Cyclosporin-erythromycin interaction in renal transplant patients.

1. The interaction between cyclosporin (CyA) and erythromycin was studied in renal transplant patients following oral and intravenous administration of CyA. 2. Blood and plasma CyA concentrations and blood concentrations of metabolite 17 were measured by h.p.l.c. 3. Erythromycin produced almost a two-fold increase in bioavailability, from 36% to 60%; with a small (13%) decrease in clearance of CyA. 4. The metabolite 17 data further support the postulate that erythromycin increases the absorption of CyA rather than inhibits its metabolism, as generally believed.

Management of human keratomycosis with miconazole.

A prospective clinical study was undertaken to evaluate the efficacy of 1% miconazole in 85 cases of smear-positive fungal corneal ulcers. Miconazole therapy healed 44 ulcers, thereby achieving a success rate of 64.7%. The most common organism encountered was Aspergillus followed by Fusarium, Philophora and Candida. The average healing time was found to be 22 days. The drug was well tolerated by the eye and no symptoms of ocular toxicity were observed. The findings of this study suggest that miconazole is an effective agent for the management of human keratomycosis.

Asymptomatic salpingitis in intrauterine contraceptive device users.

Serial histological sections of the fallopian tubes of 50 asymptomatic subjects undergoing voluntary tubal ligation were evaluated for morphological evidence of inflammation. Twenty-five subjects had history of IUD use (the study group) and the rest 25 had never used a device (the control group). Evidence of inflammation was found in 11 out of 25 subjects in the IUD users compared to only 2 out of 25 subjects in the non-users (p less than 0.001). The relative risk was 5.5 times. Both acute and chronic inflammation of the oviducts were observed more often in the IUD users, though clinically and at surgery there was no apparent evidence of pelvic inflammatory disease.

Long-term effects of immunization with porcine zona pellucida on rabbit ovaries.

Groups of rabbits were immunized with 4 doses of porcine zona pellucida antigen along with different adjuvants. The presence of antibody after immunization was checked periodically using different assay systems. The impact of immunization on ovaries was evaluated at different time intervals up to 2 years. Results of the study showed generation of high titre antibodies which could be detected until the end of the period of observation. The antibodies specifically bound to zona pellucida in situ were demonstrated by immunoperoxidase staining, and in vitro by inhibition of enzymatic digestion of zonae. Histologically, antibodies had numerous pathological effects on ovaries including thinning of the cortex, aberrant cell clusters, stromal cell hyperplasia, perifollicular fibrosis, irregular zona formation and arrest of follicle development. These changes could be correlated with the length of exposure to antibodies. It is concluded that heat-solubilized porcine zona (HSPZ) protein is a very strong immunogen and causes infertility by arresting the maturation of developing follicles. There is a need to identify the sperm receptor so that immunization could selectively impair sperm-egg interaction.

Inter-state migration and national integration.

"In this paper an attempt has been made to assess the pattern of social mobilization, and also to identify the heterogeneity profile of the different states of the Indian union." Interstate migration flows are analyzed, and open or hospitable states are compared with insular ones in terms of net migration hospitality ratios. Findings indicate that geographical and cultural proximity of states encourages migration and thus integration. Data are from the 1961 and 1971 national censuses.

Antibody response to phenolic glycolipid I and Mycobacterium w antigens and its relation to bacterial load in M. leprae-infected mice and leprosy patients.

Twenty-six inbred BALB/cBy mice were infected with live Mycobacterium leprae by injecting 6 x 10(3) bacilli in the hind footpad. Bleeds were collected at monthly intervals. After 6 months, acid-fast bacilli (AFB) were harvested monthly from the footpad of mice. The sera were analysed in enzyme immunoassay for antibodies against phenolic glycolipid I (PGL-I) of M. leprae and antigens of Mycobacterium w (M. w); 21 out of 26 (80.7%) mice demonstrated the presence of antibodies against PGL-I and M. w . Anti-M. w antibodies appeared slightly earlier than did anti-PGL-I antibodies. The titre of anti-M. w antibodies was higher than that of anti-PGL-I antibodies. The mice giving a positive antibody response had more than 7 x 10(5) AFB/footpad. The coefficient of correlation (r) between the number of AFB and antibody titres at the time of harvest was 0.566 for PGL-I and 0.628 for M. w. The value of r for bacterial index and antibody titres in 188 leprosy patients was 0.510 for PGL-I and 0.418 for M. w; these values were statistically significant (P < 0.001). The decrease in bacterial index and antibody titres in treated lepromatous leprosy patients correlated with increase in the duration of chemotherapy. The measurement of anti-PGL-I antibodies of IgM class may serve as an adjunct to skin biopsy and skin-slit smear for serial monitoring of the bacterial load in the course of chemotherapy in leprosy control programmes.

