[Recent dermatologic aspects in collagenoses].

Advances in sero-immunological diagnostic procedures have led to more precise nosologic classification or subdivision of the family of the connective tissue diseases. Assessment of the prognosis and the course of the disease has thereby been improved, and more specific therapeutic measures have become possible as well. The current clinical and serological classification of lupus erythematosus and scleroderma are reviewed, new atypical and overlapping forms of connective tissue diseases are also pointed out. Recent clinical and diagnostic aspects of dermato-(poly-) myositis are considered. Several scleroderma-like conditions which are especially important in the differential diagnosis of acroscleroderma are discussed.

Collagen Diseases↗

[Balanitis and differential diagnosis].

In this synopsis the most common balanitides are presented and illustrated in a plate. Special consideration is given the differentiation of the balanitides from each other as well as from malignant diseases. Some modern concepts in the etiopathogenetic development and treatment are mentioned.

[Mycosis fungoides and Sézary syndrome. A retrospective study of 60 patients with correlation between histologic classification and survival time].

47 patients suffering from mycosis fungoides and 13 with Sézary's syndrome were retrospectively studied with regard to prognosis in relation to the histologic substrate of cutaneous lesions. On account of the wide spectrum of T-cell cytomorphology as well as the pattern and composition of the infiltrate, we distinguished 4 histopathological types: small cell lymphoid type (45%), mixed cell polymorphic type (40%), large cell type (immunoblastic and pleomorphic, 10%), and medium-sized cell type (5%). Statistical analysis revealed survival rates of 10 years in 68.3% for the small cell lymphoid type and in 30.3% for the mixed cell polymorphic type. All patients showing the large cell type died within 6 years after diagnosis; those bearing the medium-sized cell type died within 2 years after diagnosis. The forms of mycosis fungoides showing the last two types of infiltrate may be classified as lymphomas of extreme malignancy. The different histological features are not only significant with regard to the diagnosis but also permit prognostic estimation before systemic signs have become evident.

[Classification and prognosis of cutaneous T-cell lymphomas].

Modern concepts on the classification and prognosis of cutaneous T-cell lymphomas are discussed in this paper. The full spectrum of cutaneous T-cell lymphomas is not yet known. A new classification of histomorphological types of mycosis fungoides and Sézary's syndrome in correlation with prognostic features is proposed.

[Side effects of beta receptor blockers on the skin. Review and personal observations].

This review presents the wide spectrum of cutaneous and extracutaneous side effects of beta-adrenoreceptor-blocking agents. In addition, personal clinical observations are described. An induction period before the occurrence of clinical manifestations is characteristic, which is usually of approximately 1 year duration. The clinical picture is extremely polymorphous and may imitate many classical dermatoses. Psoriasiform, lichen planus-like, and eczematous "chronic" rashes are mainly observed. When these are associated with serious, sometimes life-threatening damage to other organs, they result in syndromelike symptom complexes. This is exemplified by the oculomucocutaneous syndrome and the pseudolupus erythematosus syndrome. Fibrinous peritonitis, in particular, may be lethal. In most cases, the histology of the cutaneous side effects corresponds to a lichenoid drug eruption. The pathogenetic mechanisms which appear to operate are the pharmacological effect on the epidermal beta-adrenergic adenylate cyclase-cyclic AMP system, on the one hand, and heterogeneous immunological reactions on the other. It may be concluded that caution should be observed in long-term therapy with beta-adrenoreceptor-blocking drugs.

[Importance of the proliferating trichilemmal cyst].

The biological behavior of proliferating trichilemmal cysts is commented on, based on two cases. Two female patients, 71 and 70 years of age, developed solitary monstrous tumours of the scalp in the course of repeatedly recurring trichilemmal cysts. The biological behaviour of both was marked by infiltration of the galea aponeurotica, and in the latter case, invasion of the calvaria and the venous sinuses of the brain with a lethal outcome. Histologically, the differentiation of the cyst walls ranged from pseudoepitheliomatous hyperplasia to areas largely lacking differentiation, indicating a trichilemmal carcinoma. Therefore, a more cautious evaluation of proliferating trichilemmal cysts, commonly underestimated as pseudocarcinomatous, is indicated. The second case in particular shows that on occasion an invasive carcinoma can arise in a trichilemmal cyst. Radical surgery can not only prevent recurrence with loss of differentiation, but also enable a better histological evaluation of the deep-lying tumour tissue.

