Beyond implementation: the role of physicians, health care professionals and consumers in the development and use of food products.

Physicians and consumers play an essential role in determining the ultimate success of food products. Effective communication of product efficacy and safety data to physicians, health care professionals and consumers can be a deciding factor in whether a food product is recommended or consumed. Food products deemed safe and beneficial will be further judged for cost and cost effectiveness. Physician and consumer reaction to newly launched products can be a valuable source of feedback if used to create communication plans and to reevaluate products. Long term monitoring of safety, efficacy, and consumer use patterns in the context of post-marketing surveillance studies is another potential feedback mechanism. The present review focuses on how physicians and consumers are involved in food product development and refinement, how they use the information available to make choices, and what kinds of information must be supplied to allow the public to be fully informed.

A newly recognized cause of wheezing: AIDS-related bronchial leiomyomas.

We present two human immunodeficiency virus-infected children who developed wheezing and radiological evidence of pulmonary air trapping due to intra- and peribronchial leiomyomas. At autopsy, leiomyomas were also found in their spleens, which to our knowledge, has never been reported. The smooth muscle tumors were strongly positive for the Epstein-Barr virus, as demonstrated by in situ hybridization to Epstein-Barr virus-encoded ribonucleic acid, confirming the findings of recent investigators and linking these tumors to the Epstein-Barr virus.

Mesenteric pleomorphic liposarcoma in an adolescent.

We report a case of pleomorphic liposarcoma arising in the root of the mesentery of an adolescent girl. Pleomorphic liposarcoma is an extremely rare tumor in the pediatric age group, and few well-documented cases are found in the literature. To the best of our knowledge, none have been described in the abdomen. The histologic and ultra-structural features of this tumor are described, and the literature concerning pediatric pleomorphic liposarcoma is reviewed.

Postpartum multivessel coronary dissection.

A 29-year-old woman had an acute myocardial infarction 5 days after giving birth. Serial coronary angiography showed multiple progressive coronary artery dissections, which eventually involved both the right and left coronary trees. Persistent cardiogenic shock necessitated emergent orthotopic heart transplantation. Examination of the cardiectomy specimen confirmed the presence of multiple myocardial infarctions, coronary artery dissection, and fibromuscular dysplasia of the coronary arteries. Fibromuscular dysplasia combined with changes in the arterial ground substance and hormonal milieu attributable to pregnancy or parturition are proposed as possible causes of coronary artery dissection in this case.
