Occlusion of renal arterial tumor supply with absolute ethanol. Experience with 20 cases.

Absolute ethanol was used for renal ablation in 20 patients with carcinoma of the kidney. The procedure was followed by nephrectomy in 6 patients without metastases, all of whom are alive without evidence of recurrence 4 to 20 months after treatment. Fourteen patients with metastatic or locally advanced disease were embolized to palliate pain or hematuria without subsequent nephrectomy. Of these only 2 are alive, 4 and 18 months following the procedure. The technique for ethanol embolization is described and the use of occluding balloon catheters is recommended. No complications were encountered and the 'postembolization syndrome' was less marked as compared with a previous material of equal size where other methods of renal artery occlusion were employed. Microscopic examination in 10 of the infarcted kidneys showed extensive tumor necroses, sometimes with no evidence of viable tumor cells at all. Natural killer cell activity was determined before and 48 hours after renal artery occlusion in 7 patients, showing somewhat increased activity in 4. It is concluded that absolute ethanol is a safe, effective and easily administered agent for the infarction of renal tumors.

Blood hormone levels related to stages and grades of prostatic cancer.

Human follicle stimulating hormone (FSH), human luteinizing hormone (LH), human prolactin, testosterone, estradiol, and sex hormone binding globulin (SHBG) were measured in the blood of 100 consecutive new patients with prostatic carcinoma. According to the VACURG classification, 14% of the patients were in Stage I, 25% in Stage II, 25% in Stage III, and 36% in Stage IV. Within each stage the tumors were further subdivided into either highly, moderately, or poorly differentiated carcinomas. Thus the whole group consisted of 12 categories (stages and grades) of prostatic carcinoma. No specific hormonal pattern emerged which could distinguish any particular category or group of categories. We, therefore, conclude that single hormone measurements are of no practical value for diagnostic purposes related to staging and grading of prostatic carcinoma.

In vitro and in vivo effects of diethylstilbestrol and estramustine phosphate (Estracyt) on the mitogen responsiveness of human peripheral blood lymphocytes.

The responsiveness to diethylstilbestrol (DES) and estramustinephosphate (EMP) of human peripheral blood lymphocytes to T-cell mitogens has been investigated in vitro and in vivo. EMP demonstrated potent inhibition of both Con A- and PHA-induced lymphocyte proliferation in vitro, while it had no detectable effects when given to patients with cancer of the prostate. DES reduced the response to Con A in vitro, but had only marginal effects on PHA-induced mitogen response. In contrast, the response to Con A was unaltered, while the response to PHA was significantly diminished after DES therapy in patients with prostatic cancer. This effect, however, was only seen when high doses of DES not included in conventional regimen were given. The proliferative response to T-cell mitogens in patients with prostatic cancer was not affected by serum source in the assay, indicating the absence of humoral factors able to inhibit mitogen response in these patients.

Effect of treatment with diethylstilbestrol-polyestradiol phosphate or estramustine phosphate (estracyt) on natural killer cell activity in patients with prostatic cancer.

We evaluated the effect of treatment of patients with prostatic cancer with estramustine phosphate or the combination diethylstilbestrol-polyestradiol phosphate on the natural killer cell activity in peripheral blood. Although estramustine phosphate did not affect natural killer cell activity, diethylstilbestrol-polyestradiol phosphate substantially reduced natural killing after a treatment period of 1 week. The activity was only slightly further lowered after 4 weeks of treatment. Possible clinical implications of the difference in susceptibility of natural killer cells to these agents are discussed.

Distribution of microspheres with different diameters in the small bowel wall of the cat.

The purpose of the present study was to investigate the distribution in the small bowel wall of intracardially injected microspheres with different diameters, and to find a sphere size that could be used for determination of blood flow to the different layers of the bowel wall. Microspheres were injected into the left ventricle of the hearts of cats in the following order of succession, 8--10, 15 +/- 5, 25 +/- 5 and 50 +/- 5 micrometer. Samples of the proximal part of the jejunum and the distal part of the ileum were examined microscopically, and the size and the location of the spheres recorded. The following distribution of the microspheres was found: villi 6--16 micrometer, crypt layer 6--20 micrometer, propria below the crypts and the muscularis mucosa 11--24 micrometer, submucosa 9--66 micrometer and muscularis 6--30 micrometer. Spheres between 8 and 13 micrometer appeared to be fairly uniformly distributed in the different layers of the intestinal wall.

Independent prognostic importance of microvessel density in clinically localized prostate cancer.

BACKGROUND: Previous studies have reported a possible prognostic importance of microvessel density (MVD) in prostate cancer, although the significance after radical prostatectomy is not clear. The purpose of this study was to assess the prognostic value of MVD in clinically localized prostatic adenocarcinomas, focusing on moderately-differentiated tumours. MATERIALS AND METHODS: We examined a series of 104 patients treated for presumed organ-confined cancer in the period 1988-94. The area of highest tumour grade was selected from the prostatectomy specimens and vessels were high-lighted by staining for factor-VIII-related antigen. MVD was quantitated in the "hot spot" area and related to biochemical failure and clinical recurrence. RESULTS: In moderately differentiated tumours (WHO grade) (n = 66), MVD was associated with preoperative s-PSA and positive surgical margins. In univariate 5-year analysis, microvessel density (MVDmean > 122 mm-2, median) (p = 0.0074), s-PSA, tumour dimension, capsular penetration, seminal vesicle invasion and positive surgical margins were all significant predictors of biochemical failure, while MVDmean (p = 0.0084) was the only statistically significant predictor of clinical recurrence. In multivariate Cox' analysis, MVDmean (p = 0.0003), capsular penetration and tumour dimension remained as independent predictors of biochemical failure. CONCLUSION: Assessment of MVD in moderately differentiated prostatic adenocarcinomas may improve the prognostic stratification of patients after radical prostatectomy.

Maximum Ki-67 staining in prostate cancer provides independent prognostic information after radical prostatectomy.

BACKGROUND: We have evaluated the prognostic value of Ki-67 growth fraction after radical prostatectomy, especially focusing on intermediate grade carcinomas. MATERIALS AND METHODS: 104 patients treated by radical prostatectomy for clinically localized prostate cancer were studied. The area of highest tumour grade was selected from the prostatectomy specimens and used for Ki-67 immunostaining The fraction of Ki-67 positive tumour nuclei in the area of most intense proliferation ("hot spot") was estimated, and related to biochemical failure. RESULTS: Ki-67 expression (median 6.7%, range 1.2-42.6%) was significantly associated with WHO histological grade. In univariate analysis of all 104 carcinomas, Ki-67 expression was associated with time to biochemical failure as were age, tumour dimension, WHO histological grade, pathological stage, positive surgical margins and pre-operative s-PSA. In multivariate Cox' analysis, Ki-67 expression, pathological stage and pre-operative s-PSA remained as independent predictors of time to biochemical failure. Ki-67 expression (HR 4.8, p < 0.001) was also found to be an independent predictor among moderately-differentiated carcinomas. CONCLUSION: Estimates of Ki-67 growth fraction in areas of highest tumour grade may prove to be a useful prognostic biomarker after radical prostatectomy.
