Effect of phytohemagglutinin on concentrations of glucocorticoid receptors in lymphocytes from patients with connective tissue diseases.

We examined the effect of phytohemagglutinin (PHA) on glucocorticoid receptors in lymphocytes, using peripheral mononuclear leukocytes from patients with connective tissue diseases. Glucocorticoid receptor concentrations and dissociation constants (Kd) for [3H]dexamethasone binding to lymphocytes from patients with rheumatoid arthritis, Sjögren's syndrome, and other connective tissue diseases did not differ significantly from values for normal volunteers. Kd values correlated positively with glucocorticoid receptor concentrations after PHA stimulation both in normal volunteers and in patients with connective tissue diseases. The correlations (r values) for the control group and patients were not significantly different.

Trapidil, a coronary vasodilator, inhibits IgE-mediated histamine release from human basophils.

The effect of trapidil, a coronary vasodilator, on histamine release from human basophils was studied. Trapidil inhibited IgE-mediated, anti-IgE-induced histamine release in a dose-dependent fashion (ID50 : 5.8 x 10(-4) M). In contrast, histamine release induced by calcium ionophore A23187, formyl-L-methionyl-L-leucyl-L-phenylalanine and polymyxin B was not affected by this agent. Two-stage experiments suggested that the effect of trapidil was exerted not only on the first Ca2+-independent activation stage, but also on the second Ca2+-dependent stage. These results, together with recent reports that trapidil has a bronchodilator action, suggest that trapidil could be used in the treatment of asthmatic patients.

Pachymeningitis in microscopic polyangiitis (MPA): a case report and a review of central nervous system involvement in MPA.

A case of microscopic polyangiitis (MPA) with pachymeningitis is described. The patient had renal, skin, gallbladder and peripheral nervous system involvement, simultaneously with pachymeningitis. Necrotizing glomerulonephritis with crescent formation, and necrotizing small vessel vasculitis in the kidney and skin were confirmed by biopsy. A highly elevated titer of antineutrophil cytoplasmic antibody for myeloperoxidase (MPO-ANCA) was observed. All of the clinical and laboratory abnormalities improved with high-dose pulse and conventional steroid therapy. The literature on central nervous system involvement in MPA and perinuclear-ANCA (p-ANCA)-related vasculitis is reviewed. This case serves to emphasize that pachymeningitis can occur as one of the features of MPA.
