature. Slot blot and Northern blot analyses demonstrated that the former two were preferentially expressed in senescent B-32 cells and the last one was less expressed in B-32F immortal cells.

Prevalence of IgE antibody to crude and purified allergens of Japanese cedar pollen among different troops of Japanese monkeys (Macaca fuscata).

We measured specific IgE antibodies to the crude allergen as well as two purified allergens (Cry j I and Cry j II) of Japanese cedar (Cryptomeria japonica--CJ) pollen in the serum of 276 Japanese monkeys in nine troops. Of 45 monkeys with CJ specific IgE in eight of nine troops, 23 (51%) were found to have IgE to both Cry j I and Cry j II, 21 (47%) only to Cry j I, and one (2.2%) only to Cry j II. The positive rate of specific IgE antibody to each allergen varied among the troops.

[Epididymal sperm invasion apparently suggestive of a testicular tumor].

A 49-year-old man visited the outpatient clinic suffering from painless swelling of the left testicular content which he had recognized 1 week earlier. The lower portion of the testis or the epididymis gave rise to a thumb-head sized irregular tumor. At the exploring operation, the tumor was revealed to be the swollen epididymal tail. Epididymectomy was performed. Microscopically, the tumor was diagnosed as an epididymal granuloma invaded by many sperm, histiocytes, macrophages, neutrophil leukocytes and lymphocytes around the epididymal tubules. Eight cases of epididymal sperm invasion reported in Japan during the recent 20 years are reviewed and briefly discussed.

[A rare case of total anomalous pulmonary venous connection to the right atrium confounding with cor triatrium].

Three-days-old newborn baby underwent ligation of the ductus arteriosus under the diagnosis of TAPVC and PDA with high flow heart failure. Her UCG showed the direct connection of common pulmonary venous chamber (CPVC) to the upper and medial portion of right atrium. CPVC positioned just above the left atrium but does not communicated with the left atrium. These findings characterized this anomaly as II A cor triatriatum classified by Lucus-Schumidt. At 5-month-old, corrective operation was performed. Both right and left venous chambers joined just before the connection to right atrium and drained to right atrium at the upper portion of atrial septum. CPVC and left atrium had independent wall. Accordingly, operative diagnosis was made as TAPVC (IIb). Intraatrial routing was performed using porcine pericardial patch. At 6 months after operation, patient is doing well without the sign of venous obstruction. This type of TAPVC is rare and may confound with cor triatriatum. Our case may suggest the pathological mechanism of cor triatriatum and that at least some of cor triatriatum may be identical with TAPVC.

Accumulation of cardiac lipofuscin in crab-eating monkeys (Macaca fasicularis): the same rate of lipofuscin accumulation in several species of primates.

Previously, we have reported that the aging process begins at sexual maturation (Nakano, M. et al., Mech. Ageing Dev., 52 (1990) 93-106). In this paper, we reported the cardiac lipofuscin accumulation of crab-eating monkeys. The first appearance of cardiac lipofuscin was around sexual maturation, and the rate of accumulation in crab-eating monkey was 0.45. Several primates which have different life spans show the same rate of lipofuscin accumulation in the life stage. Namely, even in a different life span, the amount of lipofuscin accumulation in a given period of life such as puberty, middle age, old age was the same. From these results, it is suggested that the amount of lipofuscin accumulation is the same in the life span of primates having different life spans.
