A cause and effect relationship of Aflatoxin and liver cancer is well established. The incidence of liver cancer is 3.6% in Karachi (Zuberi et al., 1976) and Aflatoxins have been detected in 43% of 28 samples of food analysed in this study. The long term ingestion of aflatoxin with food may have some effect on the existing pattern of liver cancer in Karachi.

The number of rosette forming T cells was significantly reduced in patients with acute viral hepatitis, cirrhosis of the liver and liver cancer. The frequency of T cells in various hepatic disorders correlated well with the impairment of delayed hypersensitivity response to DNCB.
Levels of immunoglobulins in serum were determined in 41 healthy subjects and 128 professional blood donors. In healthy subjects mean levels +/- S.D. were: IgG 1260 +/- 492.9 mg/dl, IgM 62.4 +/- 18.2 mg/dl, and IgA 183.4 +/- 70.1 mg/dl. Of 128 blood donors 112 were hepatitis Bs antigen (HBsAg) negative and 16 were positive. A significant (P is less than 0.001) elevation of all three immunoglobulins was observed in HBsAg positive group suggesting increased antibody formation due to persistance of antigen.
Serum immunoglobulins were determined in 39 healthy subjects and 55 patients with a variety of acute and chronic liver diseases. Elevation of IgG and IgA was frequently observed in healthy subjects and patients with acute viral hepatitis, liver cancer and miscellaneous liver disorders. IgG and IgM were elevated in cirrhosis of the liver.
The composition of gall stones removed surgically from 31 patients was determined by microchemical analysis. Twenty six had mixed infective, 2 cholesterol and 3 pigment stones. All the stones consisted of combination of various constituents in varying amount. Bilirubin was detected in the centre and peripheral layers of all, protein was found in the centre of 84.6% and cholesterol in the centre of 54% and the peripheral layers of all the gall stones. The findings in this study suggest that infection is the most likely cause of biliary lithogenesis.

Twenty Six mixed infective and 2 cholesterol gall-stones were stained for cholesterol, calcium, fat and mucoproteins. Calcium and cholesterol were found to be the main constituents of all the calculi. Calcium was found mainly in the centre and mucoprotein in the peripheral layers. The fine crystals of cholesterol were demonstrated in those biliary calculi which did not contain mucoprotein. It appeared that calcium might have acted as a cementing force rather than mucoprotein in this study.

Clinical examination and liver function tests of 14 HBAg positive and 8 HBAg negative blood donors were done. A history of Jaundice, contact with hepatitis and hepatosplenomegaly were more frequently observed in the antigen positive donors.
Amino acid imbalance ratio was determined in apparently healthy Pakistanis and patients with hepatitis and cirrhosis of the liver. The ratio was normal in 75% of the patients with actue viral hepatitis but in only 5% with cirrhosis of the liver. The ratio was abnormal in 25% cases of acute viral hepatitis possibly due to aminoaciduria. The abnormal ratio in cirrhosis of the liver indicated the functional capacity for albumin synthesis and correlated well with serum albumin concentration.

During a period of 16 months 114 patients with cholelithiasis were studied. The diagnosis was confirmed by cholecystogram in 41 cases, by surgery in 28 and both surgery and the x-ray in 45 cases. There were 100 females and 14 males and their average age was 43.2 years. The average number of pregnancies in females was 6.5. Clinically the jaundice was observed in 9% and the gall bladder was palpable in 47% of cases. Cholelithiasis appeared to be related to increased parity but not to the body weight.
The levels of cholesterol, calcium and bilirubin were determined in the serum and gall stones in 47 cases with cholelithiasis. The mixed stones were observed in 87.5%, high cholesterol stone in 8.5% and high bilirubin stones in 4.2%. Serum and stone cholesterol levels were inversely proportional and the bilirubin levels were directly proportional to each other.
