Selective expression of mitogen-inducible cyclooxygenase in macrophages stimulated with lipopolysaccharide.

Two forms of cyclooxygenase are known to be present in eukaryotic organisms: a cyclooxygenase (COX-1) first purified from ram seminal vesicles encoded by a 2.8-kilobase mRNA, and a newly discovered mitogen-inducible cyclooxygenase (COX-2) encoded by a 4-kilobase mRNA. Expression of these two forms of the enzyme in rat alveolar macrophages stimulated with lipopolysaccharide was investigated by 1) determining the activity of newly synthesized enzyme after inactivating the endogenous enzyme with aspirin; 2) comparing levels of newly synthesized enzyme proteins in cells treated with or without lipopolysaccharide; and 3) assessing the expression of the mRNAs encoding COX-1 and COX-2. Levels of enzyme proteins were assessed by Western blot analysis and immunoprecipitation of 35S-labeled enzyme using two different antibodies, one specific for COX-2 and the other recognizing both forms of the enzyme but preferentially recognizing COX-1. We report here that the enhanced cyclooxygenase activity induced by the bacterial lipopolysaccharide in rat alveolar macrophages is caused by selective expression of the COX-2. Expression of COX-2 in macrophages stimulated by lipopolysaccharide was completely inhibited by dexamethasone, whereas COX-1 was unaffected. In resting unstimulated macrophages, only COX-1 but not COX-2 was detected. Levels of mRNA for the COX-2 in macrophages were increased, but those of the COX-1 were not affected by lipopolysaccharide as assessed by reverse transcription coupled with polymerase chain reaction. These results indicate that increased synthesis of prostaglandins and thromboxanes in lipopolysaccharide-stimulated macrophages results from selective expression of COX-2.

Resurgence of congenital rubella syndrome in the 1990s. Report on missed opportunities and failed prevention policies among women of childbearing age.

OBJECTIVE: To assess previous missed opportunities for rubella screening and vaccination of women delivering infants with congenital rubella syndrome and to discuss prevention strategies. DESIGN: Descriptive analysis of data collected through interviews and review of medical records. POPULATION STUDIED: Twenty-one women who delivered infants with congenital rubella syndrome in four Southern California counties from January 1, 1990, through January 8, 1991. RESULTS: Twelve (57%) of the women had a total of 22 known missed opportunities for rubella screening or vaccination. Of the 22 missed opportunities, three (14%) were missed screenings at the time of marriage, two (9%) were missed screenings during previous pregnancies, five (23%) were missed screenings during induced abortions, and 12 (55%) were missed opportunities for vaccination postpartum or after induced abortions. Nine (43%) of the women had no known missed opportunities for rubella screening or vaccination. Of 12 women educated in California, only four (33%) were subject to the 1982 California school rubella immunization requirement. CONCLUSIONS: Congenital rubella syndrome could have been prevented in more than half of the infants born to these women if missed opportunities for rubella testing and/or vaccination had not occurred. Because premarital rubella testing and school immunization requirements do not ensure that all women of childbearing age are immune to rubella, physicians and hospitals should establish procedures for postpartum rubella vaccination of susceptible women. Family planning and abortion clinics should implement routine rubella testing and follow-up vaccination of susceptible women.

The tumor promoter pristane activates transcription by a cAMP dependent mechanism.

Pristane is a naturally occurring isoprenoid which is believed to be derived from the phytyl moiety of chlorophyll. Thus it is not surprising that pristane is present in many common fruits or vegetables and furthermore can be detected in tissues of fish and mammals. Using the rat as an animal model, pristane can function as a potent tumor promoter. It is conceivable that pristane could play a role in the development of certain malignancies in higher mammals since it is commonly found in the diet. At the molecular level, pristane can induce changes in the plasma membrane, alter the conformation of chromatin, as well as selectively activate gene expression. This study was undertaken to identify specific transcriptional motifs which are responsive to pristane. A transcriptional promoter which contained a cAMP response element (CRE) was consistently stimulated by pristane in several mouse and primate cell lines. A promoter construct which contained a single copy of the TPA response element (TRE) was also activated by pristane but surprisingly a promoter which contained multiple copies of the TRE was not. Activation of the TRE required 10 fold higher concentrations of pristane relative to activation of the CRE. Within two hours after addition of pristane to monkey fibroblasts (CV-1) levels of cAMP were increased more than two fold relative to controls. These data indicated that pristane can increase the level of cAMP in CV-1 cells and consequently stimulate transcriptional promoters which contain a CRE.

Color Doppler imaging of the cystic artery.

