Structural and biosynthetic studies on linkage region between poly(galactosylglycerol phosphate) and peptidoglycan in Bacillus coagulans.

The HF treatment of teichoic acid-glycopeptide complexes isolated from lysozyme digests of Bacillus coagulans AHU 1366 cell walls gave a disaccharide, glucosyl beta (1 leads to 4)N-acetylglucosamine, along with dephosphorylated repeating units of the teichoic acid chain, galactosyl alpha (1 leads to 2) glycerol. Mild alkali treatment of the complexes yielded the disaccharide linked to glycopeptide, whereas direct heating of the cell walls at pH 2.5 yielded the same disaccharide linked to teichoic acid. The Smith degradation of the complexes revealed that the galactose residue is a component of backbone chain. Thus it is concluded that this disaccharide is involved in the linkage region between poly(galactosylglycerol phosphate) and peptidoglycan in cell walls. Membrane-catalyzed synthesis of this disaccharide on a lipid followed by transfer of glycerol phosphate from CDP-glycerol to the disaccharide-linked lipid in the absence or in the presence of UDP-galactose also supports this conclusion.

Impalement wounds of the head and chest by reinforced steel bars with recovery: an unusual case report.

A 31-year-old male who sustained completely penetrating impalement wounds of the head and chest by reinforced steel bars in a fall at a ferroconcrete building construction with a miraculous survival is reported. The bars were successfully removed; a surgical mallet was required to loosen the bar impacted in the patient's head. After 12 days he was discharged, and he has returned to construction work and is well 3 years postinjury.

Short course intermittent chemotherapy in childhood tuberculosis.

A total of 130 children diagnosed as having pulmonary and extrapulmonary tuberculosis who received short course intermittent chemotherapy between 1978-1992 were evaluated retrospectively. One hundred and ten children with tuberculosis were treated with isoniazid (10-15 mg/kg, maximum 400 mg), rifampin (10-15 mg/kg, maximum 600 mg), and streptomycin (30 mg/kg, maximum 1 g) daily, for 15 days. Treatment was completed with similar doses of isoniazid and rifampin twice a week for a period of 9 months. Since 1986, 20 children with tuberculosis were being treated with the same regimen but without streptomycin. The majority of patients in these cases had pulmonary tuberculosis (75%), followed by lymph nodes (9%), pleural (7%), bone and joint (5%), miliary (3%), and abdominal tuberculosis (1%). The clinicoradiologic response to treatment was observed to be excellent. Only one case of relapse was detected, which was the case of a patient with lymph node tuberculosis that occurred 18 months after the completion of treatment. No serious adverse drug reaction was observed in any of the cases mentioned. In conclusion, short-course low-dose intermittent chemotherapy is an effective and economical treatment with minimal side effects for pulmonary and extrapulmonary tuberculosis in childhood.

Squamous cell carcinoma of the lower lip.

PURPOSE: Most patients with squamous cell carcinoma of the lower lip present with early disease and follow a rather indolent clinical course. Determinant 5-year survival rates range from 85% to 95%. This study was undertaken in an attempt to gain insight into the cause of failure in those few patients who develop recurrent disease. PATIENTS AND METHODS: A retrospective review was completed on patients treated between 1964 and 1990. Patients were staged according to the American Joint Committee. Patients with no palpable adenopathy had either a unilateral or bilateral suprahyoid dissection performed. Patients with palpable adenopathy underwent radical neck dissection. All patients were followed for evidence of recurrent disease. RESULTS: The records of 92 patients treated surgically for squamous cell carcinoma of the lower lip were available and complete. Palpable adenopathy was present in 38 patients; however, only 8 of these patients (21%) were histologically positive. Of the 54 patients judged to be free of disease, 3 (5.5%) had histologic evidence of metastasis. Overall, the incidence of cervical metastasis was 12%. CONCLUSION: The incidence of cervical metastasis in patients with squamous cell carcinoma of the lip is low; however, these data suggest that the size of the primary tumor does not correlate closely with predicting the incidence of regional lymph node metastases.

Growth patterns, inflammatory cells and prognosis in AIDS-associated Kaposi's sarcoma.

