Monophasic action potentials of the right atrium during atrial fibrillation in man.

In 28 patients with established atrial fibrillation [AF], right atrial monophasic action potentials [MAP] were recorded before DC shock. A close correlation was found between the atrial rate and MAP duration of the fibrillatory waves [FW]. The duration of MAPs ranged between 1 and 6 mV. The atrial rate ranged between 311 and 578 per minute, the highest rates were found during lone AF. In two patients in whom cardioversion failed, a prolongation of right atrial MAP duration of the FW was noted. The efficacy of DC shock and the maintenance of sinus rhythm after cardioversion was greater in patients with AF having a slower rate and a longer MAP.

Complex atrial arrhythmias studied by suction electrode technique.

Two elderly patients with coronary heart disease, chronic pulmonary disease, and complex atrial arrhythmias are presented. The suction electrode technique for right atrium monophasic action potential recording was used. This recording allows better analysis of the atrial arrhythmias and estimates the stability of focal activity.

A simple intracavitary bedside method for estimation of the P-R interval.

A simple bedside intracavitary electrocardiographic technique is described. Two electrode catheters are used: one tripolar catheter positioned across the tricuspid valve in order to record the His electrogram, and a hexapolar catheter positioned through the superior caval vein near the sinus node with the proximal electrode and 6 cm lower in the right atrium with the tip electrode. This technique allows a better analysis of intraatrial conduction, widening the interpretation of the P-A interval obtained by the His bundle electrogram.
