[Spontaneous urinary extravasation: report of three cases].

We report three cases of urinary extravasation successfully recovered by conservative measures. Two of them were regarded as spontaneous peripelvic extravasation caused by small ureteral stones. After several days' indwelling ureteral catheter extravasation was ceased and the stones were discharged spontaneously in both cases. The other was regarded as spontaneous rupture of the renal pelvis with urinoma formation due to ureteral obstruction by bladder cancer invasion. Intra-arterial chemotherapy, radiation and hyperthermotherapy to the bladder under percutaneous nephrostomy yielded recanalization of obstructed ureter and resolving of urinoma. We shortly review previous reports and discuss the role of diagnosis and treatment.

Nucleotide sequence analysis of the DNA binding region of the chicken fibronectin gene.

We have determined the nucleotide sequence of 2.0 kb EcoRI segment from the clone lambda FC32 of the genomic chicken fibronectin gene, which is called DNA binding domain. This segment overlapped another clone lambda FC36 and contained three exons which were 16, 17 and 18. They were classified as Type III repeat as originally shown in bovine plasma fibronectin. The average homologies of these three exons among the chicken, rat and human fibronectins in amino acid level are very high (87-98%) compared with that (79-88%) of the exons in the cell binding domain, indicating that this region is highly conservative during the evolution.

Variations of activities in the segments of respiratory chain among tissues in a patient with mitochondrial encephalomyopathy.

Enzymatic activities of NADH cytochrome c reductase and cytochrome c oxidase were determined in the mitochondria from various tissues of a patient with mitochondrial encephalomyopathy and compared with those of controls. NADH cytochrome c reductase in the present patient decreased significantly in the liver and spleen and to a less extent in the kidneys. On the other hand, cytochrome c oxidase of the patient decreased severely in the skeletal muscle and kidneys and partially in the heart. Difference spectrum of reduced-minus oxidized form of mitochondria from patient's skeletal muscle and heart showed a decrease of cytochrome aa3 peak in the alpha region at 605 nm. These results indicate that there are cryptic deficiencies in the segments of the respiratory chain in the mitochondria from several tissues of the present patient, such as liver, kidney, spleen without any clinical manifestation. The weakness and atrophy of skeletal muscle was, however, well correlated to the biochemical analysis.

Actinomycosis of urachal remnants.

We report a case of urachal actinomycosis. The patient presented with complaints of micturition pain and a lower abdominal mass. Computerized tomography and an echogram showed the mass extending from the dome of the bladder to just beneath the rectus muscle. Exploration revealed a hard mass in the urachal cord, which was near the dome of the bladder and extended to the umbilicus. The mass and urachal cord were resected, and histopathological examination revealed actinomycosis of the urachal remnants.

Revascularization surgery for moyamoya disease.

Moyamoya disease is a disease characterized by chronic occlusion of the circle of Willis with subsequent development of fine vascular networks in the ganglionic region, which is common in Japanese people. The term of 'moyamoya' means puff of smoke in the Japanese language and represents the characteristic angiographic findings of subsequent development of fine vascular networks in the ganglionic region. Although reconstructive surgery for moyamoya disease has been widely accepted nowadays, there is still no definite consensus as to a surgical indication for patients with an haemorrhagic attack and as to the selection of an operative method for each individual patient. This paper presents our overall surgical results and introduces a new operation, devised by Y. Nakagawa (i.e. EMAS). It refers also to surgical intervention for patients with haemorrhagic attacks.

Mechanism of methotrexate-sensitivity of choriocarcinoma cells in culture.

Four cell lines established from choriocarcinoma were compared for sensitivity to methotrexate (MTX). In this paper, we have compared the relative gene copy number of dihydrofolate reductase (DHFR), the target enzyme of methotrexate (MTX), in order to clarify whether or not amplification of the gene is involved in the relative resistance to MTX observed for one of the cell lines, designated NaUCC-1, which is 4- to 5-fold more resistant to MTX as compared with the other cell lines and exhibits a reduced uptake of [3H]MTX. Neither dot blot nor Southern blot hybridization revealed any significant difference in the gene copy number among the four cell lines. Therefore, the resistance to MTX of the NaUCC-1 line is explained by a reduced uptake of the drug, rather than amplification of the target gene.