Cholangiographically guided aspiration cytology in the management of malignant biliary obstruction.

Fifteen patients with malignant biliary obstruction were subjected to fine needle aspiration cytology (FNAC) under fluoroscopic guidance. Fourteen (93%) of them were correctly diagnosed to have a malignancy at FNAC. Simultaneous with FNAC or at a later date, all the patients underwent percutaneous biliary drainage with a drop in serum bilirubin from a mean of 23.2 mg/dl to 8.5 mg/dl. Ten patients were subjected to exploratory laparotomy and biopsy at which the diagnosis of FNAC was confirmed in nine of them. The tenth patient with a negative yield at FNAC had a 1.0 cm cholangiocarcinoma. The usefulness of FNAC combined with biliary drainage as an alternative to surgery is highlighted.

Generation and characterization of a human monoclonal antibody against phenolic glycolipid-I of M. leprae.

The development of an Epstein-Barr virus transformed human B-cell line secreting a monoclonal antibody (MoAb), KR2/B5 is described. KR2/B5 is an IgM type of antibody and is highly specific for phenolic glycolipid-I (PGL-I) a component unique to M. leprae. The MoAb appears to be directed against the terminal sugar residue of the immunodominant trisaccharide component of PGL-I.

Serum leucine aminopeptidase estimation: a sensitive prognostic indicator of invasiveness in breast carcinoma.

Serum Leucine Aminopeptidase (LAP) was studied in 25 patients of breast tumors, and compared with 50 age matched controls. Clinical staging was done in 13 cases of breast carcinoma. Serum LAP was increased but not significantly in stage I (P less than 0.1), while in stage II & III, significant increase was observed (P less than 0.05), but a sharp peak was noted in stage IV with metastasis in liver (P less than 0.001). Serum LAP values were correlated with histological sub-types of breast carcinoma. Highest serum LAP values were observed in filtrating duct carcinoma with productive fibrosis (P less than 0.01), followed by medullary carcinoma and the enzyme values were slightly decreased in intraductal carcinomas.

Mean frontal QRS axis and pulmonary artery pressures in rheumatic mitral stenosis.

The relationship between the mean frontal QRS axis calculated from the scalar 12 lead ECG, and the pulmonary artery (PA) pressures obtained by cardiac catheterisation in 64 cases of isolated rheumatic mitral stenosis (MS), was analysed. An overall trend of rightward axis shift with increasing PA pressures was observed; the best correlation was seen with systolic PA pressures (r = 0.51). It was possible to recognise 3 categories of patients: (1) With an axis of 70 degrees and below, systolic PA pressures were below 70 mm Hg in 82.4% of cases and below 80 mm Hg in 94% of cases; diastolic pressures were below 40 mm Hg in 88.2%; mean PA pressures were below 50 mm Hg in 88.2% cases. (2) With an axis of 71 degrees to 100 degrees, systolic PA pressures ranged from 30 to 120 mm Hg, diastolic PA pressures from 12 to 60 mm Hg and mean PA pressures from 19 to 80 mm Hg. (3) With an axis of above 100 degrees, systolic PA pressures were over 70 mm Hg in 95.5% of cases, diastolic PA pressures more than 30 mm Hg in 90.9%, and mean PA pressures more than 45 mm Hg in 90.9% cases. It was therefore possible to predict, with reasonable accuracy, the range of PA pressures in patients with isolated MS, except in those cases with an axis between 71 and 100 degrees.

Fine needle aspiration cytology of intrathoracic lesions--a repraisal.

FNAC was performed in 186 cases. Inadequate material was obtained in 41 cases. A definite diagnosis of malignancy could be made in 120 cases. In five cases a diagnosis "suspicious" of malignancy could be made. No serious complication was encountered with the technique. FNAC is of definite help in diagnosing the intra thoracic lesion whether malignant or inflammatory in nature. It also avoids unnecessary thoracotomy for diagnostic purposes.

