Computerized cephalometric orthognathic surgical simulation, prediction and postoperative evaluation of precision.

A new computerized, cephalometric, orthognathic surgical program (TIOPS) has been evaluated in surgical simulation, prediction and postoperative assessment of precision. Records of 10 consecutive patients admitted for orthognathic surgical treatment were analysed and prediction plans produced by computerized surgical simulation. Predicted and postoperative positions of maxilla and mandible were compared with linear and angular measurements. No statistically significant differences between predicted and postoperative positions could be demonstrated (p greater than 0.05).

Does metronidazole prevent alveolitis sicca dolorosa? A double-blind, placebo-controlled clinical study.

The effect of a single preoperative dose of metronidazole in the prevention of alveolitis sicca dolorosa (ASD) after surgical removal of one impacted, non-infected mandibular third molar was investigated. A patient sample of 270 were given either 1000 mg of metronidazole or placebo at least 30 min before surgery. The preoperative recordings included gender, age, tooth to be removed, experience of surgeon, time of test medication, and duration of surgery. No difference was found between the metronidazole and placebo groups in the occurrence of ASD. The duration of surgery and the experience of the operating surgeons had no effect on the occurrence of ASD. The present study failed to demonstrate any preventive effect of a single dose of metronidazole on the development of ASD.

Influence of skin graft pathology on residual ridge reduction after mandibular vestibuloplasty. A 5-year clinical and radiological follow-up study.

The present study is a 5-year clinical and radiographic follow-up of 51 patients who had undergone a combined vestibuloplasty with a split skin graft and lowering of the floor of the mouth. Our purpose was to examine the effect of clinically evident graft pathology on the residual ridge reduction. The clinical records of the graft condition were classified into: healthy graft, partial loss of keratinization, and total loss of keratinization. The residual ridge reduction was monitored by ridge height measurements at different locations, and by the symphyseal and mandibular body areas. Patients with clinical signs of graft pathology (reddening, loss of keratinization) suffered a significantly more severe residual ridge reduction than those with a healthy skin graft (p less than 0.01). Subsequent findings including smear tests showing Candida albicans hyphae, and improvement of graft condition after antimycotic therapy, indicated that the graft pathology observed is a candidiasis.

Preprosthetic vestibular sulcus extension by the operation of Edlan and Mejchar. A 2-year follow-up study-I.

Nineteen patients suffering from denture retention problems and/or pain on function due to atrophy of the mandibular alveolar ridge were operated on by the method of EDLAN & MEJCHAR. All patients felt an improvement of denture retention. Three out of seven still had pain on function after the operation. An average of 80% of the surgically created sulcus extension remained after 2 years.

Profile changes of bone and soft tissues following vestibular sulcus extension by the operation of Edlan and Mejchar. A 2-year follow-up study-II.

The progression of alveolar resorption and the alteration of the soft tissue profile of the chin were studied in 15 patients operated on by the vestibular sulcus extension technique of EDLAN & MEJCHAR. Lateral cephalograms and tracings exhibited an apparently accelerated bone resorption of the alveolar crest. Furthermore, a significant reduction of lower lip height and increase of mental soft tissue protrusion have been recorded.

Aneurysmal bone cyst--simple bone cyst, two aspects of the same pathologic entity?

An unusual case of aneurysmal bone cyst recurring after operation as a simple bone cyst is reported. Different theories on etiology and pathogenesis of aneurysmal bone cyst, simple bone cyst, and central giant cell granuloma of the jaws are resumed and similarities demonstrated. It is suggested that these three lesions have a common dysvascular etiology and that local environmental factors within the bone may differentiate the pathogenesis.

Diagnosis of sarcoidosis from oral manifestation.

The diagnosis of sarcoidosis may be a matter of complexity. Two cases of sarcoidosis in which oral manifestations were helpful in the diagnosis are presented. Similar cases from the literature are reviewed and discussed, and it seems that sarcoid involvement of clinically normal-appearing oral mucosa occurs with some frequency.

[Diagnostic and therapeutic aspects of myeloma from the point of view of the dentist and the oral surgeon].

Data on 10 patients with myelomatosis from three different maxillo-facial surgery departments are compiled and case histories, clinical courses, radiographs and therapies are studied. The data are compared with 51 cases reported in the literature. The main emphasis of the studies was on: 1. Diagnosis on the basis of the first symptoms in the maxillary region. 2. Prophylaxis and therapy of oral complications.

Tattoo marking for registration of relapse after oral vestibuloplasty.

A simple method for registering extension and relapse after vestibuloplasty based upon tattoo marking has been developed. Tattoo marking with autoclaved India ink was tested on 10 rats and seems to be harmless. This method is considered an alternative to other numerical expressions of extension and relapse after vestibuloplasty, such as radiocephalometrics and analysis of standardized impressions and castings.
