Topographical origin and ganglion cell type of the retino-pulvinar projection in the cat.

In order to examine the pattern of the retino-pulvinar projection in the cat, the existence of which has been recently demonstrated using autoradiographic fiber tracing technique, a small amount of horseradish peroxidase (HRP) was injected into the lateral part of the pulvinar nucleus at various rostocaudal levels. The retrogradely labeled ganglion cells were analyzed in terms of their topographical location and cell size, as seen inretinal whole mounts. The results were compared with those obtained following injections into the lateral geniculate nucleus. Retrogradely labeled cells were found in the retina bilaterally after injections of HRP into the pulvinar nucleus. Pulvinar injections produced labeling of retinal cells in the nasal half of the retina contralaterally, and in the temporal half ipsilaterally. The labeled cells were diffusely distributed in a retinotopically organized fashion. The representation of the area centralis in the retino-pulvinar projection is displaced rostrally as compared with the retino-geniculate projection. All labeled cells after pulvinar injections were medium to small size and no large cells were encountered.

Malignant melanoma producing serotonin.

A case of malignant melanoma, the tumor of which seems to produce serotonin, is presented. Neither hyperserotoninemia nor carcinoid syndrome was observed in the clinical course. An autopsied sample of the metastatic lung tumor contained a significantly higher concentration (1.35 microgram/g wet weight tissue) of serotonin than that of normal lung (below 0.5 microgram wet wt.). Tryptophan hydroxylase activity, the key enzyme of the biosynthesis of serotonin, was substantial in the same tumor sample when assayed by a sensitive radioisotopic method, supporting the productivity of the amine of the tumor. The ability of melanoma to produce serotonin could be attributed to its neural crest origin.

Endoscopic diagnosis of intestinal metaplasia in asymptomatic (control) volunteers.

The F-P border and strain phenomenon of the gastric mucosa were investigated by the application of methylene blue dye spraying method in endoscopy to 105 asymptomatic control volunteers and the following results were obtained; 1. The pyloric metaplasia is observed in the fundic gland area from the twenties in age and becomes increasing in its number and move widely spreading from the lesser curvature to the anterior and/or posterior wall of the corpus with advancing age. 2. The intestinal metaplasia arises from the thirties in age. 3. The intestinal metaplasia is observed either in the pyloric gland area of following the pyloric metaplasis in the fundic gland area. 4. Histologically, the strain phenomenon of the gastric mucosa is closely related to the intestinal metaplasia. Then, methylene blue dye spraying method is reevaluated to be useful for a precise endoscopic diagnosis of intestinal metaplasia.

[A fundamental study on a bioassay for the antiphlogistic effect of topically applied antiinflammatory agents (author's transl)].

To establish a reliable laboratory assay for quantitating topical anti-inflammatory efficacy, the method of Tonelli et al was modified by employing felt fixed forceps, felt and sharp punch in Wistar rats. Croton oil, applied topically to the rat ear, elicited an acute phlogistic response which was maximal 6 hr after the application. The phlogistic response elicited by a single topical application of croton oil (1, 2, 5 and 10%) was increased in a dose-dependent manner, and croton oil, 5%, induced 63.2% increase of ear weight and was the optimal concentration for the experiment. Using this procedure, the antiphlogistic potencies of two corticoids were assayed under conditions of a blind test. ED50 of betamethasone valerate and diflucortolone valerate was 0.26 mg/ml and 0.0097 mg/ml, respectively, in Wistar rats, and 0.86 mg/ml and 0.016 mg/ml in Sprague Dawley rats, suggesting that diflucortolone valerate has an antiphlogistic potency of 27-56 relative to that seen with betamethasone valerate with minor differences in the strain of the rat. Our method should prove to be a useful assay for rapidly quantitating antiphlogistic activities of topically applied corticoids.

Methylmalonic acidemia.

A patient presenting with severe metabolic acidosis accompanied by hyperglycinemia, hyperuricemia, hypoglycemia and hypertammonemia is described. Metabolic acidosis was found to be due to accumulated methylmalonic acid and did not respond to vitamin B12 administration. The patient showed favorable growth and development when kept on a low isoleucine, methionine, threonine and valine diet. In vitro studies using a lymphoid cell line derived from the patient showed a deficiency of methylmalonyl-CoA carbonyl-mutase.

Acidosis

Demonstration of argyrophil granules in small cell carcinoma of the lung.

The Grimelius silver nitrate stain has enabled us to demonstrate the presence of tumor cells with argyrophil granules (argyrophil cells) in small cell carcinoma of the lung. Of the 22 tumors, 11 showed varying numbers of argyrophil cells. The occurrence of the cells differed in frequency among the subtypes of small cell carcinoma. The fusiform cell type showed the cells more frequently than the other types. Both tumors with numerous argyrophil cells belonged to the fusiform cell type. The number of positive cells seen under the light microscope did not correlate with the number of cells containing neurosecretory granules under the electron microscope, nor with the amount of either ACTH or serotonin in the tumor extracts. The demonstration of these cells in a pulmonary carcinoma may be of help in making correct histological diagnosis.

A ventral lateral geniculate nucleus projection to the dorsal thalamus and the midbrain in the cat.

