[Correlation of symptoms and clinical findings in polyneuropathy].

A scoring system for neuropathic symptoms (neuropathic symptom score) and signs (neurologic disability score) has been applied on 30 patients with polyneuropathy. Vibration sense thresholds were also measured by vibrametry. A statistically significant association between the neuropathic symptom score and the neurologic disability score was demonstrated. Disability scores expressing sensory items were significantly associated with vibration threshold on the index finger and even stronger with that on the big toe, which also correlated well with the sum of the patients' neuropathy deficits (total neurologic disability score). We recommend that registration of neurologic disability score in particular and if possible also vibrametry (and other quantitative sensory tests) are performed in the primary evaluation of patients with polyneuropathy, and especially in subsequent examinations to clinically monitor the patients.

[Central nervous symptoms and findings in patients with systemic lupus erythematosus].

Systemic lupus erythematosus is a disease characterized by multiple autoimmune phenomena, and a broad clinical spectrum. Involvement of the central nervous system is common, and in the majority of patients occurs mainly as an organic brain syndrome or as migraineous headache. Cerebral atrophy as judged by CT scan is common. Cerebral infarction occurs in a minority of patients, mainly those with high disease activity and a high titer of anti-phospholipid antibodies. Systemic lupus erythematosus should be considered as a possible diagnosis in patients with certain central nervous system aberrations, especially young females.

[Neuromuscular symptoms and findings in systemic lupus erythematosus].

Systemic lupus erythematosus (SLE) gives rise to a variety of autoimmune and clinical phenomena in a multisystem manner. Both the central and peripheral nervous systems are involved as well as skeletal muscle. Based on our work with SLE patients, we describe the different categories of peripheral neuropathy and myopathy. We suggest that the disease be considered as a possible diagnosis, especially when evaluating women with symptoms and signs of neuromuscular abnormalities.

[A neurologic day care clinic. Presentation of the concept, and the first 271 patients].

Most of the patients remitted to this service are former inpatients who need more comprehensive examinations than usually performed in an ordinary outpatient setting. They attend for one or more days during office hours, spending the night at home or in a hotel. Investigative procedures are ordered in advance, and the patients receive written information about the various tests beforehand. Of the first 271 patients, 206 attended for one day, 63 for two days and two for three days. The examinations comprised 81 by lumbar myeloradiculography, 59 EMG, 56 EEG, 46 cerebral CT scan, 33 SER (somatosensory evoked response), 32 BAER (brainstem auditory evoked response), and 16 VER (visual evoked response). A spinal fluid tap was performed in 49 patients, muscle biopsy in five patients, and 51 were examined by doppler of the precerebral arteries. The fact that the patients attend for one or more days instead of one hour is also of great benefit for teaching and research purposes.

Multivariate analyses of factors associated with unemployment in people with multiple sclerosis.

Variables at onset of multiple sclerosis (MS) as predictors of time to unemployment have been studied using multivariate survival (time-to-response) analyses. The study consisted of 79 patients with MS diagnosed in the two most northern counties of Norway between 1974-82. Diagnostic category, age at onset, sex, county, symptom groups, course of disease and occupation groups were identified as possible prognostic variables. Forty one patients (51%) were unemployed at last follow up. None of the 15 patients with probable or possible MS were unemployed due to MS. The multivariate analyses selected the nonremittent course or heavy physical work and age over 30 at onset as high risk factors associated with early unemployment.

Early mitochondrial changes in chronic progressive ocular myopathy.

Two sisters with chronic progressive external ophthalmoplegia (CPEO) and their in all 7 healthy children were investigated. Both ophthalmoplegic patients had histopathological changes typical of mitochondrial myopathy. The same type of muscular pathology was also found among the healthy children. The most common muscular changes were subsarcolemmal accumulation of pathological mitochondria, including vacuoles, abnormal cristae and sometimes also inclusion bodies. Biochemical studies showed partial complex III deficiency, with low succinate-cytochrome c reductase activity in 1 of the ophthalmoplegic patients. These findings suggest that CPEO is a slowly progressive muscle disease, starting early in life. The widespread occurrence among the children may indicate maternal inheritance.

[Dementia evaluated by means of a mini-mental status examination. Clinical neurologic patient material].

The mini-mental state examination (MMSE) was administered to a neurological patient material with various disorders. The test was found useful for detecting cognitive impairment. The test did not differentiate reliably between different causes of cognitive impairment. To detect dementia a cut-off score of 25 or less yielded 100% sensitivity and 97% specificity. The validity of the DSM-III-diagnostic criteria for dementia, primary degenerative dementia and multi-infarct dementia was high. The modified ischemic score (MIS) seemed to be of limited value in differentiating primary degenerative dementia from multi-infarct dementia.

[Parkinson's disease in a population group in northern Norway].

Parkinson's disease has been studied in a community of Northern Norway. The main object of the study was to find the prevalence of the disease. The prevalence of Parkinson's disease was 133 per 100,000 population (men 113, women 134) and 189 adjusted to the total Norwegian population (both sexes). Prevalence increased with age in both sexes, especially after the 60th year. The average age at onset was 63.1 years (men 60.5, women 65.4 years). Tremor was present in 98% of the patients, hypokinesia in 78%, rigidity in 65%, all three in 56% and pain in 35%. The prevalence, sex distribution, and the age at onset is similar to that in other European and North American studies.

