Stone recurrence after shockwave lithotripsy: possible enhanced crystal deposition in traumatized tissue in rabbit model.

To evaluate the possible traumatizing effect of high-energy shockwaves (HESW) on new stone formation as indicated by crystal deposition in the renal parenchyma, we performed an experimental study in 50 rabbits. During severe oxaluria induced by continuous ethylene glycol (0.75%) administration, animals in the first group (N = 15) received 500 to 1500 shockwaves. Animals in the second group (N = 15) underwent no specific therapy apart from ethylene glycol administration. In a third group of animals (N = 15), only shockwave administration was applied. Sham group animals constituted the last group in our study (N = 5). Three months after shockwave application, tissue sections obtained from treated and untreated kidneys were evaluated histopathologically under light and transmission electron microscopy (TEM) for the presence and degree of crystal deposition in the cortical parenchymal region subjected to HESW. Crystal deposition was evident in the intercellular region and intratubular parts of the parenchyma in animals subjected to HESW application, especially in those receiving relatively high (1000 or 1500) numbers of shockwaves. On the other hand, no crystal formation and deposition was detectable in animals undergoing only ethylene glycol therapy or shockwave administration alone. Sham group animals demonstrated no significant renal histopathology. The traumatic effects of HESW should be evaluated as a factor in new stone formation after SWL.

Agenesis of bladder associated with multiple organ anomalies.

Agenesis of the bladder is an extremely rare congenital anomaly, which involves the development of urogenital sinus. Renal agenesis and some other congenital malformations are often associated. We report a 12-year-old girl with absence of bladder and solitary pelvic ectopic kidney together with some other systemic abnormalities.

Human chorionic gonadotropin in serum and neoplastic tissue from patients with bladder carcinoma.

Serum levels and tissue expression of beta subunit of human chorionic gonadotropin were determined in 25 patients with bladder carcinoma. Elevated gonadotropin levels in serum were observed in 5 (20%) patients and immunoreactive cells were found in 5 (20%) tumours. The correlation was poor between serum level and immunohistochemical gonadotropin status. Only one patient showed both elevated serum level and immunohistochemical gonadotropin expression. Our observations suggest that elevated serum levels of gonadotropin may be an indicator of poor prognosis. However, immunohistochemical expression of gonadotropin did not seem to have a similar impact on prognosis in this series.

Extracorporeal shockwave lithotripsy in anomalous kidneys.

Traditionally, stones in anomalous kidneys have been removed by open or percutaneous surgery. Extracorporeal shockwave lithotripsy (SWL) with the Dornier MPL 9000 lithotripter was performed in seven patients with horseshoe kidneys, four with pelvic ectopic kidneys, and six with malrotated kidneys. Twelve patients (71%) needed repeated treatments. A total of 11 patients (65%) in all the groups were stone free, and four patients had asymptomatic residual fragments no more than 5 mm in diameter. In the remaining two patients, no sign of stone disintegration was observed, and they underwent open surgery. Extracorporeal lithotripsy is the treatment of choice for stones in horseshoe or malrotated kidneys but is not useful for stones in most pelvic kidneys.

Multidisciplinary evaluation of diabetic impotence.

Patients referring to the Urology and/or Endocrinology Departments of Ankara Medical School with complaints of diabetes mellitus (DM) and related complications were evaluated during the last year. A detailed history was obtained and all of the patients were questioned especially about sexual function problems. Following this evaluation, all patients were divided into two main groups, i.e. patients with sexual disorders, and those with normal sexual function. Factors such as BPH, cerebrosclerosis and other important vascular-neurologic pathologies which may play a role in the etiology of impotence were excluded from the study and 38 patients with sexual dysfunction and 15 with normal sexual activities have undergone further evaluation. Following routine blood and urine analyses, serum hormone levels (testosterone, FSH, LH, prolactin) were determined. Penile color-flow doppler analysis, cavernosometry, cavernosography, bulbocavernous reflex latency time and evaluation of somatosensory evoked potentials were performed. Additionally, all patients were evaluated from the psychiatric aspect using the Hamilton depression scale and MMPI questionnaire. The presence of vascular or neurologic pathology in 89.4% of our patients and of both pathologies in 39.4% of the patients, indicated the importance of multifactorial evaluation of diabetic impotence in order to plan a complete and efficient therapy program.

Efficiency of extracorporeal shockwave lithotripsy on calcium-oxalate stones: role of copper, iron, magnesium and zinc concentrations on disintegration of the stones.

Clinical use of extracorporeal shockwave lithotripsy (ESWL) in the noninvasive management of urinary calculi is highly effective and practical. But its results vary in relation to several factors including localization, size and the chemical composition of the stones. Moreover, the anatomy of the kidneys treated is another contributing factor to the success rate of ESWL in upper urinary tract stones. Currently, except cystine and struvite stones, the efficiency of ESWL has been proved and has become the most outstanding therapy for calculi in a great number of stone centers. In this study, the efficiency of ESWL treatment in respect to the concentrations of 4 different trace elements (Cu, Fe, Mg and Zn) in the chemical composition of Ca-oxalate monohydrate stones have been evaluated in 20 patients.

Multiparametric evaluation of testicular atrophy due to varicocele.

63 men with palpable left varicoceles were evaluated by semen analysis, scrotal ultrasonography, Doppler sonography, scrotal contact thermography, and radionuclide scanning. 11 (17.4%) patients had bilateral varicoceles detected by scrotal ultrasonography as compared with an incidence of 3.1% by scrotal palpation. Testicular atrophy was found in 29 (46%) patients by scrotal ultrasonography. This was found to be 39.6% by scrotal palpation alone. In this study using advanced diagnostic procedures, the incidences of bilateral varicocele and testicular atrophy due to varicocele were found to be much higher than when scrotal palpation alone was performed.

Iron, copper, cadmium, zinc and magnesium contents of urinary tract stones and hair from men with stone disease.

Forty-seven urinary tract stones consisting of 5 pure magnesium ammonium phosphate (subgroup 1), 5 mixed calcium oxalate-magnesium ammonium phosphate (subgroup 2), 8 mixed calcium oxalate-calcium phosphate (subgroup 3), 6 mixed calcium oxalate-uric acid (subgroup 4) and 23 pure calcium oxalate stones (subgroup 5) and hair from these patients and control subjects were analyzed for iron, copper, cadmium, zinc and magnesium content by atomic absorption spectroscopy. Mean values +/- SD for each group were determined and statistical correlations were made to establish differences among the stone elements themselves, the hair elements themselves, and the stone and hair elements for group 5 (calcium oxalate stone group). We found significant differences among the element levels of the stones, patient hair and control hair. In particular, hair magnesium and cadmium levels of control and patient groups appear to have promising features for future work concerning urinary tract stone disease.

Emphysematous pyelonephritis with pneumocystitis.

Emphysematous pyelonephritis is an acute infectious process mostly associated with diabetes and obstruction. It is a life-threatening illness and treatment generally involves antibiotic therapy, surgical drainage and nephrectomy. The prognosis is poor regardless of therapy. Herein, we present a case with pneumocystitis. To our knowledge, there have been 54 cases previously reported. There is no doubt that emphysematous pyelonephritis with pneumocystitis as in our case is less frequent.
