[Screening for Salmonella in pregnancy].

National and Regional Health Authorities advise a stool culture in pregnant women before term in order to detect Salmonella carriers, prevent the spread of this microorganism to the newborn and avoid outbreaks of this infection in the nurseries. The Tuscany section of the Italian Association of Obstetricians and Gynecologists decided to test the usefulness of this Salmonella screening. In 7431 pregnant women at the 36th week a stool sample was examined for the presence of Salmonella. The occurrence of diarrhoea in these women was also investigated. The prevalence of Salmonella excretors in our obstetric population was 0.27%. Thirty per cent of the positive women complained of diarrhoea; that means that the risk of being positive in the presence of this symptom is 11.8 times larger. The positive cases were detected essentially in late summer and fall. No nursery outbreak occurred during the period studied. The Salmonella-carrying mother can not take advantage of an effective antimicrobic therapy and a single stool sample allows the detection of only part of the carriers. Therefore screening can not prevent the possibility of transmission during birth. The unfavorable ratio between costs and benefits suggests that stool culture for Salmonella may be useful only in late summer and fall and in symptomatic women. In order to obtain better results in the prevention of infections among newborns the observance of careful hygienic rules in the delivery rooms and in the nurseries is mandatory.

[The treatment of postoperative shivering with nefopam hydrochloride].

After having considered the physiopathological mechanism of shivering, the authors have estimated in a randomised study the effects of the resolution of shivering by nefopam hydrochloride. This drug a dose of 0.20 mg/kg resolved the symptoms in 100% of cases and the placebo hasn't resolved a case so that case it also obtained the same effects. The authors have proved the efficacy of the nefopam hydrochloride in the control of postoperative shivering in the recommended dose.

Two pigmented lesions of the female genital system. Blue nevus of the uterine endocervix and melanin pigmentation of the vagina.

Five cases of benign pigmented lesions in unusual sites are reported. They consist of four blue nevi of the uterine endocervix, respectively the 38th, 39th, 40th and 41st cases according to our review of the literature, and one melanin pigmentation of the vagina, the 12th case according to our review of the literature. The theories about the presence of melanin-containing cells (melanocytes) in unusual sites are reported and the singular finding of melanin pigmentation observed in vaginal epithelium is discussed.

[Contribution to the serologic study of toxoplasmosis in pregnancy].

Toxoplasma antibodies in five women sera during the pregnancy were detected by direct agglutination and immunofluorescent tests both for IgG and IgM. The presence and the changes of the IgG and IgM titers are related to the abortion and/or foetal pathology. In order to prevent such a risk the authors suggest to test all the women before or early in the pregnancy for toxoplasma antibodies.

Synthesis and biological activity of digitoxigenin aminoesters.

A series of 3 beta-esters of digitoxigenin (3 beta-hydroxy-14 beta-hydroxy-5 beta-card-20(22)-enolide) with alpha-aminoacids, were synthesized and tested for inotropic activity on the guinea-pig isolated heart and by slow infusion in the cat in comparison with digitoxigenin, Lanatoside C and Strophantin K. Esterification of the 3 beta-hydroxy group of digitoxigenin with various amino acids led to compounds still retaining inotropic activity with low in vivo potency and short duration of action. The compounds are inactive when administered orally.

Oral clonidine: an effective provocative test of growth hormone release.

The authors tested the efficiency of clonidine as a stimulant for growth hormone (GH) release in a group of 31 undersized children aged from four to fourteen years. The efficacy of clonidine was compared with insulin-induced hypoglycaemia, which has so far been considered the most valid test for somatotropic function measurement. The average GH peak after clonidine was 13.7 +/- 1.9 ng/ml, while after insulin it was only 7.0 +/- 0.9 ng/ml. The number of patients responding to clonidine and insulin reacting with GH values below 5, between 5 and 8, and higher than 8 ng/ml were reported, and the difference between the two sets of values was found to be statistically significant (p less than 0,01) in favour of clonidine.

Synthesis and antibacterial activity of some ureido cephalexin and cefadroxil derivatives.

The antibacterial activity of some ureido, acylureido and carbamoylureido derivatives of cephalexin and cefadroxil, prepared in our laboratories, as compared to parent compounds is reported. Only the 7-[D-alpha-(imidazolidin-2-one-1-ylcarbonylamino)-alpha-phenylacetamido]-3-methyl-3-cephem-4-carboxylic acid (IV d) and the 7-[D-alpha-(imidazolidin-2-one-1-ylcarbonylamino)-alpha-p-hydroxyphenylacetamido]-3-methyl-3-cephem-4-carboxylic acid (V c) showed some antibiotic activity. However we found no activity improvement against Pseudomonas strains (contrary to the results obtained in similar penicillin derivatives) and a lower beta-lactamase resistance.

Synthesis of 6alpha-methyldigitoxigenin 3-acetate.

In order to determine the influence of a 6alpha-methyl group activity, the 6alpha-methyl derivative of digitoxigenin 3-acetate 14 was prepared and pharmacologically tested in comparison with digitoxigenen 3-acetate. The synthesis of 6alpha-methyldigitoxigenin 3-acetate (14) was performed starting from 21-hydroxy-4-pregnene-3,20-dione (1). According to the cardiac activity determined on guinea-pig isolated heart and by slow infusion in the cat, the 6alpha-methyldigitoxigenin 3-acetate (14) is not more active than digitoxigenin 3-acetate.

A new product of rearrangement of penicillin V (S)-sulphoxide.

Penicillin V (S)-sulphoxide (I), treated with methanesulphonic acid in benzene and dimethylacetamide, gives the decarboxylated increment3-cephem compound (IV) and a new product of rearrangement to which the structure of the 4-[N-(1'-carboxy-2'-methylenepropyl)carbamoyl]-2-phenoxymethylthiazole (VIII) is ascribed.
