Phase I study of etoposide with SDZ PSC 833 as a modulator of multidrug resistance in patients with cancer.

PURPOSE: To determine the maximum-tolerated dose (MTD) and toxicity of PSC 833 infusion administered with etoposide for 5 days in patients with cancer, and to determine the effect of PSC 833 on etoposide pharmacokinetics. PATIENTS AND METHODS: Thirty-five patients were entered onto the study, one of whom was ineligible. Etoposide was delivered from day 1 as a 2-hour infusion over 5 consecutive days at a dose of 75 to 100 mg/m2/d. PSC 833 was administered from day 2 as a 2-hour loading dose and as a 5-day continuous infusion. Doses were escalated from 1 to 2 mg/kg (loading dose) and 1 to 15 mg/kg/d (continuous infusion). RESULTS: Thirty-four patients were treated with 53 cycles of PSC 833 and etoposide. Steady-state blood PSC 833 levels more than 1,000 ng/mL were achieved in all patients treated at PSC 833 doses > or = 6.6 mg/kg/d by continuous infusion. Myelosuppression was the most common toxicity. The major dose-related toxicity of PSC 833 was reversible hyperbilirubinemia, which occurred in 83% of cycles. The dose-limiting toxicity of PSC 833 was severe ataxia, which occurred in two of nine patients treated at 12 mg/kg/d and in both of the single patients treated at 13.5 and 15 mg/kg/d. PSC 833 concentrations more than 2,000 ng/mL resulted in an increase in etoposide area under the curve (AUC) of 89%, a decrease in etoposide clearance (Cl) of 45%, a decrease in volume of steady-state distribution (Vss) of 41%, and an insignificant increase in alpha half-life (t 1/2 alpha) and significant increase of beta half-life (t 1/2 beta) of 19% and 77%, respectively. CONCLUSION: PSC 833 can be administered in combination with etoposide with acceptable toxicity. The recommended continuous infusion dose of PSC 833 for this schedule is 10 mg/kg/d over 5 days. PSC 833 results in an increase in etoposide exposure and etoposide doses should be reduced in patients receiving PSC 833.

Cytochrome oxidase genes from Thermus thermophilus. Nucleotide sequence of the fused gene and analysis of the deduced primary structures for subunits I and III of cytochrome caa3.

Cytochrome caa3, a cytochrome c oxidase from Thermus thermophilus, has been purified and extensively characterized as a two-subunit enzyme containing the metal centers characteristic of cytochrome c oxidases (cytochromes a and a3; copper centers CuA and CuB) and an additional cytochrome c (Fee, J. A., Kuila, D., Mather, M. W., and Yoshida, T. (1986) Biochim. Biophys. Acta 853, 153-185). We have now cloned and sequenced the genes encoding the subunits of this enzyme. The smaller subunit consists of a typical oxidase subunit II sequence fused to a cytochrome c domain (Mather, M. W., Springer, P., and Fee, J. A. (1991) J. Biol. Chem. 266, 5025-5035). The larger subunit, the A-protein, is encoded by a fusion gene lying immediately downstream of the subunit IIc gene. The 5' portion of this gene encodes an oxidase subunit I homolog, whereas the 3' portion is homologous to oxidase subunits III. The A-protein from the purified enzyme appears too small from SDS-polyacrylamide gel electrophoresis and quantitative amino acid analyses to be a complete subunit I/III fusion, but it is currently not known if proteolytic processing occurs. Analyses of the sequences of oxidase subunits are presented which clearly identify T. thermophilus cytochrome caa3 as a bona fide member of the greater family of heme- and copper-requiring oxidases. As one consequence, it is confirmed that the set of invariant histidine residues (potential ligands of the metal centers) in cytochrome c oxidase subunits I and II is reduced to 8. Possible topological and helix packing models are developed based on considerations of homology, hydropathy, and variability.

[Experimental research on temporomandibular joint function in dysgnathic patients].

In an experimental study we examined the function of the TMJ of patients with a considerable aberration from ideal dynamic occlusion and with obvious symptoms of frontal dysgnathia. Using a measuring seat for the simultaneous recording of opening and closing movements without occlusal interference and the sounds produced by the TMJ, we could not find any significantly impaired TMJ function compared with an orthognathic control group of similar age.

