The overstimulation of everyday life: I. New aspects of male homosexuality.

Western heterosexual culture surrounds the homosexually inclined boy in a climate of erotic overstimulation that powerfully affects his development and adult sexual adaptation. This assertion is illustrated through a case presentation of a homosexual man who shared a bed with his brother from childhood into adolescence. Analysis of the patient's transference enactment--repeatedly falling asleep on the couch--gradually revealed the psychic impact of this everyday overstimulation: the creation of a tantalized inner world of longing. The regular occurrence and developmental understanding of adolescent homosexual boys' unrequited love affairs with adolescent heterosexual boys are described and explored. Finally, the overstimulation of everyday life is proposed as a new model for understanding certain behavioral aspects of male homosexuality, such as the avoidance of rough-and-tumble play in childhood and homosexual cruising.

A new analytic dyad: homosexual analyst, heterosexual patient.

The paradoxical thesis is presented that the extraordinary aspect of the analytic experience of a homosexual male analyst and his heterosexual male analysand is that it was ordinary, that the fundamental processes of transference, countertransference, and analysis of defense and resistance were determinative. The unique variations of these processes with this particular patient are explored. The patient entered treatment unaware of the analyst's homosexuality, which he discovered during the analysis. The course of this discovery, its transformations, its defensive uses, its transference meanings, and its fate in the termination are delineated. Through viewing the patient's reactions to the analyst's homosexuality as potential entry points to the transference, the analytic process was enhanced and facilitated.

Trauma and war. A fragment of an analysis with a Vietnam veteran.

The author examines the impact of the catastrophic trauma of war on symptom formation and development through the detailed, clinical account of a psychoanalysis with a Vietnam veteran. Theoretical dilemmas regarding adult trauma are explicated and a reformulated theory of etiology is proposed.

Tourniquets.

All tourniquet equipment must be regularly maintained. Cuffs and tubing must be checked for leaks, gauges calibrated regularly and automatic apparatus which depends upon reservoirs of gas must not be allowed to become depleted. A tourniquet is an invaluable piece of equipment when used correctly. Its advantages far outweigh its disadvantages, provided that proper precautions are observed.

Adolescent elopement from a psychiatric hospital. Multiple dimensions.

The authors report an unusual epidemic of elopement from an adolescent ward of a psychiatric hospital and demonstrate the utility of the single case study method with the hospital ward as the unit of study. The authors found that elopement was associated with patient factors (such as the absence of the diagnosis of affective disorder and a previous history of running away from psychiatric treatment) and patient-therapist interaction factors (separations from treatment team members), both of which seemed to interact with the milieu. The authors discuss how the single case study permits examination of multiple levels of sociopsychological processes.

Development of prototype equipment for innovative employment of blind and partially sighted persons.

Sensory Aids Foundation of Palo Alto, California, has funded development of a number of microprocessor-based or computer-related sensory aids to enable blind and partially sighted persons to enter innovative areas of employment previously inaccessible to them. Three of these devices are discussed in this paper: the prototype TSPS (Traffic Service Position System) console for blind telephone operators; an Optacon Tracking Guide for CRT displays; and a Digital Indexing System for cassette audio recorders. In addition, three other microprocessor-based products recently acquired by Sensory Aids Foundation for placement with blind or partially sighted clients are briefly described: the Speech Annunciator for digital laboratory instruments; the Large Print Video Terminal; and the TIM-II (Talking Information Management System). A new grant received from the Department of Health, Education and Welfare, the Projects With Industry Section, has allowed expansion of services to develop computer-related jobs for blind, deaf, and orthopedically disabled persons.
