[The interrelation of vascular, neurological and functional disorders in the hand of rheumatoid arthritis patients].

The hands of 45 RA patients were studied for vascular, neurological and functional characteristics. The patients underwent functional tests, dopplerography (to study regional arterial flow) and electroneuromyography (to study the peripheral nerves). Induration and defective elasticity of the vascular wall in the radial and ulnar arteries were disclosed in 74% of the cases. Digital arteries suffered occlusive lesion of degree III-IV in 46% of the examinees. One-third of the patients showed signs of polyneuropathy. Among functional defects, the upper grasp incompetence appeared the first. It is stated that parallel affections of the vascular, neurological and functional origin contribute to deficient functional performance of the hand in RA patients.

[Lung and pleural involvement in lymphosarcoma].

The pulmonary-pleural syndrome is often encountered in practice as a manifestation of lymphosarcoma that may be suspected on the basis of "atypical" pneumonia characterized by relapses and tendencies towards bilateral polysegmental lesions with the involvement into the process of the intrathoracic lymph nodes of complicated by hemorrhagic pleurisy, with the presence of pronounced alterations on the part of the blood in the form of anemia and lymphocytosis. Biopsy of an enlarged peripheral lymph node is the only reliable method of verification of lymphosarcoma diagnosis.

[Value of echocardiography in the diagnosis of cardiac lesions in systemic scleroderma].

A total of 20 patients with systemic scleroderma (SSD) were examined with the use of a single-measure and sectoral echocardiography. Despite the absence of clinical signs of circulatory insufficiency a disorder of intracardiac hemodynamics with the indices of the central hemodynamics being normal were revealed in the examined patients. Latent exudative pericarditis producing no substantial effect on the hemodynamics was diagnosed in four patients. Fibrosis of the valvular apparatus was observed in three patients. It has been found advisable to include echocardiography into complex examination of patients with SSD.
