[Bucillamin induced lung injury in rheumatoid arthritis].

Thirteen cases with rheumatoid arthritis who experienced lung injury during the treatment with bucillamine (Bc), about whom the questionnaires were answered by the physicians and whose X-ray films could be rechecked, were studied. Nine cases out of the 13 showed patchy mottled infiltrates in the bilateral center sparing the periphery, and the other 4 showed diffuse infiltrates. In these 9, serum gamma-globulin level decreased when lung injury appeared. The gamma-globulin level before the start of Bc administration, the level when lung injury appeared, the gamma-globulin decrease, and its ratio to the level before Bc were 1790 +/- 661 mg/dl, 1297 +/- 666 mg/dl, 459 +/- 320 mg/dl and 29.1 +/- 18.0%, respectively. In 5 out of the 9, gamma-globulin level reincreased when they recovered from the injury: 2 out of the 5 showed the reincrease even after steroid therapy. The data were obtained only from 2 out of the 4 with diffuse pattern in X-ray, and the decrease and the decrease ratio were 200 mg/dl (5.8%) in 1, and 49 mg/dl (3.6%) in the other. The characteristics of Bc-induced lung injury might be mottled infiltrates in the center appearing concurrently with serum immunoglobulin decrease.

Expression of nm23-H1 gene product in thyroid, ovary, and breast cancers.

The nm23 gene product is one of several possible mediators of cancer invasion and metastasis. As the amounts of nm23-H1 mRNA and gene product are reduced in metastatic lymph nodes from patients with papillary carcinoma of the thyroid, we examined the expression of nm23 gene product in 115 thyroid cancers, 78 ovarian cancers, 63 breast cancers, and metastatic lymph node tissues by immunohistochemistry. It was found that nm23-H1, but not nm23-H2 gene product, was expressed in primary sites of thyroid, ovarian, and breast cancers, except for medullary and anaplastic carcinomas of the thyroid, but expressed only weakly or poorly in metastatic lymph nodes. Although nm23-H1 gene product expression was lower in anaplastic and medullary carcinomas of the thyroid, there was no significant difference in nm23-H1 gene product expression among histological types of ovarian and breast cancers. Our data indicate that the nm23-H1 gene product may play a role in metastasis in these hormone-producing organs and that other factors may be involved in metastasis of anaplastic and medullary carcinomas of the thyroid.

[Immunoglobulin and LST in RA patients treated with bucillamine].

In 79 RA patients treated with Bucillamine (Bu) we monitored IgG, A, M and total protein concentration x gamma-globulin% (Ig) before and after Bu. All of these four were lowered after Bu in both groups with and without adverse reaction. In the group with adverse reactions the serum level of IgG, A and Ig was significantly lower after Bu treatment than in the group without adverse reactions. The decreases of IgG and IgA were statistically significantly greater in the group with adverse reactions than those in the group without adverse reaction. The serum level of IgM after Bu in the effective group was significantly lower than that in the non-effective group. We also examined lymphocyte stimulation test (LST) in 44 RA patients treated with Bu. In the effective group Bu inhibited lymphocyte proliferative response to PPD more significantly than in the non-effective group. Bu also inhibited lymphocyte proliferative response after stimulation with PPD in the non-effective group doseresponsively. We concluded that the considerable decreases of IgG and IgA might correlate with the adverse reactions of Bu. The decrease of IgM and inhibition of LST with Bu might correlate with the efficacy of Bu.

[Clinical feature and pathology of airway disease in Sjögren's syndrome].

Airway disease of Sjögren's syndrome (SjS) has been studied since years ago. Many researchers have thought xerotrachea induced dry cough. Recently it was revealed that there was no xerotrachea but atrophic change and proliferation and growth of blood vessel. Various airway diseases of SjS have been reported, including bronchitis, bronchiolitis and follicular bronchiolitis. Rarely patients may lose their life for them. Patients of SjS without respiratory manifestations often show pulmonary function abnormalities. We compared pulmonary function of SjS and collagen disease without SjS. To our surprise, both of them show obstructive pattern with almost same frequency. Probably it means that obstructive airway diseases are not characteristic for SjS. Recently bronchial hyperresponsiveness in SjS is drawing the attention. Treatment and prognosis of the airway diseases in SjS have been not clarified, so further studies have to be piled up from now on.

