Of 83 women who responded to surveys exploring postabortion coping, 30 women reported anniversary reactions associated with the abortion or due date. Women in the anniversary reaction group more often reported ambivalence about the decision to abort (p less than 0.007). This group also acknowledged fewer suicidal thoughts and attempts, but expressed more concern about verbally abusing their children (p less than 0.04). Strong trends toward dependency and somatoform disorders were present among anniversary group members (p less than 0.06). The authors recommend that physicians be alert to the possibility of hidden anniversary reactions of both physical and emotional nature during patient evaluation.
Sexual attitudes and behavior of adolescent females have been the topic of much interest over the past decade. Feelings about contraception, conception, and abortion have been described in relation to the adolescents' beliefs about the possibility of becoming pregnant, who will or will not "protect" them, and the influence of significant others on their decision making. This study explores differences in 35 women who had abortions during their teenage years with 36 women whose abortions occurred after the age of twenty. A demographic questionnaire, the Millon Clinical Multiaxial Inventory, and the Beck Depression Inventory were completed by women who were members of a patient-led support group. Premorbid psychiatric histories, the decision-making process itself, and distressing symptoms postabortion are reported. Specific differences in perceptions of coercion, preabortion suicidal ideation, and nightmares post-abortion were found in the adolescent group. Antisocial and paranoid personality disorders as well as drug abuse and psychotic delusions were found to be significantly higher in the group who aborted as teenagers. Hypotheses regarding the influences of adolescent development on mother/child relationships, power struggles, and the use of fantasy as a coping device are explored.
Smoking cessation programs are usually evaluated in terms of the percentage of participants who have stopped smoking at least 6 months after the program's completion. This paper shows how a relatively low rate of behavior change may actually result in a high ratio of benefits to costs. Cost-benefit analysis is done for a specific program but the procedures and cost estimates are generalizable to other smoking cessation programs. Benefits to the firm included reduced costs of insurance and the savings due to employee absenteeism and disability. Costs included the program costs as well as the opportunity costs of the participants. Data from the literature and from the specific program are combined to form conclusions about the effectiveness of the program.
Parental report of development milestones influences the physician's decision to refer for developmental evaluation and early education. The predictive validity of commonly reported milestones was evaluated by collecting data from records in a comprehensive care clinic. Multivariate analyses determined the relationships among age of milestone achievement and Wechsler intelligence Scale for Children-Revised (WISC-R) intelligence quotient (IQ) scores. Other routinely collected medical and demographic information was then added, and stepwise regressions determined the best combinations of predictors of WISC-R IQ scores. Milestones alone predicted 24 to 42% of the variance in WISC-R IQ scores, suggesting that there are meaningful relationships between the age of attainment of milestones and later intellectual functioning. However, when other medical and demographic information was added to the prediction equation, the predictive power of milestones was overshadowed by that of the new variables. Milestones are validated as one criterion for referral for specialized early development services, but adding other objective data, including standardized developmental screening, is recommended for optimal decision making.
The practice of allowing juvenile participants in a drug intervention program to extend their participation in the program was examined. The program's therapists felt that such additional participation would be therapeutically beneficial to the juveniles, but at the same time it would deny other juveniles entrance into the program. The results suggest that the practice of allowing participants to attend additional groups in programs which are rather highly structured is not beneficial, and in some cases may result in a loss of the gains made during the initial group.