[Side-effects of beta-receptor blockaders on the skin].

Cutaneous side effects due to beta-adrenoreceptor-blocking agents are clinically polymorphous. Apparently they appear after several months of continuous therapy. Atypical psoriasiform, lichen planus-like, and eczematous "chronic" rashes are mainly observed. The histological picture usually corresponds to a lichenoid drug reaction. More rarely the so-called pseudo-lupus erythematodes-syndrome is found, which cannot be differentiated from the "idiopathic" disease. Noteworthy extracutaneous manifestations are fibrosing processes, which may be irreversible. Pathogenetically, a delayed hypersensitivity reaction, and an action on the epidermal chalone system (epinephrine-adenylcyclase-c-AMP-complex) may be assumed. Consequently, continuous observation of the skin and mucous membranes is necessary, and medication should be withdrawn if cutaneous side-effects appear.

[Plasmocytoma-associated bullous hemorrhagic amyloidosis of the skin].

Case report of a 70-year-old woman with plasmocytoma-associated bullous "amyloid purpura" and a lambda light-chain IgA paraproteinemia. Cutaneous hemorrhage with blisters, erosions, and milia in the intertriginous areas were the main clinical features. Amyloid deposits in the walls of blood vessels, and the pericollagenous affinity of the amyloid in the dermis are likely to be responsible for the increased capillary fragility, i.e., the hemorrhagic tendency and the infrapapillary blisters. A differentiation from epidermolysis bullosa acquisita and porphyria cutanea tarda is possible by means of the histological level of the blisters.

[Lichen myxedematosus and scleromyxedema].

Review of the clinical picture, dynamics of the disease, associated symptoms and signs, etiopathology as well as treatment and prognosis of lichen myxedematosus and scleromyxedema. Diagnostic criteria are wax-like papules and leather-like skin thickening, an increase in concentration of acid mucopolysaccharides consisting mainly of hyaluronic acid in the dermis, lympho-plasmacytoid infiltrates in the skin and the bone marrow, normal thyroid function, and paraproteinemia of light chain type lambda or kappa. The nosologic connection of both manifestations implying a single disease entity is emphasized by two of our clinical observations of the transition of lichen myxedematosus to scleromyxedema. An attempt will be made to interpret this as a certain kind of myelomesenchymal syndrome.

[Bowenoid papulosis of the genitals (author's transl)].

Clinical report, histology and course of bowenoid papulosis of the genitals in 17 patients. Numerous papules are found on the external genitals of both sexes. There is a predominance of young adult males. The discrepancy between Bowen's disease-like histology and the patient age, as well as obvious benign course is noticeable. As a result of the benign course and age of patients and the distinct histological characteristics, it therefore appears correct to consider bowenoid papules of the genitals as an atypical morphological variation of condylomata acuminata. In any case an examination of the partners is indicated to discover possible "contact-infections".

[Genital bowenoid papulosis. Pseudo-Bowen's disease of the genital area].

This report deals with a new disease, which is characterized by multiple reddish papules on the genitalia of young adults. The histological picture shows changes of squamous cell carcinoma in situ. The possible etiology is discussed. It is too early to predict the biological behaviour of these lesions.

[Possible relations between Reiter's disease and psoriasis (author's transl)].

Description of a characteristic venereal type of Reiter's disease with a pustular exanthem and a balanitis circinata. In the course of the disease the pustular exanthem probably transformed into a typical Psoriasis vulgaris. In search of genetical markers it was not possible in this case to show the HLA-antigens, which are characteristic of the Reiter's disease and the Psoriasis vulgaris; the HLA-antigens A 10, B7 and B12 however could be proved. The question is up for discussion, wether a HLA-B 27-negative Reiter's disease might include a disposition to the appearance of a phenotypical Psoriasis vulgaris, the genotype of which is represented by rather "unusual" histocompatibility-gens.
