Effect of the alpha-1 adrenoceptor blocker on tissue norepinephrine contents in human benign prostatic hyperplasia.

To see the effect of the alpha-adrenergic blocking agent on tissue norepinephrine contents of the prostate, norepinephrine (NE) levels were investigated in patients with symptomatic benign prostatic hyperplasia (BPH). Morphometrical analyses were also performed to detect the differences in tissue composition. Nineteen patients were subdivided into two groups. Patients in Group 1 were given tamsulosin hydrochloride (0.2 mg/day) for at least 4 weeks before transurethral resection of the prostate (TURP), while patients in the control group (Group 2) underwent no previous treatment for BPH before TURP. Tissue NE contents were investigated by high performance liquid chromatography on a chip of the prostate obtained by TURP. These two groups were closely similar in age distribution, prostatic size, results of preoperative symptom scoring and relative proportion of smooth muscle component within the evaluated specimen. In the present series, tissue NE contents in these groups were not statistically different (p = 0.64). Chronic and acute administration of the alpha-1 antagonist did not apparently influence the tissue NE level in patients with BPH.

A case of primary giant calculus in female urethra.

Urinary calculus is rarely seen in the urethra and is usually encountered in men with urethral stricture or diverticulum. Primary urethral calculi are extremely rare in females. We describe a case of a giant urethral stone impacted in the urethra of a 103-year-old female.

Juxtaglomerular cell tumor of the kidney: report of a non-functioning variant.

A case of a juxtaglomerular cell tumor (JCT) in a 46-year-old man is reported. The tumor, 2.4 cm at its greatest dimension, was incidentally detected by ultrasonography. Although histological, immunohistochemical, and electron microscopic examinations revealed typical features of a JCT, the patient had no history of hypertension or hypokalemia. This is the first report of a non-functioning JCT in the literature.

Ice water test in patients with overactive bladder due to cerebrovascular accidents and bladder outlet obstruction.

To investigate the factors which influence the results of the ice-water test (IWT) in patients with cerebrovascular accidents or infravesical obstruction, IWT was performed on these patients with urge urinary incontinence. Thirty-two patients with cerebrovascular lesions (group 1) and 16 patients with benign prostatic hyperplasia (group 2) who demonstrated overactive detrusor function on cystometry were evaluated. Ice-chilled sterile saline was infused into the empty bladder through a balloon catheter, and detrusor pressure was simultaneously recorded. Twenty of the 32 patients (62.5%) in group 1 showed positive IWT. In the positive IWT group, threshold volume at involuntary detrusor contraction was significantly lower than that of the negative IWT group (p < 0.05). The incidence of a positive IWT appeared to be higher in patients with pontine lesions than in those with multiple cerebral infarctions. In group 2 IWT was only positive in 1 patient (6.3%). The location of cerebral lesion and the threshold volume at mechanoreceptor-induced involuntary detrusor contraction appeared to influence the results of the IWT.

Pure silicate fragment in a recurrent stone former of calcium oxalate.

Long-time oral intake of magnesium-silicate-containing drugs is thought to be a causative factor inducing silicate urolithiasis. Besides, magnesium seems to play an anti-urolithogenic part in the formation of calcium oxalate stones. We report a recurrent calcium oxalate former who had been treated with magnesium aluminometasilicate antacid for gastric ulcer for approximately 17 years. One of the fragments found during extracorporeal shock wave lithotripsy was identified as 100% silicate. Deposition of silica was also found on other fragments. A large dose of magnesium (given in a part of the drug) might have little influence on the formation of calcium oxalate.

Correlation between morphometric differences and norepinephrine content in benign prostatic hyperplasia.

Tissue norepinephrine (NE) content and quantitative morphometric analysis of benign prostatic hyperplasia (BPH) were evaluated in 30 patients with symptomatic BPH. BPH specimens were obtained by transurethral resection, and NE content was evaluated by high-performance liquid chromatography. The proportions of smooth muscle fibrous, and glandular elements were determined by the light microscopic stereological method. Norepinephrine content of the prostate correlated well with the proportion of smooth muscle component (r = 0.749, P < 0.0001). The percentage of fibrous tissue element was positively correlated with prostate size (r = 0.459, P = 0.0099). Norepinephrine content and histological components did not correlate with subjective symptom score. Morphometrical findings and NE content did not correlate with uroflowmetry parameters and postvoid residual urine rate. In conclusion, NE content of the prostate was probably determined by the amount of smooth muscle element in BPH tissue. The fibrous tissue element was increased in large hyperplastic tissue. The severity of BPH could not be explained by differences in histological composition alone.