A population-genetic study of crab-eating macaques (Macaca fascicularis) on the island of Angaur, Palau, Micronesia.

Blood protein polymorphisms of an introduced population of the crab-eating macaques on Angaur Island, Palau, Micronesia, were examined electrophoretically to assess genetic variability. Results showed a high degree of genetic heterozygosity and some distinctive features in the genetic constitution of this island population. Negative evidence is presented regarding ancestry from a single pair of macaques. Their origin is discussed in relation to the genetic structure of the present population.

[Experimental knee pain model in rats and analgesic effect of sodium hyaluronate (SPH)].

Using the model of knee pain reaction induced by intra-articular injection of endogenous pain substances, especially bradykinin (BK) in rats, the mechanism of the analgesic effect of sodium hyaluronate (SPH) was investigated. The simultaneous administration of prostaglandin E2 with BK or hyaluronidase digestion of endogenous hyaluronic acid (HA) in our experiments brought remarkable hyperalgesia on BK-induced knee pain. These results suggest that higher sensitivity to the pain reaction is induced in a diseased joint (higher prostaglandin content, lower concentration and molecular size of HA in synovial fluid) than in a normal one. SPH definitely decreased BK-induced pain, and its analgesic effect was observed for a longer period, depending on its dose in pre-treatment and the degree of its distribution in synovial tissues. As the analgesic effect of SPH was observed in the hyaluronidase-treated joint as well, it is suggested that the increasing viscosity of synovial fluid caused by increasing HA concentration can decrease the pain even without normalizing molecular size of HA in the joint. HA oligomer and other compounds with similar viscosity or with similar polyanionic character as SPH showed no analgesic effect. From these results, it seems that the characteristic steric configurations of higher molecular HA are needed for the manifestation of the analgesic effect. SPH seems to show its analgesic effect by covering pain receptors in synovial tissues and holding endogenous pain substances in its molecule.

[Recent advances in the treatment of choriocarcinoma].

Recent advances in intensive care procedures have improved the therapeutic results of choriocarcinoma treatment. The mortality of patients with choriocarcinoma, which was 100% in 1958, has gradually decreased, and finally fell to about 10% in 1983. These favorable results have been mainly due to the progress made in chemotherapy, including combined administration of methotrexate and actinomycin-D. However, recent progress has been chiefly dependent on intensive care procedures, involving so-called multidisciplinary treatment, including surgical treatment for pulmonary or intracranial metastatic foci and whole-brain irradiation. When the treatment results for choriocarcinoma were compared between metastatic and non-metastatic cases, the mortality, which had decreased for both groups with time, had already reached 0% in non-metastatic cases, although it was still about 20% in metastatic cases. These results have made it clear that the most important aspect is how to treat choriocarcinoma metastatic foci. With the goal of finding some form of treatment that will help to attain complete cure of choriocarcinoma, we present here details of recent progress made in choriocarcinoma treatment.

[Urinary bladder tumor manifesting multiple bone metastases similar to prostatic carcinoma--a case report].

A 72-year-old patient was found to have a non-papillary bladder tumor extending from the vesical floor to the retrotrigone and manifested multiple bone metastases without evidence of any hematogenic metastases in other organs. The pattern of the above metastases were very similar to that of a prostatic carcinoma and suggests that the so-called specific pattern of bone metastases observed in prostatic carcinomas might be caused by the location of the original tumor and not by features of the prostatic carcinoma cells themselves.

Genetic analysis of the cell binding domain region of the chicken fibronectin gene.