Electrophysiological studies (MNCV, H-RL) in full-term newborn babies.

Twenty-five full-term newborns and their mothers constituted study subjects. Mothers were subjected to weight and height measurements, as well as, serum albumin and hemoglobin estimations. Offsprings were subjected to measurements of weight, crown heel length and head circumference along with motor nerve conduction velocity (MNCV) and H-reflex latency (H-RL). The MNCV was found to be significantly lower in growth retarded (FT-IUGR) babies compared to appropriately grown babies (FT-AGA) whereas H-RL was similar in both FT-IUGR and FT-AGA babies.

Anaphylactoid reactions to protamine: an often lethal complication in insulin-dependent diabetic patients undergoing vascular surgery.

Protamine is used routinely at our institution during arterial surgery to reverse the anticoagulant effect of heparin. Adverse fatal reactions to protamine are generally believed to be rare. However, major anaphylactoid reactions occurred in 11 of the last 1150 patients receiving this drug at our institution. Nine of these reactions occurred in 325 insulin-dependent diabetic patients (incidence, 3%), whereas only two occurred in the 825 patients not receiving insulin (incidence, 0.2%) (p less than 0.001). Ten of these reactions occurred within 10 minutes of protamine administration (15 to 35 mg), whereas one reaction occurred immediately after administration of a 5 mg test dose of protamine. Systolic blood pressure fell below 60 mm Hg in all of the 11 patients, and three patients had to be resuscitated with closed-chest massage. Initial treatment with epinephrine and steroids was successful in seven cases. Four patients required further resuscitative measures, including closed-chest massage. However, one of the patients died as a result of ventricular fibrillation resistant to treatment. Ten of the 11 patients, including the patient who died, had significant preexisting cardiac disease; six of the surviving 10 patients (60%) had perioperative myocardial infarctions and three died. Thus the total mortality rate was 36% (4/11). These data support the implication that neutral protamine Hagedorn (NPH) insulin produces an adverse reaction through immunologic presensitization of the patient. These data also show that, in the older vascular surgery population with a high incidence of significant cardiac disease, protamine reactions can be potentially lethal. Thus routine use of protamine should be avoided in diabetic patients receiving insulin.

Ankle and foot fasciotomy: an adjunctive technique to optimize limb salvage after revascularization for acute ischemia.

Some patients with severely ischemic lower limbs continued to have severe ischemic changes in the foot after revascularization procedures and appropriate four leg compartment fasciotomies. Rather than abandon efforts at limb salvage, we performed adjunctive fasciotomies of the ankle and foot in 11 of these patients. These included five cases of acute traumatic superficial femoral or popliteal artery occlusions (three penetrating injuries; two blunt injuries) requiring primary repair or interposition vein graft; three cases of penetrating injury to all infrapopliteal arteries requiring tibiotibial vein bypasses; two cases of iatrogenic thrombosis of the common femoral artery requiring thrombectomy and patch angioplasty; and one case of a traumatically amputated leg requiring replantation. All bypasses remained patent after the distal fasciotomies. Limb salvage and good functional results were achieved in 10 of the 11 patients from 4 to 28 months postoperatively. One patient required a below-knee amputation because of bleeding from an infected graft. Thus fasciotomy of the ankle and foot should be considered during acute revascularizations when a distal bypass occludes without obvious reasons, or when the foot remains ischemic or shows signs of compartment syndrome unrelieved by standard leg fasciotomy.

Treatment of sarcoidosis patients by steroid aerosol: a ten-year prospective study from Eastern India.

In a consecutive series of 113 cases of biopsy - proven sarcoidosis from Eastern India (1972-1986), steroid aerosols were used in 345 episodes, either alone or with one of the non-steroidal anti-inflammatory drugs (and added chloroquin) or with oral steroids. These, being analysed in detail, revealed satisfactory results only when used with other drugs. Used alone, its role is limited due to high cost, absence of systemic action unless used in high dosage, "hand to lung" delivery problem and difficulty of using in children. It is unsuitable in cases with life - threatening symptoms, neurological or cardiac problems. If used judiciously with suitable companion drugs, it can be used very effectively and can replace oral steroids in many patients.
