[A case of a premenopausal woman with advanced breast cancer treated with aromatization inhibition in combination with luteinizing hormone-releasing hormone agonist].

A 44-year-old premenopausal woman having local recurrence and pleural and bone metastases of breast cancer was treated with aromatization inhibition in combination with Luteinizing Hormone-releasing Hormone (LH-RH) agonist. The dominant site of metastasis was a painful local lesion invading the chest wall. A partial response by reducing the size of the local lesion was attained 3 months after initiation of treatment. This result suggested that treatment using aromatization inhibition in combination with LH-RH agonist would be effective in premenopausal breast cancer. To confirm the effectiveness of this treatment, comparative study between aromatization inhibition in combination with LH-RH agonist aromatization inhibition alone and anti-estrogen in combination with LH-RH agonist are needed.

[Clinical effects of FUT-187 in reflux esophagitis after gastrectomy].

FUT-187 was orally administered to 38 patients with postgastrectomy reflux esophagitis for 4 weeks. The drug reduced the chief subjective symptoms of reflux esophagitis, such as heartburn, chest pain, precordial pain, and dysphagia for solids in 78.1% of patients. Redness, edema and erosion were also reduced in 53.3% of patients as determined endoscopically. Overall, FUT-187 exhibited an excellent therapeutic effect on the reflux esophagitis which was refractory to conventional treatments.

Cloning and expression of the Erwinia herbicola tyrosine phenol-lyase gene in Escherichia coli.

The tyrosine phenol-lyase (TPL) gene of Erwinia herbicola was cloned and expressed in Escherichia coli, and the complete nucleotide sequence of the gene determined. The TPL gene comprises 1368 bp, encoding 456 amino acids which have 90% amino acid identity with TPL from Citrobacter freundii. After replacing the 5'-flanking region of the TPL gene with the E. coli lac promoter, TPL protein could be hyperproduced constitutively in E. coli without induction by L-tyrosine.

Percutaneous treatments for biliary diseases.

Percutaneous treatment of biliary disease was administered in 173 cases, mainly among high-risk or elderly patients. Diagnosis was acute cholecystitis in 32 cases, acute cholangitis in 16, hepatic abscess in five, gallbladder stones in 28, common bile duct stones in 11, intrahepatic stones in five, malignant obstructive jaundice in 61, and benign biliary stenosis in 15. The treatment was successful in 158 of 173 cases (91.3%). Complications were encountered in 17 (9.8%). Most of these occurred during the initial period and were mild and controllable with conservative treatments. There were two deaths (1.2% of patients); one was not related to the procedure. These data suggest that percutaneous treatments are safe and effective even among elderly or high-risk patients with biliary diseases.

[Clinical observations of hepatic artery infusion therapy combined with hepatic artery ligation in metastatic liver cancers].

Twenty-five patients with liver metastases from advanced gastrointestinal cancers were treated by hepatic arterial infusion chemotherapy by insertion of a catheter into the hepatic artery. All patients also underwent ligation of the hepatic artery. The anticancer agents used were adriamycin 10-30 mg, mitomycin C 10-20 mg and 5-fluorouracil 250-500 mg. The effect was assessed according to the criteria for clinical evaluation of chemotherapy for solid cancers by Koyama and Saito. The response rate for partial response was 24.0% among all 25 patients, including 25.0% (2/8) for stomach cancer and 28.6% (4/14) for colorectal cancer. The survival time with palliative resection of the primary lesion was prolonged significantly (p less than 0.05) in comparison with no resection. As for the side effects of hepatic arterial infusion chemotherapy, bone marrow suppression was most frequently found in 80.0% of patients, followed by gastrointestinal symptoms in 68.0%. In addition, fever occurred in 20.0%, stomatitis in 20.0%, liver dysfunction in 16.0% and alopecia in 4.0%. However, the side effects were all controllable. As to hepatic artery ligation, there was no serious complication except for one case of liver abscess.

[Adenoma of the breast: histopathological study of 9 cases].

Adenomas of the breast are cellular epithelial lesions, which are subclassified into two types, tubular adenomas (TA) and lactating adenomas (LA). In 3365 cases of benign epithelial tumors of the breast, including fibroadenomas, filed in the Hiroshima Tumor Tissue Registry between 1973 to 1985, only nine adenomas (TA-4, LA-5) or 0.3% were filed. All adenoma cases involved females, with an average of 27.4 years. Grossly, the tumors were found to vary from 1.5 to 5 cm in diameter and were sharply demarcated. Microscopically and ultrastructurally, the small tubules found in TA are identical to normal ducts. According to progress from pregnancy to lactation, the tubular tissue in LA demonstrate breast increasing secretory activities. The absence of TA during pregnancy, the structural similarity and the fact that some LA had already developed into a palpable mass before pregnancy are signs that indicate that all TA and LA share the same biologic process.