Studies on cytochrome-c oxidase, XIV. The amino-acid sequence of subunit I--proteinchemical methods for the analysis of a large hydrophobic membrane protein.

The amino-acid sequence of bovine heart cytochrome-c oxidase subunit I, previously deduced from mtDNA was corroborated by proteinchemical methods. The protein consists of 514 amino acids, the Mr is 57,060 including the N-terminal formyl group, which is positively identified. The study describes methods for the purification of the hydrophobic polypeptide by BioGel-chromatography in 3% SDS and/or HPLC and the sequence analysis via complete peptide maps obtained either by chymotryptic or cyanogenbromide cleavage in the presence of residual amounts of SDS. The methods may be used either for a stand alone sequencing of large integral membrane proteins or for obtaining probes to find the gene and provide the necessary complement for DNA sequencing. The results present the only protein-derived evidence for a family of about 20 DNA-deduced sequences of the catalytic subunit of cytochrome oxidases from bacteria to man.

[Frequency and localization of facets in adults with correct dental arches and malocclusion in anterior teeth region regarding periodontal reaction and occlusal guidance].

We investigated 322 patients, 20 and 40 old to find connections between the occurrence and localisation of facets, malocclusion and dynamic occlusion and estimated special aspects of periodontal reactions. The symptoms "anterior crowding", "increased overbite and overjet" didn't effect an increasing of frequency of facets in comparison with eugnathic denture. We found, that the facets decreased in lateral segments if the overbite became deeper and on the other hand number of facets increased with advancing years. The localisation of facets and occlusal guidance of dynamic occlusion coincided only in one third of investigated cases. Balancing and hyperbalancing teeth showed frequent facets. There was a direct relationship between increasing frequency and intensity of gingival retraction and the level of the abrasion of the hard tooth substance.

Evidence for cytochrome oxidase subunit I and a cytochrome c--subunit II fused protein in the cytochrome 'c1aa3' of Thermus thermophilus. How old is cytochrome oxidase?

The terminal cytochrome c1aa3 of the respiratory chain of Thermus thermophilus has been isolated and purified to homogeneity by a novel procedure. The two subunit proteins (55 and 33 kDa) have been characterized chemically. Computer searches with partial amino acid sequences obtained from both subunits show that the larger subunit belongs to the cytochrome oxidase subunit I protein family while the smaller covalently heme-binding subunit is not a cytochrome c1 but appears to be a fused protein between cytochrome c and cytochrome oxidase subunit II. With respect to the 16-S rRNA-derived phylogeny of procaryotes, the results show that the genetic information for an O2-reacting cytochrome oxidase (EC 1.9.3.1) existed already in early eubacteria.

[Metrical studies of the soft-tissue behavior in the anterior dental arch in correct and prognathic anterior occlusion].

By using an intraoral electronic pressure transducer we estimated the pressures exerted by lips and tongue on the central incisors of both jaws. Our subjects were children with normal occlusion and class-III malocclusion. The forces were considered in action (swallowing, speech) and rest. We found that the pressure depends on the activity and where the measurements are taken. During the actions in general tongue pressure is higher than lip strength. But in resting the pressure on the vestibular soft tissues is higher. Forces during rest have an important influence on the total pressure, which takes duration and frequency of action as well as pressure during action and rest. The relationship between soft tissues strength and morphology was found in the lingual activities of subjects with class-III malocclusion.

[Measurements of the soft tissues in patients with cleft lip and cleft palate].

A comparative study was made on the pressure of the oral soft tissues during sequences of physiological functions and at rest in eugnathic subjects and patients with cleft lips and palates. The data obtained by means of electronic measuring instruments revealed marked differences between the two groups.

[Assessment of the success of orthodontic treatment from the viewpoint of general dentistry].

Using internationally accepted indices, the authors studied the caries and gingivities frequency as well as the oral hygiene in 112 successfully treated orthodontic patients 35 months or thereabout after termination of the treatment and retention. The results obtaines are compared with those from an adequate control group and statistically evaluated. The general dental condition and the degree of oral rehabilitation of the orthodontic patients was better than those of the subjects in the control group.
