Celiac disease and keratoconjunctivitis. Occurrence with thrombocytopenic purpura.

A patient had a rare association of celiac disease, keratoconjunctivitis sicca, and autoimmune thrombocytopenia. Both the malabsorption and the thrombocytopenia responded to prednisone therapy. The development of three different autoimmune disorders in this patient may be related to the presence of HLA-B8 and HLA-Dw3, the prevalence of which is known to be increased in celiac disease and keratoconjunctivitis sicca and that of HLA-B8 in autoimmune thrombocytopenic purpura. Despite the fact that thrombocytopenia is rarely encountered in celiac disease, it is suggested that an autoimmune mechanism should be excluded in patients with this combination.

Primary nonspecific ileal ulceration as a cause of massive rectal bleeding.

A case of primary ulceration of the ileum in the absence of a Meckel's diverticulum or ectopic gastric tissue is described. Although this condition is infrequently observed, it should be considered as a potential cause of massive rectal bleeding, iron deficiency anemia of unknown cause, perforation, or partial small bowel obstruction. The diagnosis is normally made at surgery. Segmental resection with end-to-end anastomosis is the treatment of choice. Recurrence following surgery is rare.

The scintigraphic evaluation of hip pain in children.

Thirty-eight patients (28 boys and 10 girls) with hip pain were referred for bone scans. In 7 of 8 with Legg-Calvé-Perthes disease, there was decreased uptake of the radiopharmaceutical in the proximal femoral epiphysis; the other had radiographic evidence of joint destruction and showed increased uptake. Three patients with abnormal scans had normal x-rays with Gaucher's disease, toxic synovitis, and Legg-Calvé-Perthes disease, respectively. One patient with avascular necrosis associated with steroid therapy had increased uptake, possibly due to revascularization. Those with toxic synovitis had either normal bone scans for showed a slight generalized increase around the hip joint. It is speculated that the latter may represent those who later present with Legg-Calvé-Perthes. The bone scan is useful for the early detection of Legg-Calvé-Perthes disease in patients with hip pain.

The uptake of gallium-67-citrate by the parotid glands in a uremic patient.

A 9-year-old uremic girl who showed intense parotid uptake when scanned with gallium-67-citrate is discussed. None of the more frequently described predisposing factors was present. A review of 22 other children scanned for suspected inflammatory disease revealed minimal parotid and lacrimal activity in three, with lacrimal activity only in another four. It is suggested that a metabolic cause be considered when there is abnormal parotid activity with Ga-67 scintigraphy.

The lymphatic spread of osteosarcoma shown by Tc-99m-MDP scintigraphy.

Osteosarcoma usually spreads via the blood stream, resulting in pulmonary and skeletal metastases. The value of bone imaging in the management of these patients is widely accepted. Less commonly, the disease spreads via the lymphatics. Such a case is reported to show that bone imaging will detect progressive involvement of the lymph nodes.

Simultaneous 99mTc-P-butyl-IDA and 131I-rose bengal scintigraphy in neonatal jaundice.

Eight neonates with jaundice were studied simultaneously with 99mTc-p-butyl-IDA and 131I-rose bengal. Due to physical decay, 99mTc-p-butyl-IDA failed to demonstrate delayed excretion through the patent extrahepatic biliary tract in 3 of 5 patients with concomitant hepatitis; 131I-rose bengal showed small-bowel activity in all 5. Neither agent demonstrated small-bowel activity in 3 neonates with extrahepatic biliary atresia. Based on this clinical trial, 131I-rose bengal remains the radiopharmaceutical of choice for distinguishing between hepatitis and biliary atresia in these patients.

Chronic granulocytic leukemia in pregnancy. A case report and review of the literature.

Chronic granulocytic leukemia (CGL) was diagnosed in the second trimester of pregnancy in a mother of four, who decided to terminate the pregnancy immediately. According to the literature this unusual combination is not a definite indication for abortion since CGL does not adversely affect the course of the pregnancy nor the pregnancy alter the course of the disease. Prudent treatment with busulfan or splenic irradiation, when indicated, apparently does not endanger the development of the fetus.

Krypton-81m: a better radiopharmaceutical for assessment of regional lung function in children.

Krypton-81m, an inert, radioactive gas with a physical half-life of 13 seconds was used to evaluate regional lung ventilation in 25 pediatric patients. The results suggest that Kr-81m has several advantages over previous methods employing xenon-133. These include: more favorable physical characteristics for imaging with present scintillation cameras; ease of performance of the examinations; improved resolution and multiple views with better detection and localization of abnormalities; and a much lower dose to the patient.

An improved radionuclide method for the diagnosis of gastroesophageal reflux and aspiration in children (milk scan).

A radionuclide study using technetium-99m-labelled milk feeding is described for the detection of gastroesophageal reflux and aspiration in children. A comparison of findings in 39 patients referred for both radiographic and radionuclide studies showed that barium studies were positive in 25.6% and radionuclide studies in 59% of cases, reflecting the sensitivity of the radionuclide technique. This technique is also physiological and allows prolonged patient monitoring. It is simple to perform and should prove useful in the evaluation of patients suspected of having gastroesophageal reflux and aspiration.

Localization of bleeding small intestinal lesions using scanning techniques.

Gastrointestinal bleeding remains a major diagnostic problem, and the commonly employed techniques of contrast radiography, endoscopy, and arteriography may not successfully localize the site and/or define the cause of gastrointestinal hemorrhage. Technetium-99m pertechnetate scanning has been useful in identifying Meckel's diverticula, and modifications of this technique may successfully identify highly vascular lesions responsible for gastrointestinal bleeding. A patient with recurrent hemorrhage from a leiomyosarcoma of the ileum is described. The lesion was identified with flow studies and immediate static imaging after injection of technetium-99m pertechnetate. In addition, the lesion was demonstrated by scanning using in vivo technetium-99m pertechnetate labelled autologous erythrocytes. The potential value of these scanning techniques as noninvasive tools for the localization and identification of lesions responsible for gastrointestinal bleeding is discussed.

Adrenal hemorrhage in the newborn: scintigraphic diagnosis.

Four neonates with abdominal masses following adrenal hemorrhages were investigated by renal scintigraphy. Characteristic photon-deficient lesions were noted in the suprarenal areas during the phase of whole-body radionuclide distribution.

Renal uptake of 131I-rose bengal in biliary atresia. A case report.

Renal uptake of radio-iodinated rose bengal was studied in a 5-month-old infant, who was later found, at operation, to have biliary atresia. Prominent renal uptake, which could easily be mistaken for radioactivity in the bowel, was noted. While this pitfall in diagnosis has previously been described, it is thought worth while to stress this possible cause of misinterpretation.

Lung perfusion patterns after bidirectional cavopulmonary anastomosis (Hemi-Fontan procedure).

Perfusion lung scans were performed immediately prior to Fontan procedure in 45 patients with a functional single ventricle who underwent the hemi-Fontan procedure as an intermediate stage between the initial palliative shunt procedure and the final complete Fontan anastomosis. Symmetric pulmonary blood flow (PBF) distribution was found in 27% of patients, whereas moderately to severely abnormal PBF distribution was found in 35% of patients. The achieved systemic aortic saturation following the hemi-Fontan procedure was not affected by these abnormalities.
