Hip fractures in healthy patients: operative delay versus prognosis.

One hundred and forty five women who had undergone hemiarthroplasty for a subcapital fracture of the femoral neck but who were otherwise fit were studied to determine whether undue delay between injury and operation influenced their social circumstances three months after surgery. The median delay for those patients who showed good rehabilitation at three months was 29 hours, but for those who showed poor rehabilitation it was 57 hours. This difference was significant. It is suggested that a subcapital fracture in an otherwise fit elderly patient should therefore be regarded as a surgical emergency.

Organization of citric acid cycle enzymes into a multienzyme cluster.

The possibility that some of the enzymes of the citric acid cycle may be loosely associated into a multienzyme cluster has been investigated using extracts prepared by gentle disruption of cells. Gel filtration and sucrose density gradient centrifugation have shown that five sequential enzymes of the cycle specifically associate into a cluster: fumarase, malate dehydrogenase, citrate synthase, aconitase and isocitrate dehydrogenase. Ultrasonication destroys the abilities of the enzymes to associate. The cluster could catalyse the sequence of reactions leading from fumarate to oxoglutarate and has been found in extracts of several bacterial species as well as rat liver mitochondria.

Goals, objectives, and competencies for reference service: a training program at the UCLA Biomedical Library.

The UCLA Biomedical Library, in cooperation with the UCLA Graduate School of Library and Information Science, offers a medical library internship program for second-year library school students. Goals, objectives, competencies, and training guidelines have been developed for the reference services section of the internship, including reference desk experience, online searching, group discussions, assigned readings, and training new staff members, allows flexibility in meeting the differing interests, needs, and abilities of trainees.

Adult respiratory distress syndrome in babesiosis.

A patient is described who contracted transfusion-induced babesiosis, and later developed acute respiratory distress syndrome (ARDS) as a fatal complication. ARDS has been reported in patients with Plasmodium falciparum malaria, but to our knowledge has not been observed as a complication of babesiosis.

Atrial infarction complicating an acute inferior myocardial infarction.

Atrial fibrillation, coronary sinus rhythm, and slow atrial flutter developed in a patient with ECG findings of an acute inferior myocardial infarction. Hemodynamic measurements were suggestive of predominantly right ventricular involvement. A gated cardiac blood pool study demonstrated normal right and left ventricular wall motion with an enlarged, non-contracting right atrium. This led to the antemortem diagnosis of atrial fibrillation.

Acute renal failure after enhanced computed tomography.

Acute renal failure has been reported after the administration of a variety of radiographic contrast media. We treated a patient with preexisting renal insufficiency who received contrast media during computed tomography (CT) and had acute renal failure. The fractional excretion of sodium (FENa) was observed to be 0.2% before contrast media exposure and 3.5% after exposure. Patients with risk factors leading to contrast media-induced acute renal failure should be carefully screened before receiving enhanced CT.