We have determined the nucleotide sequence of the cell binding domain region of the chicken fibronectin gene and analyzed it evolutionaly. We present here the complete nucleotide sequence of 4.3 kb HindIII/EcoRI segment from the clone lambda FC23 of the chicken fibronectin gene. There were five exons in this segment. When we lined up the amino acid of exons 28, 29 and 31, three alignments, known as the Type III repeat, appeared. Tetrapeptide, -RGDS-, called the cell binding domain, existed in the second repeat, coding exon 30. It was presumed that the Type III repeats were composed of two exons in the chicken gene, the same as in the rat and humans. We found repeatedly appearing amino-acid sequences such as -TIT- (three arrays in these Type III repeats) but also found one of the amino acids substituted in the tripeptide in these Type III repeats (seven arrays). We analyzed these repeats from the point of view of evolution. We used three of the nucleotide sequences (12-18 bp) coding such -TIT- repeats as a unit length for comparing the various homologies after dividing the coding region into 56 segments. The mutual homology of the divided segments to each one of three showed 53% on average. On the other hand, the mutual nucleotide homology of the Type III repeat was 44%. This suggested that the Type III repeat may have been developed by frequent duplication of small gene units.

Analysis of N-myc amplification in relation to disease stage and histologic types in human neuroblastomas.

Both untreated and treated primary neuroblastomas from 52 patients were analyzed to determine the correlation between the amplification of N-myc oncogene and various prognostic factors. Amplification of N-myc was observed in eight of 28 untreated cases and in 12 of 24 treated cases. As a whole, 12 of 18 tumors (67%) in Stage IV had N-myc amplification, but there were fewer cases in the unadvanced disease stage, as reported previously by others. Furthermore, the authors detected N-myc amplification in three of nine tumors in Stage IV-S, although the amplification was less than 50 copies. Analysis of progression-free survival at 24 months revealed that amplification of N-myc was associated with the worst prognosis (P less than 0.001). In the untreated group, no amplification of N-myc was detected in any of two ganglioneuromas and four ganglioneuroblastomas, whereas amplification of N-myc was observed in all two round-cell and six of 20 rosette fibrillary neuroblastomas. On the other hand, the authors detected amplification of N-myc in three of eight less differentiated ganglioneuroblastomas in the treated group and observed the worst prognosis in these three patients. The total percentage of the cases from both untreated and treated groups suggest that amplification of N-myc may occur more frequently in undifferentiated types of neuroblastomas than in less malignant types. In conclusion, the amplification of N-myc in neuroblastomas was closely associated with the worst prognosis, which was suggested by both disease stage and histologic characteristics.

Pneumocystis infection in macaque monkeys: Macaca fuscata fuscata and Macaca fascicularis.

Retrospective examination of lungs from 128 monkey necropsies was attempted for Pneumocystis infection using special strains, including toluidine blue-O and Gomori's methenamine silver nitrate. Four Japanese monkeys (7.7%), Macaca fuscata fuscata, and one crab-eating monkey (7.7%), Macaca fascicularis, were found to have Pneumocystis infection. The organism was found in young and infant animals. At the time of death, one infant and two young monkeys were debilitated and/or emaciated. Pneumocystis infection was considered an important lesion which could have caused reduced respiratory function in two of the Japanese monkeys, but constituted only an incidental finding in the others.

Screening for carcinogens by the DNA synthesis inhibition test using human fibroblasts.

The Ames test has probably been the most common prescreening test for potential carcinogens. This system, however, occasionally presents false-positive or false-negative results for certain kinds of chemicals. We chose 24 agents, most of which showed a discrepancy in their results with the animal carcinogenicity test and the Ames test, and screened them by the DNA synthesis inhibition test using human fibroblasts. Among 22 agents, 13 (59%) showed consistent results, 6 (27%) showed discrepant results with those obtained by animal tests and 3 (14%) showed ambiguous results. The majority of the DNA synthesis inhibition test results for these chemicals were consistent with the results obtained by whole animal tests, suggesting that the DNA synthesis inhibition test with mammalian cells in culture is a reasonably reliable prescreening test for potential carcinogens.

[A case of right triplicate and left duplicate ureters].

Ureteral triplication is a rare anomaly of the urinary tract. A 42-year-old male was found to have right triplicated and left duplicated ureters. The chief complaint was gross hematuria, but it disappeared after admission. Computerized tomographic scan, renal angiography and urine cytology revealed no pathological findings but 3 small aneurysms at the upper pole of the right kidney. Cystoscopic examination showed 3 ureteral orifices on the right side with bleeding from the lowest ureteral orifice. Right retrograde pyelography revealed 3 pelvic systems with calyceal communication of lower and middle pelvic system. Our case is consistent with altered type-D of Smith's classification.
