Laryngofissure and cordectomy for glottic carcinoma limited to the mid third of the mobile true vocal cord.

The objective of this study was to analyze the long-term results of laryngofissure with cordectomy for invasive glottic squamous cell carcinoma limited to the mid third of the mobile true vocal cord. The authors conducted a retrospective review of the medical charts and operative files of 33 patients with invasive glottic carcinoma limited to the mid third of the mobile true vocal cord managed with laryngofissure and cordectomy. A 10-year follow-up was achieved in 30 patients (90.9%). Kaplan-Meier actuarial analysis of survival, local control, nodal recurrence, distant metastasis, and second primary metachronous tumor was performed. The 5-year actuarial survival, local control, nodal recurrence, and distant metastasis estimates were 97%, 100%, 0%, and 0%, respectively. Tracheotomy was never performed. The overall laryngeal preservation rate was 100%. The 5- and 10-year actuarial metachronous second primary tumor estimates were 3% and 11.5%, respectively. The authors' experience suggested that laryngofissure and cordectomy should still be considered a valuable oncologic option for the management of invasive glottic carcinoma limited to the mid third of the mobile true vocal cord.

Functional outcome and prognosis factors after supracricoid partial laryngectomy with cricohyoidopexy.

A retrospective analysis of the medical records and operative files of 124 patients who consecutively underwent supracricoid partial laryngectomy with cricohyoidopexy (SCPL-CHP) was undertaken. The objective of this study was to analyze the postoperative course and functional results of SCPL-CHP. The mean duration of follow-up was 7 years (range 1 to 20 years). The postoperative mortality was 1.6%. The average times until tracheostomy and nasogastric feeding tube removal were 8 and 22 days, respectively. All patients but 1 were decannulated. The rate of pneumonia from aspiration was 11.5%. Completion total laryngectomy and permanent gastrostomy were required in 2.5% and 2.5% of patients, respectively. Overall normal swallowing without permanent gastrostomy and respiration without tracheostomy were achieved by the first postoperative year in 91% and 99.1% of patients, respectively. Univariate analysis of the potential correlation between various variables and the duration of the tracheostomy and nasogastric feeding tube, the incidence and causes of mortality, the incidence and type of the various complications, the recovery of swallowing, and the length of hospitalization is presented. The report also discusses the management and follow-up care.

Differences in intravenous and subcutaneous application of recombinant human erythropoietin: a multicenter trial.

The aims of this clinical study were to compare the maintenance doses for intravenous (i.v.) and subcutaneous (SC) administration of recombinant human erythropoietin (rhEPO) and to investigate whether there is any difference in the increase of the packed cellular volume (PCV) per week under i.v. and SC administration of rhEPO from two production sites (Genetics Institute, Cambridge, USA; and Boehringer Mannheim, Penzberg, Germany). A total of 90 patients suffering from end-stage renal disease were included in the study. All patients had already been treated for at least 6 months with chronic hemodialysis. The study was carried out as a randomized, multicenter parallel group comparison study with a 1-week pretreatment phase, a subsequent 8-week double-blind phase, and a final open phase. The final open phase consisted of a correction phase and a maintenance phase. The production site had no influence on the PCV increase per week, and there were no differences with respect to tolerability. The median rhEPO dose required to maintain the target PCV of 30 to 35 vol.% was 33 U/kg body weight three times a week in the i.v. group compared with 22 U/kg in the SC group (i.e., an average of 30% less with SC administration). Development or aggravation of hypertension under rhEPO therapy was observed, especially during the correction phase and more frequently in the SC group than in the i.v. group. During the maintenance phase, there was no essential difference between the two groups.

[Inverted nasosinusal papilloma and lateronasal rhinotomy].

The authors report about 18 cases of inverting papillomas treated between 1972 and 1992. (17 men and 1 woman). The minimum follow-up was of 30 months. The mean age was 55 years with ages ranging from 26 to 85 years. The most frequent symptom encountered was unilateral nasal obstruction. A synchronous carcinoma occurred in 2 patients. Treatment was always surgical: lateral rhinotomy and total ethmoidectomy. A local recurrence occurred in 1 on 16 benign inverting papillomas (6.2%). No local recurrence occurred in malignant inverting papillomas treated by surgery and followed by postoperative radiation. A metachronous malignant papilloma never occurred. Because of the high risk of recurrence and malignant transformation and after analyzing the literature, the authors recommend a lateral rhinotomy and total ethmoidectomy with as limited after effects as possible.

[Malignant transformation of inverted papilloma of the nasal sinuses.. Apropos of a series of 7 cases].

Seven cases of malignant inverting papillomas are presented (6 men and 1 woman) with ages ranging from 31 to 72 years (mean age: 58 years); the minimum follow up was 37 months. The diagnosis was based on the histopathologic analysis of biopsy in five patients. In two patients the diagnosis was confirmed by the pathological analysis of the specimen. Treatment was always surgical: lateral rhinotomy and total ethmoidectomy. Neoadjuvant chemotherapy was given to four patients. Post operative radiation therapy was delivered in five patients. Five patients on seven died: local recurrence and systemic metastases: 3; systemic metastases: 1; second primary: 1. Reports from the literature are discussed. The authors emphasize the treatment modalities based on complete tumor removal through lateral rhinotomy.