Bronchial Hyperreactivity

[Immunoglobulin decrease accompanying adverse reactions with bucillamine].

Forty-two RA patients treated with Bucillamine (B) were divided into two groups with (n = 26) and without (n = 16) adverse reactions, and IgG, M, A and total protein concentration x globulin ratio (Ig) before and after B were compared. IgG did not differ before B administration, but was significantly lower in the adverse reactions group after B, and in each group it was significantly lower after B treatment with a much greater decrease demonstrated in the adverse reactions group. IgA before B did not differ either between the two groups or between before and after B administration in the non-adverse reaction group, but was significantly lower only after B in the adverse reaction group. IgM did not differ either before or after B treatment between the two groups, and was significantly lower following B administration in both groups. Ig showed results similar to IgA. In the adverse reaction group, IgG, A, M and Ig recovered following suspension of administration. It is concluded that the adverse reaction brought on by B administration might occur with the decrease in immunoglobulin.

[Reassessment of usefulness of salivary scintigraphy in diagnosis of Sjögren's syndrome].

Dynamic salivary scintigraphy using 99mTcO4- was performed in 95 patients (a total of 366 glands) suspected of Sjögren's syndrome (SjS) and the results were compared with SjS diagnostic criteria by the Japanese Welfare Ministry. Time-activity curve of ROI created over each salivary gland was classified into four patterns, that is, N, M1, M2, F pattern, depending upon trapping, accumulation and secretion response to the tartaric acid. In definite SjS patients, most of the patients had different patterns in the four glands, and classical F (flat) pattern in all of the four glands was uncommon (3/38, 8%). F and/or M2 (no response to secretary stimulation) patterns were seen in 68.5% (98/143) of the all glands. Provided that F and/or M2 patterns in 2 or more glands is scintigraphic criteria for diagnosis of SjS, sensitivity, specificity and accuracy were 82.9%, 59.5%, 70.8%, respectively. As for salivary uptake of 99mTcO4-, there is a significant difference between the patients with definite SjS and control subjects (parotis p < 0.05, submandibular gland p < 0.01), but there is no significant difference among the patients with definite SjS, suspected SjS and chronic sialoadenitis. In conclusion, the four patterns classification may have potential in screening patients with SjS.

[A case of primary antiphospholipid syndrome with fever, pulmonary thromboembolism and endocardial lesion].

A previously healthy 16-year-old girl complaining of fever, hemosputum, chest pain and dyspnea was hospitalized. On admission, physical examination revealed mental confusion, holosystolic heart murmur, and swelling of the left foot. Laboratory investigations showed anemia, leukocytosis, thrombocytopenia, activation of inflammatory reactions, prolongation of PT and APTT, and hypoxia. Antinuclear antibody test was negative. There were no other findings suggestive of collagen diseases such as SLE. Chest X-ray showed consolidation in the left lower lung field and pleural effusion. Echocardiography disclosed a mass lesion in the left atrium in contact with the mitral valve, and mitral regurgitation. No findings indicative of an infectious etiology were present. The patient rapidly improved with high dose corticosteroid and anticoagulant therapy. A venogram of the lower extremity disclosed deep venous thrombosis. A lung ventilation-perfusion scan revealed multiple pulmonary thromboemboli. Elevation of anticardiolipin antibody was noted. Based on these findings, the diagnosis of primary antiphospholipid syndrome was made. Further administration of steroid and anticoagulant resulted in decrease of the titer of anticardiolipin antibody. This is the second report of primary antiphospholipid syndrome in Japan. The clinical significance of this disease is also discussed.

[The correlations of respiratory function tests, chest roentgenographic manifestations and respiratory symptoms in the patients with systemic sclerosis, using a scoring-method for symptoms].

