Multi-modality therapy for epidermoid carcinoma of the anus.

Three patients presenting with surgically incurable epidermoid carcinoma of the anus were treated by radiation therapy and chemotherapy consisting of 5 fluorouracil and Mitomycin C. The first patient succumbed during the course of her therapy. The second patient is alive and well without definite evidence of disease nearly 1 1/2 years after initiation of therapy. The third patient underwent abdominoperineal resection after radiation and chemotherapy; pathologic examination revealed no residual carcinoma and the patient is well nearly 1 year after surgery. A fourth patient with previously treated epidermoid carcinoma of the anus presented with biopsy-proven pulmonary metastasis and was placed on the chemotherapeutic regimen alone. Six weeks later his chest X-ray showed essentially complete disappearance of the metastatic nodules. This experience suggests that multimodality therapy may increase salvage in even locally far-advanced and metastatic epidermoid anal carcinoma.

Carcinoid tumors of the gastrointestinal tract.

This study is based on a review of the records of 107 patients with primary carcinoids of the gastrointestinal tract diagnosed at the Memorial Hospital for Cancer and Allied Diseases during the 20 years from 1948 to 1968. The tumors were classified into two major groups according to their gross and pathologic presentation, and were correlated with clinical behavior and treatment. Carcinoids which were still intramural in location were usually asymptomatic, and yielded successfully to local excision (85% 5-year overall survival); carcinoids which had invaded into the serosa and/or beyond were usually symptomatic, and hd much poorer results (5% overall 5-year result), despite radical surgery with or without palliative radiotherapy and/or chemotherapy.

Anal fibrosarcoma: report of a case and review of literature.

A unique case of anal fibrosarcoma is reported. Review of the literature in the past half century revealed only 13 cases of rectal fibrosarcoma, and no case of anal fibrosarcoma. Abdominoperineal resection is the usual treatment if the disease is confined to the rectum or anus. An extended resection may be indicated in those patients who have more extensive disease. Long-term follow-up information was not available in the literature. The prognosis is presumed to be the same as if not worse than for fibrosarcomas in other parts of the body. Reporting of this rare tumor when found in unusual locations is encouraged.

Primary linitis plastica of the rectum.

Five cases of primary linitis plastica of the rectum are presented. The clinical, radiologic, and pathologic features of the disease are described with emphasis on the difficulty in establishing a correct preoperative diagnosis. Because the histologic features of primary and secondary linitis plastica are indistinguishable and because secondary involvement is twice as common as primary, it is mandatory that other sites of origin are excluded and that both the proximal and distal margins of the resected area be free of tumor.
