A point mutation in VP1 of coxsackievirus B4 alters antigenicity.

While coxsackievirus infections have been linked to several autoimmune diseases, very little is known about the immunogenicity of the coxsackieviruses. Using two genetically related variants of coxsackievirus B4, CB4-P and CB4-V, the relationship between virulence and antigenicity was examined. The virulent variant, CB4-V, was shown to be more antigenic than the avirulent CB4-P variant. The increased antigenicity of CB4-V was due to a single amino acid substitution in the VP1 capsid protein (a threonine residue at amino acid position 129), a site that had been previously identified as a major determinant of viral virulence. Thr-129 of VP1 is predicted to lie within a conformational B cell epitope. In addition, a nearby linear B cell epitope spanning residues 68 to 82 of VP1 was identified as a potential serotype-specific, neutralization antigenic site. The linear and conformational B cell epitopes of coxsackievirus B4 may be analogous to antigenic sites 1 and 1B of poliovirus. To address whether the increased antigenicity of CB4-V influenced the severity of disease, mouse strains that differ in their outcome to viral infection were analyzed. Mice that developed the most severe disease and succumbed to infection were more immunoresponsive than mice that survived infection with CB4-V. The data suggest that immune-mediated mechanisms play a role in the severity of CB4-V induced disease.

The Hyphecan cap: a biological fingertip dressing.

An occlusive biological dressing, the Hyphecan cap, was evaluated in the management of full-thickness pulp defects of fingers and thumb in 20 consecutive patients. The cap was applied directly to the tip of the injured finger and allowed to separate gradually over the course of several weeks as new skin regenerated. This approach proved simple and effective in achieving a good functional and cosmetic result within a relatively short time, and eliminated the pain and discomfort associated with conventional dressing changes. We recommend the use of this dressing material in the Accident and Emergency department for the out-patient management of de-epithelializing fingertip injuries in both children and adults.

One-week therapy with oral terbinafine in cases of tinea cruris/corporis.

The use of oral terbinafine in the treatment of superficial dermatophyte infections has been extensively studied, using different treatment regimens. To evaluate the efficacy of short-term therapy with oral terbinafine in cases of tinea cruris/corporis, 22 patients (21 male and one female) with mycologically proven tinea cruris/corporis, were included in the present study. Each patient received one tablet of terbinafine 250 mg daily for 1 week. Patients were followed-up for 6 weeks after completion of treatment. Clinical and mycological assessments were performed at the end of treatment, and at the end of the follow-up period. The mean sum of scores of signs and symptoms in all patients decreased significantly from 12.36 before treatment to 0 at the end of the follow-up period, and mycological investigations were negative in all patients at the end of the follow-up period. Our results show that 1-week therapy with oral terbinafine is highly effective in the treatment of tinea cruris/corporis.

Presynaptic effects of scopolamine, oxotremorine, noradrenaline and morphine on [3H]acetylcholine release from the myenteric plexus at different stimulation frequencies and calcium concentrations.

The inhibition by three modulators (oxotremorine, noradrenaline, morphine) of acetylcholine release from the myenteric plexus preincubated with [3H]choline was investigated at different stimulation frequencies and calcium concentrations. Moreover, [3H]acetylcholine release evoked by a low (0.1 Hz) or a high (10 Hz) stimulation rate was investigated at different calcium concentrations either in the absence or presence of scopolamine. A reduced calcium concentration (0.6 mmol/l) inhibited acetylcholine release more at 0.1 Hz (74% +/- 3%) than at 10 Hz (44% +/- 8%). Scopolamine enhanced the stimulated acetylcholine release at a calcium concentration of 1.8 mmol/l. At calcium concentrations higher than 1.8 mmol/l scopolamine failed to enhance transmitter release markedly. A reduction of the calcium concentration (less than 1.8 mmol/l) significantly enhanced the effect of scopolamine, when acetylcholine release was evoked at 0.1 Hz. Oxotremorine (10 mumol/l) completely suppressed acetylcholine release at 1 Hz (120 pulses). When 120 pulses were applied at 10 Hz the maximal effect was only a 64% inhibition and the concentration-response curve was significantly shifted to the right. However, after a reduction of both the train length or the calcium concentration oxotremorine produced a complete inhibition of acetylcholine release evoked at 10 Hz. In contrast to the effect of oxotremorine, the concentration-response curves for morphine and noradrenaline were similar at 1 Hz and 10 Hz.(ABSTRACT TRUNCATED AT 250 WORDS)

Therapy of bladder cancer.

Progress in the different modern methods of treatment influenced the strategy in the therapy of bladder cancer, adjusting it to the stage of the tumour. The basis of different ways of treatment is the transurethral tumour resection. Depending on the stage and grade resulting from this, further steps are taken. An exact indication for radical cystectomy and the possibility of an appropriate urinary diversion have improved the chances of success. Of the adjunctive measures we have to mention the intravesical cytostatic instillation therapy, the transurethral high-frequency hyperthermia and the intraarterial chemoembolization. The technique and the methods of application are presented here. An overall view is given of laser treatment and the immunological therapy of bladder cancer.

Ureterorenoscopy in the treatment of ureteral stones.