Color Doppler ultrasonography was used to examine the gallbladder vascular supply in 30 volunteers and in 30 biliary lithotripsy patients pre- and 2 hours posttreatment. A preliminary study of 10 cases of acute cholecystitis was also performed. A cystic artery waveform was obtained in 24 of the volunteers, 18 pre- and postlithotripsy patients, and in 1 of 10 patients with acute cholecystitis. The mean pulsatility indices obtained were 1.04 in the volunteers, 0.98 pre- and 1.02 postlithotripsy (p greater than 0.1). This study shows that the normal gallbladder blood supply can be consistently identified using color Doppler flow imaging and that there is no appreciable difference in the cystic artery pulsatility index following medium-energy shock-wave extracorporeal lithotripsy. Preliminary results in acute cholecystitis suggest that flow is decreased below the sensitivity of the color Doppler imaging system used in this investigation.

Prevalence of anti-HCV and HCV viremia in hemodialysis patients in Taiwan.

Hepatitis C viral infection in 125 hemodialysis patients from Taiwan was studied using a second-generation anti-HCV immunoassay (EIA II) (Abbott HCV 2.0 EIA) and the polymerase chain reaction (PCR) to detect the HCV RNA in the serum. A total of 59 patients (47.2%) were positive by EIA II. In comparison, the conventional C100-3 anti-HCV assay was positive in 40 (32.0%). HCV RNA was found in 47 patients (37.6%). Patients with elevated serum transaminase level had a higher positive rate of anti-HCV and HCV RNA. The dialysis time was longer for those patients positive for anti-HCV than for those who were negative. A total of 57 of the 59 EIA II-positive cases had a history of blood transfusion. The HBsAg status did not influence the anti-HCV positivity. Among the 59 EIA II-positive patients, 66.1% were also positive for HCV RNA, and of the 47 HCV RNA-positive cases 83.0% were positive for EIA II. It is concluded that the high prevalence of specific HCV infection and HCV viremia was present in these patients. Prevention of cross-contamination during dialysis and blood screening before transfusion are important for the control of HCV infection in these patients.

Quantitative evaluation of optic disc pallor in pseudotumor cerebri patients.

We quantified optic disc pallor in patients with pseudotumor cerebri with the Rodenstock Optic Nerve Head Analyzer before and after optic nerve sheath decompression. Mean frequency distribution of pallor peaked between the pallor values of 0.10 and 0.20 in four pseudotumor cerebri patients and between 0.50 and 0.60 in five normal subjects. The red dominant pallor reflectance in pseudotumor cerebri patients decreased significantly and shifted toward the reflectance of normal subjects gradually during the 9 weeks after successful optic nerve sheath decompression, coinciding with the ophthalmoscopic resolution of papilledema. The computerized pallor measurement with the Rodenstock Optic Nerve Head Analyzer allows an objective and quantitative assessment of papilledema in pseudotumor cerebri patients.

FinOP repression of the F plasmid involves extension of the half-life of FinP antisense RNA by FinO.

The transfer operon of the F plasmid is positively regulated by the traJ gene product, expression of which, in turn, is regulated by both an antisense RNA, FinP, and the FinO protein (the FinOP system). A finP- F plasmid, pSFL20, was constructed by site-directed mutagenesis and was found to produce wild-type levels of pili encoded by the transfer operon. Transcription of the traJ gene was decreased by a factor of 3-5 fold in the presence of FinOP with no accumulation of a stable RNA duplex between the FinP RNA and the portion of the traJ mRNA which is complementary to finP. Stabilization of FinP RNA by FinO occurs in the absence of traJ transcripts, suggesting that FinO may interact directly with FinP to prevent its degradation.

AlF4- and vanadate stimulate chloride secretion in rabbit colon by a Ca(2+)-dependent mechanism.

Stripped rabbit descending colon mucosae were studied in vitro in modified Ussing chambers to determine the effects of AlF4- and vanadate on Cl- transport. Serosal additions of AlF4- and vanadate increase short circuit current (Isc) and tissue conductance, while luminal addition of the agents is ineffective. Addition of aluminium potentiates the effect of NaF on Isc. AlF4- and vanadate increase serosal-to-mucosal flux of 36Cl without affecting mucosal-to-serosal flux. The effects of these agents on Isc are markedly inhibited by serosal addition of bumetanide and depend on the presence of Na+ in the serosal bathing solution. The effects of AlF4- and vanadate on Isc are dependent on the presence of Ca2+ in the bathing solution, and are completely inhibited by indomethacin, but the effect of forskolin is not affected by the removal of Ca2+ from the bathing solution and the addition of indomethacin. AlF4- and vanadate significantly increase the level of inositol phosphate metabolites. The results indicate that AlF4- and vanadate increase Cl- secretion in the rabbit colon via an increase in prostaglandin synthesis which is mediated by the increase of intracellular Ca2+ concentrations.