Relative and total volume of histological components in benign prostatic hyperplasia: relationships between histological components and clinical findings.

To investigate the correlation between histological differences and clinical findings in patients with benign prostatic hyperplasia (BPH), quantitative morphometrical analysis on prostate was performed on 26 specimens obtained by TURP. The relative and total volume of each histological component were obtained. In this series, the proportion of fibrous tissue element showed a weak positive correlation with prostatic volume (r = 0.499, P = 0.0087). The relative volume of the smooth muscle component showed an inverse correlation with prostatic volume (r = -0.488, P = 0.0105). Patient age and symptom score showed no relationship with the relative volume of histological components. The total volume of all histological components increased with prostatic volume. However, an increase in the fibrous tissue element appeared to be predominant in prostatic enlargement (r = 0.970, P < 0.0001). Although the total volume of all histological components increased with prostatic size, fibrous tissue appeared to play a major role in prostatic enlargement in BPH.

Norepinephrine contents of human prostatic hyperplasia: differences between pathological subtypes.

Tissue norepinephrine content in benign prostatic hyperplasia (BPH) was evaluated to detect possible histological differences in BPH subtypes and to investigate the correlation between norepinephrine levels and age, prostatic weight and clinical symptom score, respectively. Specimens were obtained from 28 patients who underwent transurethral resection of the prostate. Pathologically, 18 out of 28 specimens were classified as fibromyoadenomatous hyperplasia and the remaining 10 as fibromuscular type. Norepinephrine content in the fibromyoadenomatous type was 133.1 +/- 23.1 ng/g, whereas it was 340.3 +/- 60.5 ng/g in the fibromuscular type. Norepinephrine level in the former group was significantly lower than that in the latter group (p < 0.001). In both groups, there was no correlation between norepinephrine content, age and clinical symptom score, while the norepinephrine content had a reverse correlation with prostatic weight only in the former group (p < 0.05). In conclusion, norepinephrine levels were dependent upon histological differences, especially upon the amount of smooth muscle elements, in the evaluated specimen. Severity of prostatism and patients' age showed no correlation with tissue norepinephrine content.

Role of symptom scoring and uroflowmetry in patients with diabetic cystopathy.

Forty-two patients with diabetes mellitus were evaluated by uroflowmetry and the AUA Symptom Index. After urodynamic and physiological investigations 12 patients were diagnosed as having diabetic cystopathy, while 22 patients as being urodynamically normal. In patients with diabetic cystopathy, most of the parameters in uroflowmetry were significantly different from those of normal subjects. Subjective symptom assessment revealed increased total obstructive scoring (p < 0.05). However, irritative symptom indexes in patients with diabetic cystopathy were not different from those of the normal subjects. Although uroflowmetry and the AUA Symptom Index are not specific to identify detrusor impairment, these methods appeared to be useful to decide upon further urodynamic investigations.

Comparison of the nephrotoxicity between ioversol and iohexol.

To evaluate the renal toxicity of intravenous administration of ioversol and iohexol, the urinary N-acetyl-beta-glucosaminidase (NAG) to creatinine ratio was measured before and after drip infusion pyelography. Thirty patients received intravenous ioversol and 29 patients received iohexol. The urinary NAG to creatinine ratio tended to increase after respective contrast media: 5% increase in the ioversol-treated, and 8% in the iohexol-treated patients. There were no significant differences in these values; the nephrotoxicity of ioversol appeared to be just as insignificant as that of iohexol.

Electrical pelvic floor stimulation: a possible alternative treatment for reflex urinary incontinence in patients with spinal cord injury.

