Reflex sympathetic dystrophy in a 12-year-old twin with comorbid conversion disorder in both twins.

A case of reflex sympathetic dystrophy is presented in a 12-year-old girl with comorbid conversion disorder. Her identical twin also had a conversion disorder. This is the first reported case of coexistence of reflex sympathetic dystrophy and conversion disorder. It is important for clinicians to be aware that these conditions may coexist since the presentation of symptoms differ, even though there are shared features of treatment.

The ethical issues of enforcing parents' participation in the psychiatric treatment of minors.

Currently, when parents do not comply with the recommended psychiatric treatment for their child through the Mental Health Act, the mental health practitioner and the statutory social worker are faced with unclear guidelines as to how best to respond. Under the Israeli Mental Health Act and the existing Youth Law, there are no effective means of enforcing parental involvement in the psychiatric treatment of "at risk" minors. Non-involvement may seriously compromise the assessment and treatment of these minors, resulting in ongoing risk of developmental damage. Proposed changes in the current Youth Law include ratifying means of enforcing parental involvement. Ethical issues regarding such a proposal are discussed using a case vignette. Clinical guidelines for parental participation in minors' psychiatric treatment are discussed.

Masturbation in prepubescent children: a case report and review of the literature.

A 10-year-old girl observed to have episodes of flushing, pallor and weakness, was referred for investigation of possible carcinoid syndrome. The cause of these episodes was masturbation. Masturbation occurs at all ages and in most cases is considered normal behaviour. Lack of recognition of this phenomenon in children may lead to unnecessary investigations and treatment.

Enforced psychiatric treatment of minors in Israel: the interface between the Mental Health Act and the Youth Law.

Proposed changes in the current Youth Law will provide enforced psychiatric treatment, including civil commitment, for "at risk" minors. Such minors are considered at significant risk of future developmental damage without such treatment. The potential exists for a clash between the proposed criteria for enforced psychiatric treatment in the Youth Law and the existing criteria in the current Mental Health Act: "need for treatment" in the former versus "immediate and physical danger to self or others as a result of psychotic illness" in the latter. Proposed changes in the Mental Health Act include broadening the criteria for enforced psychiatric treatment of minors. The revised criteria would include non-psychotic emotional disorders (such as recurrent suicidal behavior) which endanger the minor's physical and emotional development. These changes are welcomed. The authors suggest the adoption of "need for treatment" criteria, as against the current "physical danger to self or others" criteria for enforced psychiatric treatment of minors.

An integrative model for the delivery of mental health services for minors--the role of psychiatric hospitalization.

The recent increase in psychiatric hospitalization of minors is reviewed. The Israeli mental health legislation is described and the lack of adequate coordination between agencies delivering mental health services for minors is noted. Two case vignettes are described which demonstrate the management problem posed by severely emotionally disturbed minors at risk who are not adequately catered for by the current Youth Protection Law or the mental health legislation. An integrative model for the delivery of mental health services for minors is proposed demonstrating the need for close collaboration between psychiatric and statutory welfare services and the role of psychiatric hospitalization.

Involuntary psychiatric hospitalization of minors.

Hospital records of the psychiatric hospitalization of minors living in the south of Israel were studied retrospectively for age, diagnosis and legal status on admission. The records of those minors who were involuntary hospitalized were investigated for behavioural symptomatology and justification for involuntary hospitalization. A range of psychiatric diagnoses was found, excluding diagnoses of schizophrenia and bipolar affective disorder. Explanations for these findings are offered.

Traffic accident injuries of children: the need for prospective studies of psychiatric sequelae.

This paper presents a literature review of epidemiological data relevant to traffic accidents of children, including their personality and behavioral predisposition, social factors, association with risk taking behavior, rehabilitation, and incidence of post traumatic stress syndrome. The need for prospective studies of psychiatric sequelae of such accidents is highlighted. Three case vignettes are presented.