AIDS-associated cutaneous Kaposi's sarcomas (KS) in 35 patients were the subject of a histopathological and immunohistochemical study. Histologically, the tumours were classified as granulation-tissue-like (GR, n = 10), angioma-like (A, n = 11), angiosarcoma-like (AS, n = 9) and as spindle cell type (SP, n = 5). There was a distinct relationship between the main growth patterns of KS and the CD4+ peripheral blood lymphocyte (PBL) count. A decrease in numbers of CD4+ PBL was correlated with an increase in the frequency of AS and SP growth patterns. There were no correlations between growth pattern and concentrations of protein S100+, MT1+, CD45+ and MAK387+ tumour-associated inflammatory cells. When the GR type was grouped together with the A type, and the AS type with the SP type, statistically significant differences emerged for CD45+ and MT1+ tumour-associated cells, and for survival time, respectively. In all these instances, the GR/A group values were higher than those of the AS/SP group. Additionally, linear correlation analysis revealed an inverse relationship between the concentrations of CD4+ PBL and of protein S100+ tumour-associated inflammatory cells within the GR/A group. In contrast to what is observed with certain tumours of HIV-negative patients, this suggests that a high concentration of protein S100+ tumour-associated cells may indicate a worse prognosis in the GR/A group. The concentration of MAK387+ tumour-associated macrophages did not vary as a function of the growth patterns, even when they were grouped together as described above.(ABSTRACT TRUNCATED AT 250 WORDS)

The value of elective neck dissection in treatment of cancer of the tongue.

PURPOSE: The "adequate" therapy of tongue cancer has not yet been determined. The authors report their experience with 58 N(0) patients to elucidate the role of elective neck dissection in surgical treatment of cancer of the tongue. MATERIALS AND METHODS: The files of 58 N(0) patients with tongue cancer were evaluated retrospectively. In every patient, partial glossectomy continuous with neck dissection was the mainstay of the treatment. TNM staging, intraoperative N staging, pathologically confirmed cervical lymph node metastases and their levels, and clinical outcomes (local and regional recurrences) were recorded. The sensitivity and specificity of intraoperative staging was determined. RESULTS: Fifty-four percent (31/58) of the patients presented as T(1), and 26% (15/58) as T(2). The overall occult metastasis rate was 29.3% (17/58). The occult metastasis rate for T(1) and T(2) lesions was 19.4% (6/31) and 26.7% (4/15), respectively. The sensitivity of intraoperative staging was 76.5%, and the specificity was 51.2%. CONCLUSIONS: The rate of occult metastasis to the neck is too high in all tongue cancer cases to take the risk of regional recurrence, and the surgeon can not solely depend on neck palpation for determination of neck metastasis. Radiologic investigations and fine-needle aspiration decrease, but never reduce to zero the rate of false-negative examination. There is an obvious indication for neck dissection, even in early cases.

A rare complication of subclavian vein catheterization: mediastinal haematoma, delayed resorption.

Attempts for right and left subclavian vein catheterizations were unsuccessful in a patient followed for pre-eclampsia. Meanwhile, the patient developed chest pain and dyspnea. Chest radiography revealed mediastinal widening. Later, mediastinal haematoma was diagnosed by thoracic computed tomography. At 28 days follow-up, the mediastinal haematoma was resorbed. A review of literature revealed very few cases of mediastinal haematoma secondary to subclavian vein catheterization. In all these cases the haematoma was resorbed, usually within one week. Mediastinal haematoma is a rare complication and its resolution after several weeks adds to its rarity.

The relationship between time of admittance and complications in paediatric tracheobronchial foreign body aspiration.