Localization of calcium in murine epidermis following disruption and repair of the permeability barrier.

Perturbation of the cutaneous permeability barrier results in rapid secretion of epidermal lamellar bodies, and synthesis and secretion of new lamellar bodies leading to barrier repair. Since external Ca2+ significantly impedes the repair response, we applied ion capture cytochemistry to localize Ca2+ in murine epidermis following barrier disruption. In controls, the numbers of Ca2+ precipitates in the basal layer were small, increasing suprabasally and reaching the highest density in the stratum granulosum. Barrier disruption with acetone produced an immediate, marked decrease in Ca2+ in the stratum granulosum, accompanied by secretion of lamellar bodies. Loss of this pattern of Ca2+ distribution was associated with the appearance of large Ca2+ aggregates within the intercellular spaces of the stratum corneum. The Ca(2+)-containing precipitates progressively reappeared in parallel with barrier recovery over 24 h. Disruption of the barrier with tape stripping also resulted in loss of Ca2+ from the nucleated layers of the epidermis, but small foci persisted where the stratum corneum was not removed; in these sites the Ca2+ distribution did not change and accelerated secretion of lamellar bodies was not observed. Following acetone-induced barrier disruption and immersion in isoosmolar sucrose, the epidermal Ca2+ gradient did not return, and both lamellar body secretion and barrier recovery occurred. However, with immersion in isoosmolar sucrose plus Ca2+, the epidermal Ca2+ reservoir was replenished, and both secretion of lamellar bodies and barrier recovery were impeded. These results demonstrate that barrier disruption results in loss of the epidermal Ca2+ reservoir, which may be the signal that initiates lamellar body secretion leading to barrier repair.

Chondrocalcinosis of the temporomandibular joint: an external ear canal pseudotumor.

An unusual case of calcium pyrophosphate arthropathy (pseudogout) involving the temporomandibular joint of a 53-year-old woman presented clinically as an ear canal tumor. Further evaluation and surgical exploration disclosed chondrocalcinosis of the temporomandibular joint with involvement of the bones at the left base of the skull. This case report emphasizes the importance of recognizing the lesion by frozen-section examination because excellent results can be obtained by intraoperative curettage of the calcareous deposits and local irrigation with saline solution.

Enhanced detection of respiratory viruses using the shell vial technique and monoclonal antibodies.

The shell vial technique using A549 and MDCK cells, coupled with the use of Bartels respiratory viral monoclonal antibodies, was evaluated initially for the detection of 28 previously isolated respiratory viruses. All viruses were recovered and correctly identified. The shell vial-monoclonal antibody technique was then evaluated for virus isolation from 338 respiratory specimens and compared with the conventional tube method. Both methods gave rise to a total of 83 virus isolates. Of these isolates, 68 (20.1%) were isolated and identified by the shell vial-monoclonal method; 60 (17.8%) were culture-positive by the conventional tube method; forty-five (13.3%) were positive by both methods. The shell vial-monoclonal antibody method yielded 12 isolates of influenza A, two isolates of parainfluenza type 3 and one each of parainfluenza types 1 and 3, which were missed by the conventional tube method, indicating the superior sensitivity and specificity of the shell vial-monoclonal antibody method (Chi-square analysis, P = 0.001) for the detection of these viruses. Of the 50 RSV isolates, 29 were detected by both methods and there were 21 discrepancies between the two methods. The shell vial-monoclonal antibody method also improved the turn-around time for the respiratory virus groups.

Uptake of uric acid and p-aminohippurate (PAH) by renal cortical slices of various mammals.

1. Accumulation of uric acid and PAH was measured in renal cortical slices of various mammalian species. 2. The slice to medium ratio of uric acid was above unity in the rabbit, guinea pig, pig and cow, suggesting an active accumulation of uric acid, while it was near or below unity in the rat and mongrel dog. 3. Uric acid uptake in the rabbit, guinea pig and cow was significantly inhibited by PAH. 4. Uric acid was a potent inhibitor of PAH uptake in the rabbit, guinea pig, dog and pig, but much less potent in the rat and cow. 5. Kinetic analysis showed that uric acid inhibited PAH uptake in a competitive manner in all species studied except for the cow showing a noncompetitive type. 6. These results indicate that uric acid and PAH share a common transport mechanism at the basolateral membrane of the rabbit, guinea pig and pig.

Urate excretion by the cat kidney.