A new method for internal biliary drainage in malignant obstructive jaundice. Intrahepatic cholangioenteric bypass via PTCD fistula.

A new surgical method for internal biliary drainage was devised, consisting of percutaneous transhepatic cholangio-drainage (PTCD) and Roux-en-Y anastomosis. The PTCD fistula is connected to the jejunostomy by a drainage catheter buried in the subcutaneous space. As compared with conventional methods for surgical internal biliary drainage, this method has the following advantages: It can be easily performed even when other types of internal drainage are not feasible, the catheter can readily be changed if it becomes obstructed, and percutaneous cholangiography, irrigation or drug infusion can be performed at any time via the drainage catheter.

[Synovial sarcoma of the abdominal wall].

A case of a synovial sarcoma arising in the abdominal wall in a 60-year-old man is reported. Histologically, a characteristic biphasic cellular pattern with epithelium-like cell complex and fibrosarcomatous spindle cell area was found. Mucinous materials within the epithelium-like cells, intercellular clefts and pseudoglandular spaces stained positively with PAS, alcian blue, colloid iron and mucicarmine stain. The staining characteristics of these materials remained unchanged after treatment with diastase and hyaluronidase. This histochemical finding makes the diagnosis of a peritoneal mesothelioma unlikely. A brief review of previously reported cases of synovial sarcoma arising in the abdominal wall is also presented.

[Phase II study of THP patients with gastrointestinal cancer].

A phase II study on THP((2''R)-4'-0-Tetrahydropyranyladriamycin) was performed in 47 patients with advanced or recurrent gastrointestinal cancer through the cooperation of nine institutions in Hiroshima Prefecture from April 1982 to November 1984. THP was given by means of intravenous infusion and/or intraaortic infusion and the 47 cases were divided into two groups according to the method of administration: (A) 40-60 mg/body every 3 or 4 weeks, or (B) 30 mg/body every week. Among 24 evaluable cases, partial response (PR) was observed in two cases of recurrent metastatic lymph nodes in gastric cancer patients. The (A) method of administration was more effective than (B). Subjective side effects observed were appetite loss, nausea, vomiting and general fatigue, but these were not so severe. Leukocyte nadir occurred at the 1st or 2nd week of THP administration, but thrombocytes were not appreciably decreased.

Nonsurgical treatment of cholecystocholedocholithiasis. A case report.

We have devised nonsurgical cholecystolithotomy for the extremely ill or elderly patients. Nonsurgical treatment of cholecystocholedocholithotomy is possible by combining our nonsurgical cholecystolithotomy with endoscopic sphincterotomy. The procedure is very useful for extremely ill or elderly patents because it is safe and noninvasive. This report describes the nonsurgical treatment of cholecystocholedocholithiasis.

A new method for nonsurgical cholecystolithotomy.

A new method of nonsurgical removal of gallbladder stones is presented herein. The method consists of two parts: a, ultrasonically guided percutaneous transhepatic gallbladder drainage, and b, extraction of gallbladder stones using a choledochofiberscopic maneuver. The procedure is safe, less stressful than a surgical procedure and easy to perform. The method is useful for extremely ill or elderly patients and those who prefer not to undergo surgical treatment.

[Phase II study on MCNU in patients with advanced or recurrent gastrointestinal cancer].

A phase II study on MCNU (Methyl 6-[3-(2-chloroethyl)-3-nitrosoureido]-6-deoxy-alpha-D-glucopyranoside) was performed in 33 patients with advanced or recurrent gastrointestinal cancer under the cooperation of eight institutions in Hiroshima Prefecture. MCNU was given by means of intravenous drip infusion and the 33 cases were divided into three groups according to the method of administration; (A) 50mg/m2 every 1 or 2 weeks, (B) 70mg/m2 every 2 weeks or (C) 90mg/m2 every 6-8 weeks. Among 28 evaluable cases, 1 complete response (CR) and 1 partial response (PR) were observed and these two cases were gastric cancer patients. Platelet nadir occurred at the 3rd or fifth week after MCNU administration, but the leukocyte count was not so decreased. Subjective side effects were nausea, general fatigue and vomiting, but these were observed to be only mild.