Analytical studies were performed on the respiratory symptoms in 31 systemic sclerosis cases. Respiratory symptoms were expressed by scores from 1) coughs, 2) difficulties in walking, 3) difficulties in ascending, 4) dyspnea . In early stage cases with slight clinical symptoms, %VC was always normal, but %DLco was generally reduced. Because of good correlation between respiratory functions and clinical symptom scores, it is available to use the scores as well as %DLLco and %VC to diagnose slight lung involvement in systemic sclerosis. Also the clinical symptom scores are related to the roentgenographic manifestations of lung fibrosis which are characteristic in systemic sclerosis. Therefore we felt that this system of clinical symptom scores was useful to follow-up the patients.

[Denopamine responsive pulmonary hypertension in a patient with collagen disease].

We report a 52-years-old female patient with collagen disease and pulmonary hypertension. Denopamine, beta-adrenergic agonist, decreased her high pulmonary arterial pressure and improved dyspnea on exercise after long term use. She had suffered from Raynaud's phenomenon, pulmonary fibrosis, shortening of lingual frenulum and positive ANA and RA test. Although her pulmonary fibrosis had been well controlled by azathioprine, dyspnea on exercise became worse, so she admitted to our hospital for further examination in Feb 1988. Right heart catheterization revealed her high pulmonary arterial pressure (mean 29 mmHg). Under right heart catheterization, denopamine markedly decreased her pulmonary arterial pressure and increased her cardiac output. After about 6 weeks' use of denopamine, her mean pulmonary arterial pressure decreased to 15 mmHg, PO2 increased from 43 to 62 mmHg and dyspnea improved. Denopamine has been regarded as a selective beta 1-adrenergic agonist. In this case, denopamine might have beta 2-agonist effect to dilate pulmonary vasculature, or have secondary effect to increase PO2 by the improvement of cardiac function. Denopamine might be useful for pulmonary hypertension with collagen diseases.

A patient with systemic lupus erythematosus presenting both central nervous system lupus and steroid induced psychosis.

We describe a patient with systemic lupus erythematosus (SLE) who presented with both steroid induced psychosis and central nervous system (CNS) SLE at different times. The cerebrospinal fluid (CSF) immunoglobulin indices and electroencephalogram were found to be helpful and reliable for the correct diagnosis of both conditions. There have been no reports of patients with SLE who showed both steroid induced psychosis and CNS SLE, and our patient has provided substantial evidence that both conditions can occur in the same individual.

Effect of phytohemagglutinin on concentrations of glucocorticoid receptors in lymphocytes from patients with connective tissue diseases.

We examined the effect of phytohemagglutinin (PHA) on glucocorticoid receptors in lymphocytes, using peripheral mononuclear leukocytes from patients with connective tissue diseases. Glucocorticoid receptor concentrations and dissociation constants (Kd) for [3H]dexamethasone binding to lymphocytes from patients with rheumatoid arthritis, Sjögren's syndrome, and other connective tissue diseases did not differ significantly from values for normal volunteers. Kd values correlated positively with glucocorticoid receptor concentrations after PHA stimulation both in normal volunteers and in patients with connective tissue diseases. The correlations (r values) for the control group and patients were not significantly different.

Trapidil, a coronary vasodilator, inhibits IgE-mediated histamine release from human basophils.

The effect of trapidil, a coronary vasodilator, on histamine release from human basophils was studied. Trapidil inhibited IgE-mediated, anti-IgE-induced histamine release in a dose-dependent fashion (ID50 : 5.8 x 10(-4) M). In contrast, histamine release induced by calcium ionophore A23187, formyl-L-methionyl-L-leucyl-L-phenylalanine and polymyxin B was not affected by this agent. Two-stage experiments suggested that the effect of trapidil was exerted not only on the first Ca2+-independent activation stage, but also on the second Ca2+-dependent stage. These results, together with recent reports that trapidil has a bronchodilator action, suggest that trapidil could be used in the treatment of asthmatic patients.
