[Genetic approaches to the study of ischemic heart disease].

Data on the study of the genetics of ischemic heart disease by means of the classical methods (genealogical, population-statistical, and twin) are discussed. The results of a follow-up, conducted for many years, of the condition of close relatives of sick and healthy probands, who were found to be practically healthy during the first examination are discussed. Clinical biochemical examination of the relatives of sick probands is considered a necessary stage in the further clinical study of the genetic factors in ischemic heart disease. The most important problems in the study of the genetics of ischemic heart disease in the clinic are outlined.

[Genetics of ischemic (coronary) heart disease].

On the basis of the medical history and clinical examination of members of families of 133 practically healthy probands and probands suffering from ischemic heart disease and comparison of the frequency of this disease among: (1) husbands and wives and relatives of brother-sisters of probands with ischemic heart disease; (2) husbands and wives, living together for a long time, one of whom had a favourable heredity while the heredity of the other is aggravated by vascular and metabolic diseases; (3) step-borthers and step-sisters born of remarriage of a healthy person to another healthy person in one case and to a sick person in another; (4) persons related in different extent to a sick proband, the authors conclude that genetic factors are very important in the development of coronary atherosclerosis and ischemic heart disease.

[Genetic heterogeneity of ischemic (coronary) heart disease].

Results are presented on a study of the blood coagulation system and some indices of serum lipids and proteins in 133 normal individuals and probands with ischaemic heart disease and their 681 relatives. The examination of the relatives of probands with different types of biochemical disorders revealed a similar biochemical background in the probands and the members of their families. The disorders in blood biochemistry in the probands were most similar in the parents, the children of the probands' siblings, and less distinct in more distant relatives (cousins, nephews and nieces, etc.), biochemical disorders similar to those of the probands being found in young persons (14-16 years old) and reappearing in several generations. The author concludes on the genetic heterogeneity of ischaemic (coronary) heart disease and underlying coronary atherosclerosis.

[The medical-and-genetic training of family physician].

The development of family medicine and promotion of genetic approaches to maintaining the health of population are under discussion in the paper. The necessity of training a new generation of physicians by using special programs is substantiated. The system approach to the training of family doctors must comprise, apart from clinical disciplines, the knowledge of human genetics as of a leading medical-and-biological branch of science. The significance of family-related research and analysis of pedigree (for the sake of accurate diagnosis and prevention of diseases) are demonstrated.
