Response of random skin flaps to rapid expansion.

We studied the physiologic accommodation of tissues to 5-day expansion in a porcine random flap model to assess the time taken to expand skin to provide early coverage of traumatic defects. Three 6 X 20 cm random flaps were outlined but not elevated on the backs of 12 small (20-kg) pigs. One flap was not manipulated (control). A 450-ml expander was inserted at the base of the second flap and left in place (sham). At the base of the third flap a 450-ml expander was inserted and sequentially filled to the limits of skin viability each day for 5 days (expanded). Skin tension produced by the pressure of expansion and tissue oxygen (TpO2) in each flap were measured before and after each manipulation. Flaps were raised on day 8, and survival determined on day 15. Creation of the pocket reduced TpO2 to 55% of control values, expansion to 17.5% of control. Within 24 hours both sham and expanded had recovered to 80% of control values. After the second expansion, recovery from hypoxia was again almost complete after 24 hours. Recovery after subsequent expansions was less complete, but 3 days after the final expansion there was no statistical difference between TpO2 values in expanded, sham, and control groups. High pressures generated in the flap by expansion also returned to near normal within 24 hours. Flaps utilizing expanded skin survived to greater length (14.2 cm) than did sham (11.6 cm) or control (7.2 cm) flaps (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Thiamine deficiency at a district general hospital: report of five cases.

Five biochemically severe cases of thiamine deficiency presented to one medical firm within an 11-month period at a district general hospital in Wales. In two patients the presenting feature was oliguric functional renal failure, with frank cardiovascular beri-beri. In one of these renal failure developed over a period of seven days, the cause being recognised after 20 days of continuous high-calorie intravenous feeding including daily vitamin supplements. Three further cases illustrate how early recognition and adequate treatment of the renal and cardiovascular components of thiamine deficiency may be life-saving.

Effect of bovine oviductal estrus-associated protein on the ability of sperm to capacitate and fertilize oocytes.

At estrus, an oviduct-specific, estrus-associated glycoprotein (EAP) of molecular weight 85-95,000, is secreted by the oviductal epithelium and found in cannula-derived bovine oviductal fluid (ODF). The objectives of these studies were to determine if bovine sperm were capacitated by EAP in vitro, whether this effect differed for EAP derived from ODF versus conditioned medium from oviduct ampullar explants, and to determine if sperm capacitated in vitro with EAP-fertilized bovine eggs. Sperm were incubated for up to 6 hours with partially purified EAP derived from ODF and assessed for capacitation by their ability to undergo the acrosome reaction following exposure to lysophosphatidylcholine. At 4 hours of incubation, the number of capacitated sperm in treatments containing 50% ODF or EAP plus bovine serum albumin (BSA) was similar, and it was significantly greater than the number of capacitated sperm in treatments containing antibodies to EAP. Using purified EAP derived from ampullar explant-conditioned medium, twice the number of sperm were capacitated after 4 hours with EAP from conditioned medium or with ODF than with treatments containing anti-EAP. The fertilizing ability of sperm incubated with EAP was significantly greater than that for sperm incubated without EAP or with anti-EAP. We conclude that bovine EAP, derived from both ODF and in vitro cultures, promotes sperm fertilizing ability.
