A case of epididymal metastasis from prostatic carcinoma is presented. The initial histologic findings were suggestive of adenomatoid tumor, but a diagnosis of metastatic adenocarcinoma of prostatic origin has been established by prostatic acid phosphatase and prostate-specific antigen immunoperoxidase staining.
The most common sequela of the elbow joint after injury is decreased range of motion, i.e. elbow contracture. From January 1986 to July 1987, the Orthopaedic department of Kaohsiung Medical College collected data of 10 elbow contracture on 9 cases. Treatment included capsulotomy and joint debridement with immediate postoperative rehabilitation. This paper reports the clinical follow up results with an average time of 7 months after the operation. Range of motion increased 50 degrees on the average. No residual pain was complained. Activity of daily living was improved and the results were satisfied.

The purpose of this study was to document the effect of muscle fatigue on glenohumeral kinematics. Twelve male volunteers without shoulder disease and with an average age of 27 years were studied. Glenohumeral anteroposterior radiographs were taken at 45 degrees intervals as the arm was abducted in the plane of the scapula from 0 degree to 135 degrees. This series of radiographs was performed both before and immediately after the subject performed a series of deltoid and rotator cuff fatiguing exercises. The average humeral head position or translation before and after muscle fatigue for each arm angle was compared. For all subjects, before fatigue, the position of the humeral head was below the center of the glenoid for all angles of abduction. There was essentially no change in position of the humeral head in the prefatigue state, as the arm was abducted from 0 degree to 135 degrees with no more than an average 0.3 mm of total humeral head excursion. After fatigue, excursion of the humeral head increased to an average of 2.5 mm between the tested positions. The position of the humeral head with the arm at 0 degree of abduction was lower or had migrated inferiorly compared with the rested state, with an average 1.2 mm significant increase in inferior translation. With the initiation of abduction, the humeral head demonstrated significant superior migration or translation in all positions tested. This result has important implications for conservative treatment of shoulder impingement and underscores the importance of rehabilitation to maximize the endurance and strength of the rotator cuff musculature.
To evaluate the potential for the chemical dissolution of gallstones, 480 stones from 214 patients were studied. The stones were obtained via surgery or endoscopically. They were classified into cholesterol-rich mixed stones, brown pigment stones and black stones. The composition of bilirubin and cholesterol was determined by Fourier transformed infrared spectroscopy. Two per cent tetrasodium ethylenediamine acetate (EDTA), dimethylsulfoxide (DMSO) or methyl-tert-butyl-ethylene (MTBE) were used to dissolve the stones. To enhance solubility, surfactant polysorbate-20 was used to mix two of the individual three solvents. Methyl-tert-butyl-ethylene was found to have the best dissolution ability (by dry weight) 94, 13.4 and 20% for mixed, brown and black stones, respectively. Dimethylsulfoxide resulted in 13, 14 and 25% dissolution and EDTA 9.5, 13 and 16.5%. In contrast, pure water dissolved 4, 6 and 10.4% of the stones, respectively. A combination of the dissolution agents did not enhance the dissolution rate. In fact, the combination of solvents unexpectedly reduced the solubility of the stones: EDTA/MTBE was 17.5, 6.7 and 16.0%; DMSO/MTBE 43.2, 21.9 and 18.0%; DMSO/EDTA 9.1, 7.0 and 9.6%. In conclusion, cholesterol-rich mixed stones were able to be dissolved using MTBE but results of contact dissolution for gallstones are still far from satisfactory.
Seven male nonathletes who had Bristow procedures for shoulder dislocation were analyzed by dynamic electromyography (EMG) and Cybex strength measurement to evaluate the function of elbow flexion. Bilateral strength of elbow flexion also was measured in the 10 controlled subjects to compare the dominant and nondominant arms. The EMG data showed the operated short head of biceps function at low constant intensity, compared with the nonoperated side, while the long head of biceps and brachialis increased their activity. The strength measured by the Cybex demonstrated the elbow flexion on the operated side was not significantly different from the normal group. These data demonstrated that the Bristow procedure reduced the dynamic response of the short biceps which was compensated for by the increase in level of activity of the long heads and brachialis muscle. As a result, normal range of elbow function was restored. Our patients had had intense rehabilitation programs. This appears to be an essential part of the postoperative management to develop the needed compensatory muscle action.