PURPOSE: Majority of tracheobronchial foreign body aspirations occur in paediatric age group and may constitute a life hazard. We examined the relationship between the time of admittance and complications in children with tracheobronchial foreign body aspiration. MATERIAL AND METHODS: Sex, age, time of admittance, presenting symptoms, radiological findings, and the nature of the foreign body were reviewed retrospectively in patients aged 16 and under follow-up for tracheobronchial foreign body aspiration between January 1990 and January 2005. Cases were randomly assigned into 6 groups based on admittance times. RESULTS: Within 15-year period, 263 children under the age of 16 were followed-up for tracheobronchial aspiration of foreign body. The most commonly aspirated foreign bodies included sunflower seed, peanut, hazelnut, walnut. While the pathology could be detected radiographically in 80.3% (n : 211) of the cases, in 19.7% (n : 52) radiology was normal. All cases had rigid bronchoscopy under general anaesthesia. In 220 cases foreign body was detected and could successfully be removed in 209 cases. The remaining 11 cases required bronchotomy or pneumotomy to remove the foreign body. Among 679 cases operated for bronchiectasis during the same period, 22 cases (3.2%) had foreign body as the aetiology. No complications were observed when the patients presented to the hospital within the first 24 hours after the aspiration while fever, purulent sputum, haemoptysis and bronchiectasis were noted in those presented later. Most of the complications were medically treated. CONCLUSION: Paying medical attention within the first 24 hours after the aspiration of foreign bodies is critical in order to accomplish a complication-free course. Organic foreign bodies and retention period of 30 days and over, constitute major risk factors in the development of bronchiectasis. It is advisable to perform bronchoscopy in the early stages of all suspected cases to avoid serious complications such as bronchiectasis.

Primary pulmonary lymphoma disguised as asthma.

Primary pulmonary lymphomas (PPL) only constitute 4% of extra-nodal, non-Hodgkin lymphomas (NHL), less than 1% of NHL in general, and between 0.5% and 1% of malignant pulmonary neoplasms. Fifty-eight to 87% of cases of this extremely uncommon disease are low-grade B-cell lymphomas and 11%-19% are high-grade or large B-cell lymphomas. The prognosis for.high-grade or large B-cell lymphomas is worse than for low-grade lymphomas; respiratory and general symptoms are usually present. However asthmatic symptoms are not often found in their clinic. We report the case of a 49-year old woman with resistant asthma clinic in the form of wheezing, dyspnea and non-productive painless cough; associated with an irregular lobulated mass with air bronchograms in the lower lobe of the right lung. Histological diagnosis showed a pattern of high-grade B-cell lymphoma and all asthmatic symptoms disappeared following gross total resection of this lesion.

Hydatid cyst of the pulmonary artery secondary to hepatic hydatid cyst embolism.

A cystic lesion of the liver extending into the inferior vena cava was discovered in a 33-year old female patient suffering from dyspnea, pain and swelling in the legs. Plain chest X-ray was normal. CT and MRI of the thorax showed a cystic lesion within the right pulmonary artery. The liver lesion was treated by a partial cystectomy and omentoplasty, inferior vena cava and bile ducts repair. Through a right thoracotomy, a hydatid cyst was found in the pulmonary artery and enucleated. Although very rare, pulmonary artery hydatidosis may be the cause of an unexplained dyspnea in patients with hydatidosis of the liver.

Familial microtia in four generations with variable expressivity and incomplete penetrance in association with type I syndactyly.

Familial microtia with external ear canal atresia and conductive deafness is rarely reported. Autosomal dominant and recessive inheritance have been suggested depending on various family reports. Cases with other malformations in addition to microtia have been described, although the microtia generally is an isolated finding. Here we report a family with microtia, external auditory canal atresia and conductive deafness in four generations. The mode of inheritance of the disease was autosomal dominant within this family. Also, variable expressivity, incomplete penetrance and generation skipping are evident in the pedigree. Association of microtia with type I syndactyly, which has never been reported previously, was present in the index case.

Cherubism: a radiological and clinical presentation.

Cherubism is the hereditary form of the fibrous dysplasia of the jaws, but it may be seen sporadically as well. The disease has a self-limited nature and is rarely apparent before the age of two. There is no need to interfere surgically with these lesions of the mandible or the maxilla unless the child is severely affected, i.e. the disease deteriorates respiration, deglutition, vision, or the psychiatric makeup of the child due to cosmetic reasons. The clinical presentation and radiological evaluation of these children are so typical that the pediatrician and pediatric otolaryngologist need to be informed about this rarely seen disease. A case of a cherubic child, with his clinical appearance as well as his radiological evaluation, and discussion about the clinical outcome are presented.