1. The renal handling of urate by the cat kidney was investigated during continuous infusion of urate. 2. Fractional urate excretion (FE(UA)) in cats was 0.57 +/- 0.04, indicating net reabsorption of urate. In contrast, FE(UA) in rabbits was 1.76 +/- 0.08, reflecting net secretion of urate. 3. Fractional PAH excretion (FE(PAH)) was 3.94 +/- 0.26 in cats and 4.12 +/- 0.76 in rabbits, showing net secretion in both species. 4. FE(UA) in cats was dependent on urine flow, but was independent of plasma urate concentration. 5. The urate excretion in cats was enhanced by probenecid, but was insensitive to PAH and PZA. 6. The PAH excretion in cats was reduced by probenecid, but was unaltered by urate and PZA. 7. These results indicate that urate is handled in the cat kidney by a unique transport system which is distinct from that for organic anions.

Production of interferon by peripheral blood mononuclear cells from normal individuals and patients with chronic lymphocytic leukemia.

Peripheral blood mononuclear cells (PBMC) from normal individuals were studied to identify which cells produce alpha-interferon (IFN-alpha) in response to a virus stimulus. It was found that cells both adherent and nonadherent to plastic formed IFN-alpha after induction by any one of several viruses studied. When nonadherent cells were separated on discontinuous Percoll gradients, only the cells in the less dense Percoll fractions produced IFN, whatever the virus used. By indirect immunofluorescence with monoclonal antibodies to HLA-DR and to Leu 11b, the distribution of the HLA-DR+ cells was shown to resemble most closely that of the IFN-producing population. Elimination of these cells (by complement-mediated lysis with the same antibodies) abrogated the IFN response, but NK cells remained and thus do not produce IFN-alpha. In confirmation, elimination of the Leu 11b+ cells had no effect on the amount of IFN produced. PBMC preparations from patients with chronic lymphocytic leukemia (CLL) appeared incapable of producing IFN-alpha but were shown to contain identifiable IFN-producing cells. The low or absent IFN levels in CLL are probably due to the relative scarcity of IFN-producing cells in their PBMC.

Diagnosis of cerebral arteriovenous malformation by colour Doppler examination.

A newborn presented in congestive cardiac failure, and colour Doppler ultrasonography confirmed a large aneurysm of the vein of Galen. The two-dimensional cranial ultrasound appearance was not diagnostic but colour Doppler examination highlighted the marked vascular abnormalities. The colour Doppler appearances were dramatically altered following coil embolisation of the vein of Galen. Colour Doppler examination is the technique of choice for diagnosis and follow-up of this unusual condition.

Echoencephalography

Fertilization failure of frozen mouse oocytes is not due to premature cortical granule release.

The proportion of mouse oocytes that were fertilized in vitro after storage at -196 degrees C in the presence of 1.5 M dimethylsulfoxide (DMSO) was significantly lower than in unfrozen controls (39% vs. 81%). Sperm failed to penetrate the zona pellucida of approximately 80% of the frozen oocytes that remained unfertilized. Removal of the zona restored fertilization to control levels, indicating that changes induced in the zona during freezing and/or thawing were the primary cause of fertilization failure. Sperm-oocyte fusion, sperm nucleus decondensation, and the resumption of meiosis in frozen oocytes appeared to be delayed but subsequently fertilization progressed normally. No evidence was found to suggest zona modification by the premature release of fucosyl-rich glycoconjugates of cortical granule origin onto the surface of the plasma membrane of frozen oocytes stained immediately after thawing with fluorescently labeled Ulex europaeus lectin. Only a few frozen (less than 5%) and control (less than 3%) oocytes that failed to fertilize in vitro had fucosylated molecules on the plasma membrane. Prolonged exposure of fertilized oocytes to DMSO at 4 degrees C did not alter the pattern of lectin binding. In conclusion, fertilization is inhibited in frozen-thawed oocytes by as yet undefined modifications to the zona pellucida which do not involve the premature release of cortical granules.

Hair follicle nevus.

Four cases of hair follicle nevi are described. Hair follicle nevus is an extremely rare hamartoma occurring on the face, which has had no clear histopathologic definition until now. A short review of the clinicopathologic features of this disease is presented.

The abdominal cocoon.

Since it was first described in 1978 the abdominal cocoon continues to be a rare cause of intestinal obstruction. So far this rare condition where the small intestine is encased in a fibrous membrane has been reported only in females. Diagnosis is usually made at laparotomy and the treatment of choice is lysis of adhesions. Proper recognition of this benign condition will result in the correct management of it and prevent unnecessary bowel resections. Five new cases including one male patient, together with a review of previous reports in the English literature, are presented.