Neurobehavioral functioning and social behavior of children at risk for schizophrenia.

The neurobehavioral functioning and social behavior of children with mentally ill and mentally healthy parents were examined. The children, 8 to 13 years of age, were assessed with a battery of neurobehavioral tests. The children's teachers rated their interpersonal behavior. Each of the families was rated on quality of childbearing environment. Children of schizophrenic parents performed more poorly on the neurobehavioral tests, in particular those assessing perceptual-cognitive functioning, than children of parents with other psychiatric disorders or with no history of mental illness. There were no group differences in aggressive behavior. Sons of schizophrenic parents were rated as more withdrawn than the other children in the study. The relationship between childrearing environment, neurobehavioral functioning, and social behavior was in the expected direction for the children of mentally healthy parents. An atypical pattern emerged for the offspring of mentally ill parents.

A process model for the development of schizophrenia.

Less than two decades ago scientists interested in the etiology of schizophrenia were still vigorously debating the nature-nurture controversy, with many adhering to the view that the illness grew either out of a constitutional deficit or a pathological family environment. Today, virtually all researchers adopt interactional models that include both constitutional and environmental factors. Interactional models are most often expressed in diathesis-stress terms: Development of schizophrenia requires both a biological vulnerability (diathesis) and stressful life circumstances that facilitate expression of the illness. Modern debate within the field now primarily focuses on understanding the characteristics of the biological diathesis, the stressful environment, and their interaction. Scientists using diathesis-stress theories have employed several research strategies (Rosenthal 1970): (1) consanguinity studies exploring the distribution of illness among the relatives of schizophrenics, (2) twin studies comparing the concordance of schizophrenic illness in monozygotic and dizygotic twin pairs reared together or apart, (3) retrospective studies examining the premorbid behavior and development of schizophrenic parents, and (4) prospective studies of children at high risk for schizophrenia. The population most frequently followed in prospective high-risk studies has been offspring of schizophrenics. Biological offspring of schizophrenics who are reared by their parents are at extremely high risk for becoming schizophrenic themselves, approximately ten times greater risk than the general population. The present paper will make use of the Israeli High-Risk Study to test a diathesis-stress model for the transmission of schizophrenia. The goodness of fit of this model to the data will be explored using a simple decision tree data analytic approach.

[Bartter syndrome and pseudo-Bartter syndrome--a case report].

One patient with Bartter-syndrome and two patients with pseudo-Bartter-syndrome in abuse of laxatives and diuretics, respectively, were examined clinically and biochemically. The causes of the severe disturbances of the electrolyte metabolism are demonstrated. The differential diagnosis between Bartter- and pseudo-Bartter-syndrome is discussed, exact investigations of electrolyte balance of intake and output are decisive. In consequent therapy with prostaglandin inhibitors (indomethacin), aldosterone antagonists (verospirone) and oral electrolyte supply the prognosis of the Bartter-syndrome is favourable. Little successful is the treatment of the pseudo-Bartter-syndrome. On account of the severe psychic disturbance of the personality a renunciation of laxatives and diuretics, respectively, is frequently not to be achieved.

Successive visual discrimination of forms in 10-month-old infants.

The purpose of this research was to assess the infant's ability to make successive discriminations in the size of a figure. The Ss were 90 10-month-old infants who were habituated to a block figure and dishabituated on different subtle size transformations of that figure. Dishabituation data revealed that the infants were able to detect transformations which increased the area of the habituated figure by 7 percent. There was no evidence of a differentiated sensitivity to detect a change in any special location of the original figure.

Infant responsivity to curvature.

The reactions of 112 10-month-old male infants to the property of curvature were examined using a habituation paradigm with lever pressing as an instrumental response. 4 levels of curvature (straightt line, minimal curve, moderate curve, large curve) each served as a standard (habituation) stimulus and as a transformation stimulus in 9 separate experimental conditions. Results revealed a greater initial dishabituation of reinforced instrumental responding in those conditions crossing the curved/straight boundary; furthermore, this initial dishabituation was sustained throughout the transformation phase of the experiment in those conditions which involved changes from straight to curved, but not curved to straight. Results are interpreted as implying a special attention-recruiting value for curvature in 10-month old infants.

[Results of scintigraphy with labeled leukocytes for the diagnosis of infectious hip prosthesis loosening].

The present paper reports on the results of a specific scintigraphic examination method for diagnosing infections in cases of loosening of hip prostheses by means of labeled granulocytes. In 103 patients the result of the examination was compared with the result of the bacteriological and/or histological investigation following removal or replacement of the implant. Where there was an infectious loosening of the prosthesis the findings were in agreement with the results of the scintigraphic examination in 83.1% of the cases. It was possible to rule out infectious loosening of the prosthesis in 93% of the cases thus investigated.