Peripheral neuropathy in primary Sjögren's syndrome.

Sjögren's syndrome (dryness of eyes, mouth, and other mucous membranes) may be associated with disease of joints, blood, internal organs, skin, and central and peripheral nervous systems. We reviewed 33 cases of primary Sjögren's syndrome and peripheral neuropathy evaluated by neurologic examinations and EMG at the Mayo Clinic from 1976 to 1988, and studied sural nerve biopsy specimens in 11 of them. Symmetric sensorimotor polyneuropathy occurred most frequently, followed by symmetric sensory neuropathy. Autonomic neuropathy, mononeuropathy, or cranial neuropathy (especially trigeminal neuropathy) was superimposed on generalized neuropathy in approximately one-fourth of patients. The course generally was slowly progressive, except for a few patients who may have improved with prednisone therapy. Although spinal ganglion involvement might have accounted for some of the clinical and neurophysiologic findings, we found evidence that necrotizing vasculitis was involved in fiber degeneration. All nerve biopsies revealed perivascular inflammatory infiltrates and other vessel abnormalities, which were diagnostic in two cases and strongly suggestive of necrotizing vasculitis in six cases. Axonal degeneration predominated over demyelination and sometimes was focal or multifocal. In neuropathy of unknown cause, particularly if it is sensory, autonomic, or involves trigeminal nerve, consider Sjögren's syndrome.

Clinical neurological, electrophysiological, and cerebral CT scan findings in systemic lupus erythematosus.

Thirty SLE patients underwent clinical neurological examination, electrophysiological studies, cerebral computer tomographic (CT) scans, and blood sampling. Nineteen patients (63%) had clinical neuropsychiatric and 10 patients (33%) had clinical neuromuscular manifestations. Migrainous headache affecting 11 patients (37%) was the most prevalent clinical manifestation. Electrophysiological testing revealed abnormal electroencephalography in 10 patients (33%). Abnormal electromyography and nerve conduction velocity were found in 13 (43%) and 7 (24%) patients respectively. Abnormal visual evoked response was detected in 2 patients. Cerebral CT scans displayed cerebral atrophy in 20 patients (71%), while 6 patients (21%) had cerebral infractions. Disease activity assessed by two different tests revealed a higher prevalence of cerebral infarctions, classical migraine, muscular weakness, and pathological electromyography and nerve conduction velocity in the higher disease activity groups. Cerebral infarctions were only found among anti-Ro negative patients, but apart from this, no significant association could be found between coagulopathy, circulating immune complexes, cryoglobulins, routine immunological tests, medication, and any clinical, electrophysiological or cerebral CT pathology.

Optic neuritis in the two northernmost counties of Norway. A study of incidence and the prospect of later development of multiple sclerosis.

A study of the incidence of uncomplicated optic neuritis (ON) in the period 1972-1984 and the subsequent risk of later developing of multiple sclerosis (MS) was performed in the two northernmost counties of Norway. The incidence of ON was stable during the period. Of the 36 ON patients available to follow-up, 42% later developed MS. Mean time from debut of ON to the next symptom of MS was 2.2 years, median 1 year. There was one ethnic Lapp among the ON patients. The number of Lapps in the area is estimated to be about 26,000.

Clinical neuropsychiatric and neuromuscular manifestations in systemic lupus erythematosus.

Thirty patients with SLE were studied retrospectively and subjected to clinical neurological examination. The accumulated neurological manifestations from the beginning of the disease until the time of examination were thus collected. Twenty-five patients (83%) had experienced neuropsychiatric manifestations while 11 patients (37%) had neuromuscular manifestations. The most frequent single symptom was migraine which had occurred in 40% of the patients. This was followed by severe protracted headache in 20%, vertigo in 20%, and psychiatric problems in 17%. Carpal tunnel syndrome and muscular weakness both occurring in 23% of the patients were the most prevalent neuromuscular manifestations, followed by myositis in 10%.

Sleep apnoea and organic solvent exposure.

Fifteen patients referred for the evaluation of possible organic solvent encephalopathy were studied by clinical polysomnography. Seven had more than 30 apnoeas per night and an apnoea index of higher than 5, thus fulfilling the commonly used criteria of the sleep apnoea syndrome. Another group of eight workers exposed to trichlorethane, examined without prior knowledge of their individual symptoms, showed a significantly elevated number of sleep apnoeas compared with nine controls. The results indicate that organic solvent exposure can cause sleep apnoea.

Multiple sclerosis in the two northernmost counties of Norway.

The prevalence of MS in the two northernmost counties of Norway increased from 20.6 per 100,000 in 1973 to 31.5 per 100,000 in 1983, the increase being most marked in women. The average annual incidence seems to have been stable over the past 20-30 years. None of the patients had a pure Lapp background (having a Lapp mother and father).