The present study demonstrates the clinical experience of pelvic floor stimulation using percutaneous implantable electrodes and implantable electrical stimulator for the treatment of reflex urinary incontinence in patients with spinal cord injury. Pelvic floor stimulation was carried out on six paraplegic patients who had urinary incontinence from an overactive bladder. After the percutaneous implantation of a pair of electrodes, chronic stimulation was carried out by employing an implanted receiver or an external pulse regulator. Within 4 to 16 weeks of electrical stimulation urinary incontinence was improved in four of the six patients. In two of these six patients, incontinence was completely abolished subjectively. Urodynamic investigations demonstrated an increased volume at the first unstable contraction (P < 0.01) in all of the patients. Inhibition of detrusor overactivity was obtained from this procedure. The stimulation effect appeared to be constant during chronic stimulation. This new procedure probably provides a stable and reliable stimulation effect for long term treatment, and may be an alternative treatment for previous external electrical pelvic floor stimulation.

Pathological changes of traumatic dislocated testis.

We present a 17-year-old patient with a right testicular dislocation. In this case, dislocated testis had been considered as hematoma and therefore remained untreated for 4 months. The patient underwent right orchiopexy 4 months after the injury. Intraoperative testicular biopsy revealed impaired spermatogenesis and existence of many alternative Sertoli cells, but no atrophy of seminiferous tubules. Six months after the operation, semen analysis of the patient revealed the deformity of most sperms. Eight months after the operation, rebiopsy of the right testis disclosed slight improvement in the spermatogenesis of the testis. Damage of the testis appears to be not severe in dislocation and may be recovered by repositioning.

Correlations between parameters in uroflowmetry and histological compositions in patients with benign prostatic hyperplasia.

The objective of this study was to determine whether area densities of histological elements within prostatic adenoma affect objective and subjective parameters of patients with prostatic hyperplasia. Modified uroflowmetry parameters including Qmax/T100, TQmax/T100 and voiding efficiency were also measured. The area densities of histological elements did not affect uroflowmetry parameters. In addition, subjective symptom score was not correlated with morphometric findings. Our findings suggest that the degree of infravesical obstruction as measured by uroflowmetry parameters and symptom scores cannot be explained by the tissue compositions of adenomas alone.

Age-related changes of histological composition in established benign prostatic hyperplasia.

To clarify the effect of aging on the histological composition of established benign prostatic hyperplasia (BPH), quantitative morphometric analyses were performed. Specimens were obtained from 23 patients who underwent transurethral resection of the prostate due to symptomatic BPH. Smooth muscle cells, fibrous tissue, glandular cells, glandular lumen and vascular tissue were quantified. Stromal and glandular components of established BPH did not correlate with age, whereas the proportion of vascular tissue was inversely correlated with age (r = -0.572, p = 0.0046). In the present series, the main characteristic of BPH tissue of elderly men appeared to be decreased vascularity.

Dicentric Y chromosome without evidence of mosaicism in an azoospermic male.

A phenotypically male of dicentric Y chromosome with no evidence of sex chromosomal mosaicism is reported. Staining of fluorescence in situ hybridization using Y chromosome centromere-specific DNA probe showed the presence of two bright fluorescent spots in Y chromosome. Testicular histology revealed maturation arrest at the stage of primary spermatocyte. It is suggested that structural anomaly of the Y chromosome is related to the disturbance of spermatogenesis.

Skene's duct cyst in adult women: report of two cases.

Skene's duct cysts in the adult are uncommon lesions. Complete urologic examinations are necessary because these lesions simulate other clinically important lesions. Herein we report two cases of Skene's duct cyst in adult women. In these cases we could make differential diagnosis by physical, radiological and endoscopic examinations.

Clinical and retrospective evaluation of Eviprostat: a non-hormonal and non-neuropharmacological agent for benign prostatic hyperplasia.

Eviprostat has been used as a non-hormonal and non-neuropharmacological treatment for benign prostatic hyperplasia (BPH) in Japan. We evaluated the clinical efficacy of Eviprostat in patients with symptomatic BPH and the anti-inflammatory effect of this drug was investigated by retrospective evaluation of TUR specimens. Clinically, Eviprostat subjectively relieved obstructive symptoms of BPH. Objective improvements were also demonstrated by ultrasonographic evaluation of prostatic volume and urinary flow rates. Histologically, preoperative administration of Eviprostat improved the degree of prostatic inflammation. This drug probably can have a place in the treatment of patients with mild or moderate symptoms of outflow obstruction.