Ureterorenoscopy rendered the endoscopic removal of ureteroliths possible. Big distal ureteroliths which do not pass spontaneously and for which ureterolithotomy is unavoidable, are treated by means of ureterorenoscopy which makes ultrasonic disintegration or stone extraction with a Dormia loop possible, and this under optical control too. The different techniques and strategies are presented. In animal experiments alterations of the urothelium after exposure to ultrasound were examined. There is the possibility to combine ureterorenoscopy with the percutaneous method of litholapaxy or ESWL when treating ureteroliths in the upper part of the ureter which do not pass spontaneously.

Comparison of affinities of muscarinic antagonists to pre- and postjunctional receptors in the guinea-pig ileum.

The myenteric plexus-longitudinal muscle preparation of the guinea-pig ileum was preincubated with [3H]choline, then superfused and stimulated electrically (1 Hz 120 pulses). Oxotremorine reduced the evoked outflow of [3H]acetylcholine in a concentration-dependent manner. Each of the six antagonists (scopolamine, methylatropine, trihexyphenidyl, 4-DAMP, clozapine, pirenzipine) produced parallel shifts of the concentration-response curves for the prejunctional effects of oxotremorine. Similarly, in contraction experiments, the antagonists competitively antagonized the postjunctional responses to oxotremorine. The pre- and postjunctional pA2 values did not differ significantly for any of the antagonists. It is concluded that pre- and postjunctional muscarinic receptors in the guinea-pig ileum are pharmacologically similar.

[Operative ureterorenoscope for the use of ultrasound and calculus extraction].

Ureteroscopy is used not only to clarify tumorous processes in the area of the ureter but also for removal or ultrasonic destruction of distal ureteral stones. Instruments have been developed that allow application of an optically controlled ultrasonic or metal loop. With our instrument we removed distal ureteral stones in 12 cases applying ultrasound or a Dormia loop. The different techniques and strategies are presented. In animal experiments we investigated the effect of ultrasound on the urothelium. We found that short time application (less than 30 s) does not leave irreversible damages. Removal of ureteral stones by ureteroscopy is indicated in stones which do not pass spontaneously or after unsuccessful efforts to remove the stones by means of a Zeissloop that is in cases where hitherto the only way of action possible was to carry out an ureterolithotomy.

Pirenzepine does not discriminate between pre- and postsynaptic muscarine receptors in the guinea-pig small intestine.

The effects of pirenzepine and scopolamine on presynaptic muscarine receptors (mediating inhibition of acetylcholine release) and postsynaptic receptors (mediating smooth muscle contraction) were compared in the myenteric plexus-longitudinal muscle preparation of the guinea pig. The preparation was preincubated with 3H-choline and subsequently superfused with Tyrode's solution. Field stimulation (1 Hz; 2 min) caused an outflow of 3H-acetylcholine that was depressed by the muscarinic agonist oxotremorine. Both pirenzepine and scopolamine produced shifts to the right of the concentration-response curve for the inhibitory effect of oxotremorine. Similarly, in contraction experiments, pirenzepine and scopolamine competitively antagonized the responses to oxotremorine. From the dose ratios pA2 values for pre- and postsynaptic effects were calculated. The presynaptic pA2 values for pirenzepine (6.90) and scopolamine (9.07) did not differ significantly from the respective postsynaptic pA2 values (6.66 and 9.40). Thus, pirenzepine like scopolamine does not distinguish between muscarine receptors that mediate contraction and those which mediate inhibition of acetyl-choline release.

Arrhythmias and conduction disturbances in children at the Department of Child Health, School of Medicine University of North Sumatra/Dr. Pirngadi Hospital Medan.

A retrospective study on arrhythmias and conduction disturbances was conducted in 1986 in the Division of Pediatric Cardiology of Dr. Pirngadi Hospital, Medan. There were 106 (35.33%) cases with arrhythmias and conduction disturbances out of 300 patients studied. The most common type of arrhythmia among the patients under 5 was intraventricular conduction defects, whilst in the age group of 5-10 were sinus tachycardia and intraventricular conduction defects, and in the age group of 10-15 was first degree A-V block. Rheumatic heart diseases and VSD were the most common cardiac diseases associated with arrhythmias, where the frequency were 18.86% and 10.37%, respectively. Arrhythmias associated with several extracardiac diseases were found in 60 (56.61%). Two out of 7 cases treated with antiarrhythmic agents died with SVT associated with bronchopneumonia, and atrial fibrillation associated with gastroenteritis, dehydration and malnutrition. Treatment against the main diseases (without antiarrhythmic agents) was done in the remainder. Even though the overall mortality rate was 13.20%, it was not due to arrhythmia itself but most of them died of non-arrhythmic origin.

Rheumatic fever and rheumatic heart disease at the Department of Child Health, School of Medicine, University of North Sumatera/Dr. Pirngadi Hospital, Medan (1983-1985).

A retrospective study on rheumatic fever and rheumatic heart disease in children was conducted at the Department of Child Health, Dr. Pirngadi Hospital Medan, during 1983-1985. The patients consisted of 43 females and 30 males. Most of the patients were over 12 years of age. Of the 73 patients there were 60 patients (82.19%) accompanied by valvular disorders. The most frequent major criteria of Jones found in this study were carditis and polyarthritis. Thirty one patients (42.46%) had functional status (NYHA) of grade II-IV, and 43 patients (58.90%) had cardiomegaly. The major ECG findings were enlargement of the atria or ventricles and first degree AV block. Compliance was only achieved in 25 (34.24%) cases. Rheumatic fever and rheumatic heart disease still remain a challenge for the medical professionals to cope with in the region.