Retrograde tracing experiments using horseradish peroxidase (HRP) have been utilized for demonstrating the origin of efferent projections of the ventral lateral geniculate nucleus (LGNv) in the cat. HRP-positive cells identifiable as origins of thalamic projections were found in LGNv after injections of HRP into the lateral central intralaminar nucleus. The labeled cells appeared concentrated in the medial part of the internal division of LGNv, consisting of medium-sized multipolar cells. Contralaterally, fewer labeled cells were present in the corresponding part of LGNv. In the case of injections of HRP into the midbrain (pretectum and superior colliculus), labeled cells in LGNv were distributed almost exclusively in its external division, composed of mainly small cells. Little overlap of the distribution of HRP-positive cells was seen in LGNv between the thalamic and midbrain injection cases.

Dosage schedule of gentamicin for chronic renal insufficiency in children.

Gentamicin was given to paediatric patients with chronic renal disease complicated by infections by Gram-negative organisms, in which renal function varied from normal to severely insufficient. Peak serum levels after an intramuscular dose of 1 mg/kg body weight ranged from 3.1 to 9.4 microgram/ml, which appeared adequate for therapy. The peak value was not related to the renal function of the individual patients. The serum half-life of gentamicin correlated inversely with the value for endogenous creatinine clearance. A diagram for the estimation of the serum half-life of gentamicin using the creatinine clearance value is presented. As a practical guide, it is recommended that the dose of gentamicin in children with renal function impairment be 1 mg/kg given intramuscularly and that the interval between doses be almost three times as long as the serum half-life, which can be estimated by means of the diagram for individual patients. The accuracy and safety of this method were confirmed by treating children with this adjusted dosage schedule.

Surface tension measurements on methacrylate monomer solutions, selected solvents, and gel extracts.

The surface tensions of selected methacrylate monomer and solvent solutions were measured, and calibration curves were generated. The curves were then used to deduce the amounts of water soluble extractables in methacrylate hydrogels by measuring the surface tension of the extractant. The detectability limit (parts per 10,000) and the overall sensitivity of the method suggests its possible use as an extraction monitor test for related biomaterials.

Acrylates

[A regional cerebral blood flow in patients with acute subarachnoid hemorrhage--with special references to spasm (author's transl)].

Measurements of the regional cerebral blood flow (rCBF) were performed 44 times on patients with subarachnoid hemorrhage, using the 133-Xe clearance method with aid of the 8 channel detector unit. The patients consisted of 31 preoperative cases within 3 weeks of the last subarachnoid hemorrhage. The sites of the ruptured aneurysms included 11 anterior communicating arteries, 8 middle cerebral arteries, 7 internal carotid arteries and 3 anterior cerebral arteries. In 2 cases, the aneurysms were not identified with the cerebral angiograms. The purpose of this study was to investigate the effect of spasm on the rCBF. The subjects were divided into two groups: those with and without spasm. Furthermore, the group with spasm was divided into three subgroups according to the degree of spasm. The findings obtained were as follows; The group with spasm showed a statistically significant decrease in the mean of the rCBF. The MrCBF was not influenced by the distribution of spasm, however. In the group without spasm, the MrCBF had a good correlation with the cerebrospinal fluid pressure (CSF-P) or the level of consciousness. In the group with spasm, there was no correlation between the MrCBF and the CSF-P or the level of consciousness. For example that marked reduction of the MrCBF were seen in some cases with low CSF-P or only slight disturbance of consciousness. The rCBF was measured 2 or 3 times in 5 cases with spasm. The decrease in MrCBF occured about one week after the incidence of the subarachnoid hemorrhage but MrCBF recovered with the disappearance of spasm 2 or 3 weeks after the attack. There was a poor correlation between spasm and relative flow patterns. The good correlation, however, was found in only one case with the severe spasm at the middle cerebral artery.

Acute Disease

Behavior of asynergic ventricle.

Effect of increased left atrial pressure and a positive inotropic agent upon asynergic left ventricle which was produced by acute occlusion of LAD were examined in 8 open-chest dogs. Systolic bulging in ischemic area was qualified by ESL/EDL in percentage. Left atrial pressure was controlled by the reservoir connected to left atrial appendage with a large bore tubing. Cardiac output and aortic pressure were decreased following coronary occlusion without change in isometric developed tension in nonischemic area of left ventricle. Increase of left atrial pressure from 5 mmHg to 14 mmHg produced rise in stroke volume to 191% and rise in aortic pressure to 156% in asynergic left ventricle respectively. These increases were not accompanied by change in ESL/EDL. Therefore, extracardiac factors were assumed to explain the prominent increase in left ventricular stroke work by elevation of left atrial pressure in asynergic left ventricle. Isoproterenol enhanced total cardiac performance by increasing the function of nonischemic area without deterious effect on the ischemic area.

Leukonychia striae.

A 19-year-old keypuncher had leukonychia striae. The pattern of striae suggested that they occurred as a result of the mechanical stimulation of keypunching.

Diagnosis of the level and severity of cord lesion in cervical spondylotic myelopathy. Spinal evoked potentials.

Experimental and clinical studies were performed to measure the segmental spinal evoked potential (SEP) of the cervical cord after stimulation of the median nerve, to determine the location, severity, and outcome of cord lesion in patients who had cervical spondylotic myelopathy. The SEP in control subjects consisted of two waves, the primary reactive R-wave and the secondary reactive N-wave. The R-wave related to both the dorsal root potential and the funiculus posterior potential; the N-wave related to the post-synaptic potential in the spinal cord. In the early stages of cervical spondylotic myelopathy, the N-wave weakens or disappears at the level of lesion. Then, as damage progresses, the R-wave also weakens or disappears.
